Duolin Rotacaps (Ipratropium Bromide and Levosalbutamol Inhalation Powder) - Product Information

Duolin Rotacaps contain a combination of two active ingredients called Ipratropium Bromide and Levosalbutamol. Levosalbutamol is a short-acting beta2-adrenergic agonist which eases the symptoms of inflammation in the respiratory system of the body. Ipratropium Bromide is an anticholinergic medication which acts as a bronchodilator to relax the smooth muscles located in the airways to aid the passage of air through the lungs.
Duolin Rotacaps are used by patients diagnosed with different bronchospasm ailments such as bronchitis, emphysema, or COPD (chronic obstructive pulmonary disease) and are already using another bronchodilator for the treatment of symptoms such as tightness of the chest, wheezing and shortness of breath.
The contents of Duolin Rotacaps should only be administered for inhalation and not for oral purposes. When the contents of Duolin Rotacaps are sprayed from the inhaler into the mouth, they are carried directly into the airways. Duolin Rotacaps are used to prevent the occurrence of breathing difficulties and also to reduce the frequency of such incidences.

Ltd. in the strength of 40 mcg of Ipratropium Bromide and 200 mcg of Levosalbutamol. Duolin Rotacaps can only be administered for oral inhalation through the Generic HandiHaler inhalation device.
Duolin Rotacaps are used as therapy in patients suffering from COPD (chronic obstructive pulmonary disease) and related breathing difficulties such as bronchitis and emphysema. Duolin Rotacaps are used by patients who are already using another bronchodilator for the treatment of general symptoms such as shortness of breath, tightness of the chest and wheezing.
Duolin Rotacaps are a combination of short-acting beta2-agonist bronchodilator and an anticholinergic bronchodilator which exerts its therapeutic effect synergistically by expanding the smooth muscles located in the airways to permit relaxed breathing, reducing bronchospasm (contraction of the airways) and enhancing air flow into the lungs.

Duolin Rotacaps Dosage - How to Use Duolin Rotacaps?

You should use Duolin Rotacaps exactly as suggested by your doctor. The starting dose of Duolin Rotacaps in patients is dependent on the severity of their asthmatic condition. The recommended dosage of Duolin Rotacaps is two puffs of Duolin Rotacaps, administered four times a day. The therapeutic effect of one puff of Duolin Rotacaps remains for a period of 4 to 5 hours. The maximum recommended dosage of Duolin Rotacaps is 12 puffs a day.
The contents of Duolin Rotacaps should not be swallowed and should strictly only be inhaled using the special Cipla Rotahaler. While taking out one Duolin Rotacap from the pack, tear out the foil and tap the pack to let the capsule out without touching it with your hands. Avoid cutting the foil or using any sharp object to take out the capsule. Do not take out a Duolin Rotacap from its intact pack until you are ready to use it. If other capsules in the pack come in contact with the atmospherewhen you are removing a Duolin Rotacap capsule for use, it must be thrown away. Do not preserve the capsule for future usage.
After removing Duolin Rotacap from the pack, it must instantly be placed in a special inhaler device. The device will crush the capsule so that the content inside the capsule can be easily inhaled into the lungs through the mouthpiece. While using the inhalation device, make sure that you exhale gently and deeply. Avoid breathing into the mouthpiece of the inhaler while administering the Duolin Rotacap.
Place the mouthpiece of the inhaler between your lips and try to maintain your tongue in a flat and steady position, while keeping your head straight. When you take a slow and deep breath, you should hear or feel the capsule pulsate inside the inhaler. You can hold your breath until you start to feel breathless and then slowly breathe out while keeping the lips pressed together. To ensure the usage of the whole medication from the capsule, inhale from the mouthpiece again for a second time following the same steps as before. Consult your doctor or pharmacist if you are uncertain about how to use the special inhaler device or inhale the Duolin Rotacap.
You should clean the inhaler device at least once every month by referring to the instructions in the patient leaflet. Each inhaler device may last for a period of one year, after which it should be replaced. Use Duolin Rotacaps at around the same time every day, so that you will remember to use it regularly.
Duolin Rotacaps should be used on a regular basis without missing any doses to get the maximum therapeutic benefit from it. Do not discontinue treatment with Duolin Rotacaps abruptly because it may result in the development of unpleasant withdrawal symptoms.

Missed Dose

If you miss your regular dose of Duolin Rotacaps, take it as soon as you remember. You can continue your regular schedule the next day.

Duolin Rotacaps Mechanism of Action – How Does Ipratropium Bromide and Levosalbutamol Inhalation Powder Work?

Duolin Inhaler contains a combination of two bronchodilators, Ipratropium Bromide and Levosalbutamol, which work synergistically to treat the characteristic symptoms of asthma and chronic obstructive pulmonary disease (COPD).
Levosalbutamol belongs to a class of medications known as selective short-acting beta2-agonist. Levosalbutamol exerts its mechanism of action on all the airways of the lungs, beginning from the trachea (windpipe) to the terminal bronchioles (small airways) and acts as a bronchodilator by relaxing the smooth muscle in the airway walls to expand the airways and to improve the airflow to the lungs.
Ipratropium, on the other hand, is an anticholinergic medication that works on particular receptors (muscarinic receptors) located in the bronchioles of the lung and impedes the contact of acetylcholine (a neurotransmitter or chemical messenger of the nervous system) with muscarinic receptors.
This contact inhibition obstructs the contraction of the airway wall triggered by acetylcholine. The combined therapeutic effect of the two medications which act together through different mechanisms effectively reduces bronchospasm (contraction of the airways) to a higher degree than would be achieved by each drug alone and enhances air flow into the lungs, easing symptoms of COPD such as tightness of the chest, wheezing and shortness of breath.

Storage Instructions for Duolin Rotacaps

Store the pack of Duolin Rotacaps at controlled room temperature (20°C to 25°C), protected from heat, moisture, and sunlight. Excursions permitted to 15°C to 30°C.
Retain the original packing of the medication and do not consume Duolin Rotacaps (Ipratropium Bromide and Levosalbutamol Inhalation Powder) beyond its expiration date printed on the canister. When you are not using your inhaler, always cover the mouthpiece with the cap provided to keep the inhaler protected from germs.
Keep the pack of Ipratropium Bromide and Levosalbutamol Inhalation Powder away from the reach of children and pets. Do not discard Ipratropium Bromide and Levosalbutamol Inhalation Powder along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Duolin Rotacaps are contraindicated in patients with a known hypersensitivity to Ipratropium Bromide or Levosalbutamol or any inactive ingredient present in Duolin Rotacaps.

Overdose

Levosalbutamol
The expected symptoms with overdosage are those of excessive beta-adrenergic receptor stimulation and/or occurrence or exaggeration of any of the symptoms listed under adverse reactions, e.g., seizures, angina, hypertension or hypotension, tachycardia with rates up to 200 beats/minute, arrhythmias, nervousness, headache, tremor, dry mouth, palpitation, nausea, dizziness, fatigue, malaise, and sleeplessness. Hypokalemia may also occur. As with all sympathomimetic medications, cardiac arrest and, even, death may be associated with the abuse of levosalbutamol.
Treatment consists of discontinuation of levosalbutamol together with appropriate symptomatic therapy. The judicious use of a cardioselective beta-receptor blocker may be considered, bearing in mind that such medication can produce bronchospasm. There is insufficient evidence to determine if dialysis is beneficial in treating overdosage of levosalbutamol.
Ipratropium Bromide
Dose of ipratropium bromide up to 1.2 mg (60 puffs) have been administered by inhalation without the appearance of serious systemic anticholinergic effects. Minor systemic manifestations of anticholinergic action, including dry mouth, visual accommodation disorder and increase of heart rate may occur.
Should signs of serious anticholinergic toxicity appear, cholinesterase inhibitors may be considered.

Medical History
Before beginning the treatment with Ipratropium Bromide and Levosalbutamol Inhalation Powder, discuss your personal and family medical history with your doctor. Inform your doctor if you have heart disease, high blood pressure, or congestive heart failure, epilepsy, diabetes, overactive thyroid, liver or kidney disease, glaucoma, enlarged prostate or problems with urination.
Allergy

Ipratropium Bromide and Levosalbutamol Inhalation Powder are known to have drug interactions with certain other medications. Please inform your doctor if you are taking other beta-agonists like pseudoephidrine in cold medications, anticholinergics like atropine; diuretics like furosemide or hydrochlorothiazide; MAOIs like phenelzine; tricyclic antidepressants (TCAs) like amitriptyline; beta-blockers like atenolol.
Live Vaccinations, Surgery and Regular Medical Tests
If you plan to have live vaccinations, surgery or any dental procedure, inform your doctor, anesthetist, or dentist about your course of Duolin Rotacaps. In case of an emergency medical treatment, please inform the practitioner about your course of Ipratropium Bromide and Levosalbutamol and the details of your last dose.

Side Effects of Duolin Rotacaps

The following side effects have been reported during the course of treatment with Ipratropium Bromide and Levosalbutamol Inhalation Powder:

  • Central Nervous System- headache, tremor (shaky or tense feeling), blurred vision
  • Cardiovascular System - tachycardia (rapid heartbeat)
  • Gastrointestinal System- dryness of mouth and palpitations, nausea
  • Respiratory System- pharyngitis, upper respiratory tract infection, dysphonia, pneumonia, throat irritation, viral respiratory infections, cough
  • Musculoskeletal System- back pain, pain in joints, muscle pain, muscle cramps
  • Hypersensitivity- rash, hives, itching/swelling (especially of the face/tongue/throat), severe dizziness, trouble breathing, tightness in the chest, skin redness

Levosalbutamol and Ipratropium Bromide combination has been classified by the US FDA as Pregnancy Category C. There are no adequate and well-controlled studies of Levosalbutamol and Ipratropium Bromide combination inhaler in pregnant women.
Because animal reproduction studies are not always predictive of human response, Duolin Rotacaps should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
