Vidalista is a phosphodiesterase 5 (PDE5) inhibitor used for the treatment of erectile dysfunction (ED), the signs and symptoms of benign prostatic hyperplasia (BPH) and ED and the signs and symptoms of BPH (ED/BPH) in men. Active Pharmaceutical Ingredient

The active ingredient present in Vidalista tablets is Tadalafil. Each film-coated Vidalista tablet contains 2.5 mg, 5 mg , 10 mg or 20 mg of Tadalafil.

Uses of Tadalafil Tablets

Tadalafil tablets (Vidalista 5 mg and 10 mg Tablets) are a phosphodiesterase 5 (PDE5) inhibitor used for the treatment of erectile dysfunction (ED), the signs and symptoms of benign prostatic hyperplasia (BPH) and ED and the signs and symptoms of BPH (ED/BPH). Tadalafil tablets are also indicated for the treatment of pulmonary arterial hypertension (PAH) (WHO Group 1) to improve exercise ability.

Vidalista Tablets - Dosage

Vidalista tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Vidalista tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Erectile Dysfunction:
Vidalista for Use as Needed

• The recommended starting dose of Vidalista for use as needed in most patients is 10 mg, taken prior to anticipated sexual activity.
• The dose may be increased to 20 mg or decreased to 5 mg, based on individual efficacy and tolerability. The maximum recommended dosing frequency is once per day in most patients.
• Vidalista for use as needed was shown to improve erectile function compared to placebo up to 36 hours following dosing.

Vidalista for Once Daily Use

• The recommended starting dose of Vidalista for once daily use is 2.5 mg, taken at approximately the same time every day, without regard to timing of sexual activity.
• The Vidalista dose for once daily use may be increased to 5 mg, based on individual efficacy and tolerability.

Treatment of BPH and ED/BPH:
The recommended dose of Vidalista Once-a-Day for treatment of BPH and ED/BPH is 5 mg per day, taken at approximately the same time each day, without regard to food and in men with ED, without regard to timing of sexual activity.
Pulmonary Arterial Hypertension:
The recommended dose of Tadalafil tablets is 40 mg (two 20 mg tablets) taken once daily with or without food. Dividing the dose (40 mg) over the course of the day is not recommended.
Vidalista tablets work effectively if the amount of Tadalafil is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Vidalista tablets at about the same time every day, to get the maximum benefit of the medication. Tadalafil Mechanism of Action – How do Vidalista tablets Work?

Tadalafil is a selective, reversible inhibitor of cyclic guanosine monophosphate (cGMP)-specific phosphodiesterase type 5 (PDE5). When sexual stimulation causes the local release of nitric oxide, inhibition of PDE5 by tadalafil produces increased levels of cGMP in the corpus cavernosum. This results in smooth muscle relaxation and inflow of blood into the penile tissues, thereby producing an erection. Tadalafil has no effect in the treatment of erectile dysfunction in the absence of sexual stimulation.
The effect of PDE5 inhibition on cGMP concentration in the corpus cavernosum is also observed in the smooth muscle of the prostate, the bladder and their vascular supply. The resulting vascular relaxation increases blood perfusion which may be the mechanism by which symptoms of benign prostatic hyperplasia are reduced. These vascular effects may be complemented by inhibition of bladder afferent nerve activity and smooth muscle relaxation of the prostate and bladder.
In addition to its effects on the corpus cavernosum, Tadalafil's inhibition of PDE5 in the pulmonary vasculature leads to vasodilation, reducing pulmonary arterial pressure. This makes tadalafil effective in treating pulmonary arterial hypertension (PAH).

Storage Instructions for Vidalista Tablets

Store Vidalista Tablets (Generic Tadalafil 2.5 mg, 5 mg , 10 mg and 20 mg) at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Vidalista Tablets past its expiration date printed on the blister pack. Keep the pack of away from the reach of children and pets. Contraindications

Vidalista tablets are contraindicated in patients with a hypersensitivity to Tadalafil or any inactive ingredient present in tablets. Generic Tadalafil tablets are also contraindicated in the following cases:

• Administration of Vidalista to patients who are using any form of organic nitrate is contraindicated.
• Vidalista must not be used in men with cardiac disease for whom sexual activity is inadvisable. Physicians should consider the potential cardiac risk of sexual activity in patients with pre-existing cardiovascular disease.
• Vidalista is contraindicated in patients who have loss of vision in one eye because of non-arteritic anterior ischaemic optic neuropathy (NAION), regardless of whether this episode was in connection or not with previous PDE5 inhibitor exposure
• The co-administration of PDE5 inhibitors, including Vidalista (Tadalafil), with guanylate cyclase stimulators, such as riociguat, is contraindicated as it may potentially lead to symptomatic hypotension

The following groups of patients with cardiovascular disease were not included in clinical trials and the use of Tadalafil is therefore contraindicated:

• patients with myocardial infarction within the last 90 days,
• patients with unstable angina or angina occurring during sexual intercourse,
• patients with New York Heart Association Class 2 or greater heart failure in the last 6 months,
• patients with uncontrolled arrhythmias, hypotension (<90/50 mm Hg), or uncontrolled hypertension,
• patients with a stroke within the last 6 months

Overdose

Single doses of up to 500 mg of Tadalafil have been given to healthy subjects, and multiple daily doses up to 100 mg have been given to patients. Adverse events were similar to those seen at lower doses.
In cases of overdose, standard supportive measures should be adopted, as required. Haemodialysis contributes negligibly to Tadalafil elimination.

Warnings and Precautions

• Medical History:
  A medical history and physical examination should be undertaken to diagnose erectile dysfunction or benign prostatic hyperplasia and determine potential underlying causes, before pharmacological treatment is considered.
• Cardiovascular:
  Patients should not use Vidalista if sex is inadvisable due to cardiovascular status. Serious cardiovascular events, including myocardial infarction, sudden cardiac death, unstable angina pectoris, ventricular arrhythmia, stroke, transient ischaemic attacks, chest pain, palpitations and tachycardia, have been reported either post marketing and/or in clinical trials.
• Vision
  Patients should stop Vidalista and seek medical care if a sudden loss of vision occurs in one or both eyes, which could be a sign of Non Arteritic Ischemic Optic Neuropathy (NAION). Discuss increased risk of NAION in patients with history of NAION
• Decreased Or Sudden Hearing Loss
  Patients should stop Vidalista and seek prompt medical attention in the event of sudden decrease or loss of hearing
• Priapism And Anatomical Deformation Of The Penis
  Patients should seek emergency treatment if an erection lasts for more than 4 hours. Use Vidalista with caution in patients predisposed to priapism
• Renal And Hepatic Impairment:
  Due to increased tadalafil exposure (AUC), limited clinical experience and the lack of ability to influence clearance by dialysis, once-a-day dosing of Tadalafil is not recommended in patients with severe renal impairment.
• Use with CYP3A4 Inhibitors:
  If taking potent inhibitors of CYP3A4, dose should be adjusted: Vidalista for use as needed: =10 mg every 72 hours. For once daily use: dose not to exceed 2.5 mg
• Tadalafil Tablets And Other Treatments For Erectile Dysfunction:
  The safety and efficacy of combinations of tadalafil and other PDE5 inhibitors or other treatments for erectile dysfunction have not been studied. The patients should be informed not to take tadalafil in such combinations.
  Lactose
  Tadalafil 5 mg film-coated tablet contains lactose. Patients with rare hereditary problems of galactose intolerance, total lactase deficiency or glucose-galactose malabsorption should not take this medicinal product.


• Alcohol:
  Use of Vidalista with alpha blockers, antihypertensives or substantial amounts of alcohol (5 units or greater) may lead to hypotension.
• Combination With Alpha Blockers:
  Vidalista is not recommended in combination with alpha blockers for the treatment of BPH because efficacy of the combination has not been adequately studied and because of the risk of blood pressure lowering. Caution is advised when Vidalista is used as a treatment for ED in men taking alpha blockers.
• Worsening Pulmonary Vascular Occlusive Disease:
  Pulmonary vasodilators may significantly worsen the cardiovascular status of patients with pulmonary veno-occlusive disease (PVOD). Since there are no clinical data on administration of tadalafil to patients with veno-occlusive disease, administration of tadalafil to such patients is not recommended.
• Vidalista for BPH:
  Prior to initiating treatment with Vidalista for BPH, consideration should be given to other urological conditions that may cause similar symptoms

Effects On Ability To Drive And Use Machines

Tadalafil (Vidalista 2.5 mg, 5 mg , 10 mg and 20 mg Tablets) has negligible influence on the ability to drive or use machines.
Although the frequency of reports of dizziness in placebo and tadalafil arms in clinical trials was similar, patients should be aware of how they react to tadalafil before driving or using machines.

• Skin and Appendages - pruritus, rash, sweating
• Ophthalmologic - blurred vision, changes in color vision, conjunctivitis (including conjunctival hyperemia), eye pain, lacrimation increase, swelling of eyelids
• Otologic - sudden decrease or loss of hearing, tinnitus
• Urogenital - erection increased, spontaneous penile erection

Tadalafil Tablets During Pregnancy and Breastfeeding

Tadalafil has been classified by the US FDA as Category B.
There are limited data from the use of Tadalafil ( Vidalista 2.5 mg, 5 mg , 10 mg and 20 mg) in pregnant women. Animal studies do not indicate direct or indirect harmful effects with respect to pregnancy, embryonal/foetal development, parturition or postnatal development. As a precautionary measure, it is preferable to avoid the use of Tadalafil during pregnancy.
