What are Rocketman Tablets Used For?

Rocketman tablets (Generic Sildenafil citrate), manufactured by Alpha Pharma Healthcare Limited, India, in the strengths of 100 mg of Sildenafil, are oral drugs recommended for men for the treatment of erectile dysfunction(also called sexual impotence), a medical condition due to which men are unable to experience a lasting erection during sexual activity. They are also used to treat pulmonary arterial hypertension (PAH). Sildenafil belongs to a group of medicines called phosphodiesterase 5 (PDE5) inhibitors. These medicines prevent an enzyme called phosphodiesterase type-5 from working too quickly. The penis is one of the areas where this enzyme works.
Sildenafil is also used in both men and women to treat the symptoms of pulmonary arterial hypertension. This is a type of high blood pressure that occurs between the heart and the lungs. When hypertension occurs in the lungs, the heart must work harder to pump enough blood through the lungs. Sildenafil works on the PDE5 enzyme in the lungs to relax the blood vessels. This will increase the supply of blood to the lungs and reduce the workload of the heart.

Rocketman Tablets Dosage - How to Take Sildenafil Citrate Tablets?

Rocketman tablets should be used exactly as prescribed by your doctor. Your doctor may suggest a dosage of Rocketman tablets depending on your requirement, body weight, other medical conditions, and current medications. Rocketman pills can be consumed before or after meals.

Missed Dose

Sildenafil Citrate Mechanism of Action – How Does Rocketman Work?

Erectile dysfunction in men is the failure to develop or maintain a good erection upon sexual stimulation. This malfunction occurs due to the narrowing of the blood vessels in the penis.
Sildenafil citrate (Rocketman tablets), works by inhibiting the enzyme phosphodiesterase type 5 (PDE5). PDE5 is responsible for breaking down cyclic guanosine monophosphate (cGMP), a molecule that plays a role in smooth muscle relaxation. By inhibiting PDE5, sildenafil increases cGMP levels, leading to enhanced smooth muscle relaxation, particularly in the corpus cavernosum of the penis, which is essential for achieving an erection.
The physiologic mechanism of erection of the penis involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation. NO then activates the enzyme guanylate cyclase, which results in increased levels of cyclic guanosine monophosphate (cGMP), producing smooth muscle relaxation in the corpus cavernosum and allowing inflow of blood.
Sildenafil has no direct relaxant effect on isolated human corpus cavernosum, but enhances the effect of nitric oxide (NO) by inhibiting phosphodiesterase type 5 (PDE5), which is responsible for degradation of cGMP in the corpus cavernosum. When sexual stimulation causes local release of NO, inhibition of PDE5 by sildenafil causes increased levels of cGMP in the corpus cavernosum, resulting in smooth muscle relaxation and inflow of blood to the corpus cavernosum. Sildenafil at recommended doses has no effect in the absence of sexual stimulation.

Storage

Store Rocketman tablets at controlled room temperature (20°C to 25°C). Excursions are permitted from 15°C to 30°C.

Contraindications

Rocketman tablets are contraindicated in patients with a known hypersensitivity to Sildenafil Citrate or any inactive ingredient present in Rocketman tablets.
Rocketman tablets are also contraindicated in patients with the following conditions:
Administration of Rocketman to patients using nitric oxide donors, such as organic nitrates or organic nitrites in any form. Rocketman tablets was shown to potentiate the hypotensive effect of nitrates.
Administration with Guanylate Cyclase (GC) stimulators, such as Riociguat.
Rocketman (Sildenafil Citrate tablets) should not be used in men for whom sexual activity is inadvisable (e.g. patients with severe cardiovascular disorders such as unstable angina or severe cardiac failure).
Sildenafil is contraindicated in patients who have loss of vision in one eye because of non-arteritic anterior ischaemic optic neuropathy (NAION), regardless of whether this episode was in connection or not with previous PDE5 inhibitor exposure.

Overdose

In studies with healthy volunteers of single doses of Sildenafil (Rocketman tablets) up to 800 mg, adverse reactions were similar to those seen at lower doses but incidence rates and severities were increased.
In cases of overdose, standard supportive measures should be adopted as required. Renal dialysis is not expected to accelerate clearance as sildenafil is highly bound to plasma proteins and it is not eliminated in the urine.

Medical History
Before beginning a course of Rocketman tablets, discuss your personal and family medical history with your doctor. Inform your doctor if you have had heart problems (heart attack, irregular heartbeat, angina, chest pain, narrowing of the aortic valve or heart failure, heart surgery, stroke, low or high blood pressure), a severe loss of body water (dehydration), kidney or liver disorders, penis conditions (such as angulation, fibrosis/scarring, Peyronie's disease), history of painful/prolonged erection (priapism), conditions that may increase the risk of priapism (such as sickle cell anemia, leukemia, multiple myeloma), eye problems (such as retinitis pigmentosa, sudden decreased vision, NAION), bleeding disorders (hemophilia), active stomach ulcers.
Cardiovascular Risk Factors
Prior to initiating any treatment for erectile dysfunction, physicians should consider the cardiovascular status of their patients, since there is a degree of cardiac risk associated with sexual activity. Patients should not use Rocketman tablets if sexual activity is inadvisable due to cardiovascular status.
Priapism
Agents for the treatment of erectile dysfunction, including Sildenafil, should be used with caution in patients with anatomical deformation of the penis (such as angulation, cavernosal fibrosis or Peyronie's disease), or in patients who have conditions which may predispose them to priapism (such as sickle cell anaemia, multiple myeloma or leukaemia). Patients should seek emergency treatment if an erection lasts for more than 4 hours.Use Rocketman tablets with caution in patients predisposed to priapism.
Effects on the Eye
Physicians should advise patients to stop use of all phosphodiesterase type 5 (PDE5) inhibitors, including Sildenafil tablets, and seek medical attention in the event of a sudden loss of vision in one or both eyes. Such an event may be a sign of non-arteritic anterior ischemic optic neuropathy (NAION), a rare condition and a cause of decreased vision including permanent loss of vision, that has been reported rarely post-marketing. Rocketman tablets should be used with caution, and only when the anticipated benefits outweigh the risks, in patients with a history of NAION. Patients with a "crowded" optic disc may also be at an increased risk of NAION.
Hearing Loss
Physicians should advise patients to stop taking PDE5 inhibitors, including sildenafil tablets, and seek prompt medical attention in the event of sudden decrease or loss of hearing.
Concomitant use with other PDE5 Inhibitors:
Concomitant use with other PDE5 inhibitors or other treatments for erectile dysfunction The safety and efficacy of combinations of sildenafil with other PDE5 Inhibitors, or other pulmonary arterial hypertension (PAH) treatments containing sildenafil, or other treatments for erectile dysfunction have not been studied. Therefore the use of such combinations is not recommended.
Risk of Hypotension:
Caution is advised when Rocketman is co-administered with alpha-blockers or anti-hypertensives. Concomitant use may lead to hypotension
Allergy
Please inform your doctor if you have an allergy or have had an allergic reaction in the past to Sildenafil Citrate or any inactive ingredient in Rocketman tablets.
Decreased Blood Pressure:
Decreased blood pressure, syncope, and prolonged erectionmay occur at higher sildenafil exposures. In patients taking strong CYP inhibitors, such as Ritonavir, Sildenafil exposure is increased. Decrease in Sidenafil (Rocketman) dosage is recommended.
Rocketman Interactions
Before staring the course of Rocketman tablets, please inform your doctor of all your current medications including prescription drugs, over the counter medications, mineral supplements, herbal products, and vitamins. Rocketman tablets are known to have drug interactions with certain other medications. Let your doctor know if are using nitroglycerin (Nitrostat, Nitrolingual, Nitro-Dur, Nitro-Bid), isosorbide dinitrate (Dilatrate-SR, Isordil, Sorbitrate), and isosorbide mononitrate (Imdur, ISMO, Monoket), recreational drugs called "poppers" containing amyl or butyl nitrite, alpha blocker medication (such as doxazosin, tamsulosin), azole antifungals (such as itraconazole, ketoconazole), macrolide antibiotics (such as clarithromycin, erythromycin), HIV protease inhibitors (such as ritonavir, saquinavir), hepatitis C virus protease inhibitors (such as boceprevir, telaprevir), rifampin, other erectile dysfunction or pulmonary hypertension drugs (such as tadalafil, vardenafil).
Rocketman Pills and Alcohol
Consumption of excess alcohol can reduce your ability to experience an erection. You may lose the advantage of Rocketman tablets in such situations. Alcohol consumption can also increase the risk of certain side effects.
Rocketman Tablets - Driving and Other Activities
Rocketman tablets may make you light-headed or impair your vision. Avoid activities such as driving, using machinery, or performing any activity that requires complete vigilance or perfect vision until you are sure about your safety.

If you plan to have surgery or any dental procedure, inform your doctor or dentists about your course of Rocketman tablets. In case of an emergency medical treatment, please inform the practitioner about your course of Rocketman tablets and the details of your last dose.

Side Effects of Sildenafil Citrate (Rocketman Tablets)

Most common Sildenafil Citrate side effects with an incidence rate of 2% or more include headache, flushing, dyspepsia, abnormal vision, nasal congestion, back pain, myalgia, nausea, dizziness and rash. The following events occurred in less than 2% of patients in controlled clinical trials; a causal relationship to Sildenafil Citrate Tablets (Rocketman pills) is uncertain. Reported events include those with a plausible relation to drug use; omitted are minor events and reports too imprecise to be meaningful:

• Body as a Whole: face edema, photosensitivity reaction, shock, asthenia, pain, chills, accidental fall, abdominal pain, allergic reaction, chest pain, accidental injury.

• Musculoskeletal: arthritis, arthrosis, myalgia, tendon rupture, tenosynovitis, bone pain, myasthenia, synovitis.
• Nervous: ataxia, hypertonia, neuralgia, neuropathy, paresthesia, tremor, vertigo, depression, insomnia, somnolence, abnormal dreams, reflexes decreased, hypesthesia.
• Respiratory: asthma, dyspnea, laryngitis, pharyngitis, sinusitis, bronchitis, sputum increased, cough increased.
• Skin and Appendages: urticaria, herpes simplex, pruritus, sweating, skin ulcer, contact dermatitis, exfoliative dermatitis.
• Special Senses: sudden decrease or loss of hearing, mydriasis, conjunctivitis, photophobia, tinnitus, eye pain, ear pain, eye hemorrhage, cataract, dry eyes.
• Urogenital: cystitis, nocturia, urinary frequency, breast enlargement, urinary incontinence, abnormal ejaculation, genital edema and anorgasmia.

Rocketman tablets are not intended for consumption by women. There are no adequate and well-controlled studies of Sildenafil in pregnant women to inform any drug-associated risks for adverse developmental outcomes.
