Active Pharmaceutical Ingredient
The active ingredient present in Megalis tablets is Tadalafil. Each Megalis tablet contains either 10 mg or 20 mg of Tadalafil.

What are Megalis Tablets Used For?
Tadalafil Tablets for Erectile Dysfunction
Tadalafil tablets are used for the treatment of impotence in men. This condition is medically identified as erectile dysfunction. Tadalafil tablets enhance the flow of blood to the penis during sexual stimulation. This increase in blood flow helps in achieving a lasting erection during sexual stimulation. The effects of Tadalafil tablets can last up to 36 hours.
Tadalafil Tablets for Benign Prostatic Hyperplasia (BPH)

Dosage

Megalis tablets can be administered before or after food. Megalis Mechanism of Action – How Does Tadalafil Work?
Erection during sexual stimulation can be easily attained when there is increased blood circulation in the penis. Increased blood circulation takes place due to the increase in the size of blood vessels that supply the blood to the penis. Simultaneously, the blood vessels that take out blood from the penis shrink in size, which result in lesser removal of blood from the penis.
When an erection occurs during sexual stimulation, it results in the production and release of nitric oxide in the penis. The nitric oxide activates an enzyme called guanylate cyclase to produce cyclic guanosine monophosphate (cGMP). cGMP mainly controls the increase and decrease of the size of blood vessels which supply and take away blood from the penis, resulting in a lasting erection.
An enzyme called phosphodiesterase-5 suppresses the activity of cGMP. When there is a deficiency of cGMP in the penis, the blood vessels return to their normal size, resulting in a decreased supply of blood to the penis. This situation leads to a termination of penis erection.
Tadalafil tablets belong to a group of drugs called phosphodiesterase-5 (PDE5) inhibitors. Other drugs in this group include Sildenafil (Viagra) and Vardenafil (Levitra). Tadalafil tablets inhibit the activity of the phosphodiesterase-5 enzyme. During Tadalafil treatment, phosphodiesterase-5 enzyme are no longer available to suppress cGMP. The presence of cGMP leads to an increased blood flow, resulting in an extended enlargement of the penis.
The mechanism of action of Tadalafil tablets to cure the symptoms of BPH is not still unknown. Phosphodiesterase-5 enzyme is present in the muscles of the bladder and the prostate. It has been suggested that the passage of urine can be made easier by bringing about the relaxation of these muscles. For example, the reduction of pressure in the muscle surrounding the opening to the urethra (the body part that controls the flow of urine from the bladder) may result in an easier flow of urine.
Storage Instructions for Megalis Tablets
Store Tadalafil tablets (Megalis) at controlled room temperature (20°C to 25°C). Excursions are permitted from 15°C to 30°C.

Contraindications
The use of Megalis tablets are contraindicated in patients with chest pain or heart problems who are currently using organic nitrates, nitrates and nitric oxide donors such as nitroglycerin (Nitrostat, Nitrolingual, Nitro-Dur, Nitro-Bid, Minitran, Deponit, Transderm-Nitro), isosorbide dinitrate (Dilatrate-SR, Isordil, Sorbitrate), isosorbide mononitrate (Imdur, ISMO, Monoket), or recreational drugs such as amyl nitrate or nitrite ("poppers"). The concomitant administration of Megalis tablets with a nitrate drug can result in a severe decrease in blood pressure, leading to fainting, stroke, or heart attack.
Megalis tablets are also contraindicated in men with cardiac disease for whom sexual activity is inadvisable; recent MI (within 90 days) or stroke (within last 6 months), hypotension (<90/50 mm Hg); unstable angina, heart failure, uncontrolled arrhythmias or hypertension; known hypersensitivity to Tadalafil or other inactive ingredients present in Megalis tablets.
Overdose
Single doses up to 500 mg have been given to healthy subjects, and multiple daily doses up to 100 mg have been given to patients. Adverse events were similar to those seen at lower doses.
In cases of overdose, standard supportive measures should be adopted as required. Hemodialysis contributes negligibly to tadalafil elimination.
Warnings and Precautions to be taken when using Tadalafil Tablets
Medical History
Before beginning the course of Tadalafil tablets, let your doctor know about your personal and family medical history. Inform your doctor if have or have had heart disease or heart rhythm problems; a heart attack (within the past 90 days); stroke or congestive heart failure (within the past 6 months); angina (chest pain); high or low blood pressure; liver disease; kidney disease (or if you are on dialysis); diabetes or thyroid problems; severe bladder infection; a blood cell disorder (sickle cell anemia, multiple myeloma, or leukemia); dehydration; a bleeding disorder (hemophilia); stomach ulcer; retinitis pigmentosa (an inherited condition of the eye), sudden decreased vision, NAION; painful/prolonged erection (priapism); a physical deformity of the penis (angulation, fibrosis/scarring, Peyronie's disease); or if you have been advised to abstain from sexual intercourse for health reasons (heart problems).
Tadalafil Interactions

Allergic Reactions

Tadalafil and Alcohol
The simultaneous administration of alcohol along with Tadalafil tablets increases the probability of orthostatic symptoms such as increased heart rate, decreased in blood pressure, dizziness, and headache.
Driving and Other Activities

Side Effects of Tadalafil Tablets
The following side effects have been reported during the course of Tadalafil tablets (Megalis):
- Cardiovascular System- Flushing, high blood pressure, angina pectoris, chest pain, hypotension, postural hypotension, MI, tachycardia, heart attack, palpitations, syncope, irregular heart rate, stroke and sudden cardiac death.
- Central Nervous System- Headache, asthenia, weakness, dizziness, fatigue, sensory loss, insomnia, mood changes, paresthesia, somnolence, tingling, drowsiness, vertigo, migraine, seizure and seizure recurrence, transient global amnesia, and temporary memory loss.
- Skin- Itching, rash, sweating, scaling, blistering or peeling of skin, Pruritus, swelling (especially of the face/tongue/throat), allergic reactions and hives.

- Genitourinary System- Urinary tract infection, renal impairment, increased erection, spontaneous penile erection and painful erection or a prolonged erection that persists for more than 4 hours (that can be damaging for penis), and priapism.
- Liver- Abnormal Liver function tests.
- Musculoskeletal System- Muscle ache, myalgia, arthralgia, back pain, pain in extremities, limb pain, joint pain and neck pain.
- Respiratory System -Upper and lower respiratory tract infection, dyspnea, cough, inflammation of bronchus, and difficulty in breathing.
- Hypersensitivity- Exfoliative dermatitis, Stevens-Johnson syndrome, and urticaria.
- Miscellaneous- Influenza, face edema, and pain.

Tadalafil During Pregnancy and Breast-Feeding
Tadalafil has been classified by the US FDA as Pregnancy Category B.

There are no adequate and well controlled studies of Tadalafil use in pregnant women. Animal reproduction studies in rats and mice revealed no evidence of fetal harm.
Animal reproduction studies showed no evidence of teratogenicity, embryotoxicity, or fetotoxicity when Tadalafil was given to pregnant rats or mice at exposures up to 11 times the maximum recommended human dose (MRHD) of 20 mg/day during organogenesis.
