Terbicip Tablets (Terbinafine Hydrochloride Tablets) are an allylamine antifungal indicated for the treatment of onychomycosis of the toenail or fingernail due to dermatophytes (tinea unguium),
Terbinafine tablets are also used for the treatment of fungal infections of the groin area, the skin (ring worm) and the feet (athlete's foot).

Active Pharmaceutical Ingredient

The active ingredient present in Terbicip tablets is Terbinafine Hydrochloride. Each uncoated Terbicip tablet contains Terbinafine Hydrochloride equivalent to 250 mg of Terbinafine.

Uses of Terbicip Tablets

Terbicip Tablets (Terbinafine Hydrochloride Tablets) are used to treat fungus infections of the scalp, body, groin (jock itch), feet (athlete's foot), fingernails, and toenails.

Terbicip Tablets - Dosage

Before administering Terbinafine tablets, evaluate patients for evidence of chronic or active liver disease
One tablet of Terbicip Tablets (Generic Terbinafine tablets), once a day. The duration of treatment varies according to the indication and the severity of the infection.
Skin Infections:
One tablet of Terbicip Tablets (Generic Terbinafine tablets), once a day.
Tinea pedis (interdigital, plantar/moccasin type): 2 to 6 weeks
Tinea corporis: 4 weeks
Tinea cruris: 2 to 4 weeks
Onychomycosis:
The duration of treatment for most patients is between 6 weeks and 3 months. Treatment periods of less than 3 months can be anticipated in patients with fingernail infection, toenail infection other than of the big toe, or patients of younger age.
Fingernail onychomycosis: One 250 mg tablet once daily for 6 weeks.
Toenail onychomycosis: One 250 mg tablet once daily for 12 weeks.
The optimal clinical effect is seen some months after mycological cure and cessation of treatment. This is related to the period required for outgrowth of healthy nail.
Please consult your doctor for exact dosing instructions. Terbicip tablets (Generic Terbinafine Hydrochloride tablets) should be administered in the dose prescribed by the doctor and for the duration recommended by the doctor.

Missed Dose

If you forget to take your regular Terbicip tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Terbicip Tablets Mechanism of Action – How Do Terbinafine Hydrochloride Tablets Work?

Terbinafine, an allylamine antifungal, inhibits biosynthesis of ergosterol, an essential component of fungal cell membrane, via inhibition of squalene epoxidase enzyme. This results in fungal cell death primarily due to the increased membrane permeability mediated by the accumulation of high concentrations of squalene but not due to ergosterol deficiency.
Depending on the concentration of the drug and the fungal species test in vitro, Terbinafine Hydrochloride may be fungicidal.

Storage Instructions for Terbicip Tablets

Store Terbicip tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Terbicip tablets past its expiration date printed on the blister pack. Keep the pack of Terbicip tablets away from the reach of children and pets. Do not dispose Terbinafine Hydrochloride tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Terbicip tablets are contraindicated in patients with a hypersensitivity to Terbinafine Hydrochloride or any inactive ingredient present in tablets. Terbicip tablets are contraindicated in the following cases:

Overdose

Clinical experience regarding overdose with Terbinafine tablets (Terbicip tablets) is limited. Doses up to 5 grams (20 times the therapeutic daily dose) have been taken without inducing serious adverse reactions. The symptoms of overdose included nausea,vomiting, abdominal pain, dizziness, rash, frequent urination, and headache.

Warnings and Precautions

Hepatotoxicity
Terbinafine tablets are contraindicated for patients with chronic or active liver disease. Before prescribing terbinafine tablets, perform liver function tests because hepatotoxicity may occur in patients with and without preexisting liver disease
Taste Disturbance Including Loss of Taste
Taste disturbance, including taste loss, has been reported with the use of terbinafine tablets. It can be severe enough to result in decreased food intake, weight loss, anxiety, and depressive symptoms.
Smell Disturbance Including Loss of Smell
Smell disturbance, including loss of smell, has been reported with the use of terbinafine tablets. Smell disturbance may resolve after discontinuation of treatment, but may be prolonged (greater than 1 year), or may be permanent.
Depressive Symptoms
Depressive symptoms have occurred during postmarketing use of terbinafine tablets. Prescribers should be alert to the development of depressive symptoms, and patients should be instructed to report depressive symptoms to their physician.
Hematologic Effects
Transient decreases in absolute lymphocyte counts (ALCs) have been observed in controlled clinical trials.
Serious Skin/Hypersensitivity Reactions
There have been postmarketing reports of serious skin/hypersensitivity reactions [e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis, erythema multiforme, exfoliative dermatitis, bullous dermatitis, and drug reaction with eosinophilia and systemic symptoms (DRESS) syndrome].
Lupus Erythematosus
During postmarketing experience, precipitation and exacerbation of cutaneous and systemic lupus erythematosus have been reported in patients taking terbinafine tablets.
Thrombotic Microangiopathy
Cases of thrombotic microangiopathy (TMA), including thrombotic thrombocytopenic purpura and hemolytic uremic syndrome, some fatal, have been reported with Terbinafine.

Side Effects of Terbinafine Hydrochloride Tablets

Side effects reported in 1% or more of patients treated with Terbinafine Hydrochloride tablets are Decreased appetite, Gastrointestinal symptoms (feeling of fullness abdominal distension, dyspepsia, vomiting, nausea, abdominal pain, diarrhoea), Non-serious forms of skin reactions (rash, urticaria), Musculoskeletal reactions (arthralgia, myalgia) and Headache.
The uncommon side effects include Taste disturbances, including taste loss, which usually recover within several weeks after discontinuation of the drug. Isolated cases of prolonged taste disturbance have been reported, sometimes leading to a decrease of food intake and significant weight loss.

Terbinafine Hydrochloride Tablets During Pregnancy

Terbinafine Hydrochloride tablets (Terbicip tablets) has been classified by the US FDA as Pregnancy Category B.
There are no adequate and well-controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, and because treatment of onychomycosis can be postponed until after pregnancy is completed, it is recommended that Terbicip tablets not be initiated during pregnancy.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Terbinafine Hydrochloride Tablets - Nursing Considerations

After oral administration, Terbinafine is present in breast milk of nursing mothers. The ratio of Terbinafine in milk to plasma is 7:1. Treatment with Terbicip is not recommended in nursing mothers.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
