Uses of Budecort Rotacaps (Budesonide Inhalation Powder)
Budecort Rotacaps (Budesonide Inhalation Powder) is used to help prevent the symptoms of asthma. When used regularly every day, inhaled budesonide decreases the number and severity of asthma attacks. However, it will not relieve an asthma attack that has already started.
Budesonide is a corticosteroid or steroid (cortisone-like medicine). It works by preventing inflammation (swelling) in the lungs, which makes the asthma attack less severe. Inhaled Budesonide may be used with other asthma medicines such as bronchodilators, which are also used to open up narrowed breathing passages in the lungs.
Budecort Rotacaps - Dosage
Budecort Rotacaps should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Budecort Rotacaps depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Adults (including elderly) and children over 12 years
200-1,600 mcg daily, in divided doses.
200 mcg twice daily, in the morning and in the evening. During periods of severe asthma the daily dosage can be increased up to 1600 mcg.
In patients whose asthma is well controlled, the daily dose may be reduced below 400 mcg but should not go below 200 mcg. Patients should be maintained on the lowest dose that will effectively control symptoms.
Children (5 – 12 years)
200-800 mcg daily, in divided doses.
The dose should be reduced to the minimum needed to maintain good asthma control.
The patient should be transferred to once daily dosing at the same equivalent total daily dose; the drug and method of delivery should be considered. The dose should subsequently be reduced to the minimum needed to maintain good asthma control.
Patients should be instructed to take the once daily dose in the evening. It is important that the dose is taken consistently and at a similar time each evening.
Budecort Rotacaps should only be inhaled with the Cipla Rotahaler device. Budecort Rotacaps must not be swallowed.
Budecort Rotacaps work effectively if the amount of Budesonide is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue using Budecort Rotacaps without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your administer Budecort rotacaps dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.
Mechanism of Action

Contraindications
Budecort Rotacaps (Budesonide Inhalation Powder) are contraindicated in patients with a hypersensitivity to Budesonide or any inactive ingredient present in Budecort Rotacaps.
Overdose
Acute overdosage with Budesonide, even in excessive doses is not expected to be a clinical problem. The only harmful effect that follows inhalation of large amounts of the drug over a short period is suppression of the HPA axis function. No special emergency action needs to be taken.
Treatment with Budecort Rotacaps should be continued at the recommended dose to control the asthma.
Warnings and Precautions
Localized infections:
Candida albicans infection of the mouth and throat may occur. Monitor patients periodically for signs of adverse effects on the oral cavity. Advise patients to rinse the mouth following inhalation.
Deterioration of asthma or acute episodes:
Budecort Rotacaps (Budesonide Inhalation Powder) should not be used for relief of acute symptoms. Patients require immediate re-evaluation during rapidly deteriorating asthma. Patients should be instructed to contact their physician immediately if episodes of asthma not responsive to their usual doses of bronchodilators occur during the course of treatment with Budesonide inhalation powder.
Hypersensitivity reactions:
Anaphylaxis, rash, contact dermatitis, urticaria, angioedema, and bronchospasm have been reported with use of Budecort Rotacaps. Discontinue Budecort Rotacaps if such reactions occur.
Immunosuppression:
Potential worsening of infections (e.g., existing tuberculosis, fungal, bacterial, viral, or parasitic infection; or ocular herpes simplex). Chicken pox and measles, for example, can have a more serious or even fatal course in susceptible children or adults using corticosteroids. In children or adults who have not had these diseases, or been properly immunized, particular care should be taken to avoid exposure. Use Budecort Rotacaps with caution in patients with these infections.
Transferring patients from systemic corticosteroids:
Risk of impaired adrenal function when transferring from oral steroids. Taper patients slowly from systemic corticosteroids if transferring to Budecort Rotacaps .
Hypercorticism and adrenal suppression:
Budesonide inhalation suspension will often help control asthma symptoms with less suppression of HPA function than therapeutically equivalent oral doses of prednisone. . It is possible that systemic corticosteroid effects such as hypercorticism, and adrenal suppression may occur with very high dosages or at the regular dosage in susceptible individuals. If such changes occur, reduce Budecort Rotacaps dose slowly.
Reduction in bone mineral density with long term administration
Decreases in bone mineral density (BMD) have been observed with long-term administration of products containing inhaled corticosteroids. Monitor patients with major risk factors for decreased bone mineral content.
Effects on growth:
Orally inhaled corticosteroids, including Budesonide, may cause a reduction in growth velocity when administered to pediatric patients. Monitor growth of pediatric patients.
Glaucoma and cataracts:
Glaucoma, increased intraocular pressure, and cataracts have been reported following the long-term administration of inhaled corticosteroids, including Budesonide. Close monitoring is warranted.
Paradoxical bronchospasm:
As with other inhaled asthma medications, bronchospasm, with an immediate increase in wheezing, may occur after dosing. If acute bronchospasm occurs following dosing with Budesonide inhalation powder, it should be treated immediately with a fast-acting inhaled bronchodilator. Discontinue Budecort Rotacaps (Budesonide Inhalation Powder) and institute alternative therapy if paradoxical bronchospasm occurs.
Eosinophilic conditions and Churg-Strauss:
In rare cases, patients on inhaled corticosteroids may present with systemic eosinophilic conditions. Be alert to eosinophilic conditions.
Side Effects of Budecort Rotacaps
Mild irritation in the throat, Candida infection in the oropharynx, hoarseness, coughing, nasopharyngitis, nasal congestion, pharyngitis, allergic rhinitis, viral upper respiratory tract infection, nausea, viral gastroenteritis, otitis media, oral candidiasis, respiratory infection, sinusitis, headache, pain, back pain, fever, neck pain, syncope, abdominal pain, dry mouth, vomiting, weight gain, fracture, myalgia, hypertonia, migraine, ecchymosis, insomnia, infection, taste perversion, voice alteration, asthenia, dyspepsia, arthralgia, increased cough, rhinitis were more common than placebo in the clinical trials with budesonide.

Results from a large prospective epidemiological study and from worldwide post marketing experience indicate no increased teratogenic risk associated with the use of inhaled Budesonide. In animal studies, glucocorticosteroids have been shown to induce malformations.
