The active ingredient present in Seroflo inhaler are Fluticasone propionate and Salmeterol Xinafoate. Each actuation delivers a combination of Fluticasone Propionate (50, 125, or 250 mcg) and Salmeterol (25 mcg) from mouthpiece.

Uses of Seroflo Inhaler

Asthma:
Seroflo Inhaler is indicated for the regular treatment of asthma (Reversible Obstructive Airways Disease) in adults, adolescents and children aged 4 years and over, where use of a combination product (bronchodilator and inhaled corticosteroid) is appropriate.
This may include:

• Patients on effective maintenance doses of both long-acting beta-agonists and inhaled corticosteroids using separate products.
• Patients who are not adequately controlled on current inhaled corticosteroid therapy.

Patients who are not adequately controlled on “as needed” short-acting beta-agonists, as an alternative to initiation of maintenance therapy with moderate or high doses of inhaled corticosteroid alone.
Seroflo Inhaler should not typically be used for the initial management of asthma, unless symptoms are severely uncontrolled, nor in patients whose asthma can be managed by occasional use of short-acting beta-2 agonists.
Seroflo Inhaler should not be used in the treatment of acute asthmatic symptoms.
Chronic Obstructive Pulmonary Disease (COPD):
Seroflo Inhaler is indicated for the symptomatic treatment of adult patients with moderate to severe COPD (pre-bronchodilator FEV1<60% predicted normal), who have significant symptoms despite bronchodilator therapy.

Seroflo inhaler (Generic Fluticasone propionate and Salmeterol inhaler) delivers a combination of fluticasone propionate (50, 125, or 250 mcg) and salmeterol (25 mcg) from mouthpiece per actuation. Seroflo inhaler contains 120 metered doses.  The dosage for the various indications is given below:
Asthma
Adults and Adolescents (12 years and older)
Seroflo 50 Inhaler: Two inhalations twice daily
Seroflo 125 Inhaler: Two inhalations twice daily
Children (4 years and older)

Not recommended for children below 4 years of age.
COPD
Seroflo 125 inhaler: Two inhalations twice daily
Seroflo Inhaler work effectively if the amount of Fluticasone Propionate and Salmeterol Xinafoate is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Missed Dose

Seroflo Inhaler Mechanism of Action

Seroflo Inhaler contains Fluticasone Propionate and Salmeterol Xinafoate which have differing modes of action. The respective mechanisms of action of both drugs are discussed below.
Fluticasone Propionate:
Fluticasone propionate given by inhalation at recommended doses has a glucocorticoid anti-inflammatory action within the lungs, resulting in reduced symptoms and exacerbations of asthma, with less adverse effects than when corticosteroids are administered systemically.
Salmeterol Xinafoate:
Salmeterol is a selective long-acting (12 hour) beta2 adrenoceptor agonist with a long side chain which binds to the exo-site of the receptor.Salmeterol produces a longer duration of bronchodilation, lasting for at least 12 hours,than recommended doses of conventional short-acting beta2 agonists.

Storage Instructions for Seroflo Inhaler

Store Seroflo Inhaler at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep Seroflo Inhaler away from the reach of children and pets. Do not dispose Seroflo Inhaler (Fluticasone Propionate and Salmeterol Xinafoate Inhaler) along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Seroflo Inhaler is contraindicated in patients with a hypersensitivity to Fluticasone Propionate or Salmeterol Xinafoate or any inactive ingredient present in Seroflo Inhaler.
Seroflo Inhaler is also contraindicated in the primary treatment of status asthmaticus or other acute episodes of asthma where intensive measures are required.

Overdose

There are no data available from clinical trials on overdose with Salmeterol/Fluticasone; however data on overdose with both drugs are given below: Salmeterol Xinafoate:
The signs and symptoms of Salmeterol overdose are tremor, headache and tachycardia. The preferred antidotes are cardioselective beta-blocking agents, which should be used with caution in patients with a history of bronchospasm.
If Seroflo Inhaler therapy has to be withdrawn due to overdose of the beta agonist component of the drug, provision of appropriate replacement steroid therapy should be considered. Additionally, hypokalaemia can occur and potassium replacement should be considered.
Acute: Acute inhalation of fluticasone propionate doses in excess of those recommended may lead to temporary suppression of adrenal function. This does not need emergency action as adrenal function is recovered in a few days, as verified by plasma cortisol measurements.
Chronic Overdose of Inhaled Fluticasone Propionate:
Adrenal reserve should be monitored and treatment with a systemic corticosteroid may be necessary. When stabilised, treatment should be continued with an inhaled corticosteroid at the recommended dose. In cases of both acute and chronic Fluticasone Propionate overdose Seroflo Inhaler therapy may still be continued at a suitable dosage for symptom control.

Warnings and Precautions

LABA monotherapy
LABA monotherapy increases the risk of serious asthma-related events.
Deterioration of asthma and acute episodes:
Do not use for relief of acute symptoms. Patients require immediate re-evaluation during rapidly deteriorating asthma.
Additional medicine containing LABA
Do not use in combination with an additional medicine containing LABA because of risk of overdose
Localized infections:
Candida albicans infection of the mouth and pharynx may occur. Monitor patients periodically. Advise the patient to rinse his/her mouth with water without swallowing after inhalation to help reduce the risk.
Immunosuppression:
Potential worsening of existing tuberculosis, fungal, bacterial, viral, parasitic infection, or ocular herpes simplex. Use with caution in patients with these infections. More serious or even fatal course of chickenpox or measles can occur in susceptible patients.
Transferring patients from systemic corticosteroids:
Risk of impaired adrenal function when transferring from systemic corticosteroids. Taper patients slowly from systemic corticosteroids if transferring to Seroflo Inhaler.
Hypercorticism and adrenal suppression:
May occur with very high dosages or at the regular dosage in susceptible individuals. If such changes occur, discontinue Seroflo Inhaler slowly.
Risk of Pneumonia:
Increased risk of pneumonia in patients with COPD. Monitor patients for signs and symptoms of pneumonia.
Paradoxical bronchospasm:
Discontinue AirDuo Digihaler and institute alternative therapy if paradoxical bronchospasm occurs.
Cardiovascular Or Central Nervous System Disorders:
Use with caution in patients with cardiovascular or central nervous system disorders because of beta adrenergic stimulation.
Decreases in bone mineral density:
Monitor patients with major risk factors for decreased bone mineral content.
Monitor growth:
Monitor growth of pediatric patients.
Monitoring For Glaucoma And Cataracts:
Glaucoma and cataracts may occur with long-term use of inhaled corticosteroids. Consider referral to an ophthalmologist in patients who develop ocular symptoms or use Seroflo Inhaler long term.
Eosinophilic Conditions:
Be alert to eosinophilic conditions, hypokalemia, and hyperglycemia.
Use With Caution:
Use with caution in patients with convulsive disorders, thyrotoxicosis, diabetes mellitus, and ketoacidosis

Side Effects of Seroflo Inhaler (Fluticasone Propionate and Salmeterol Xinafoate Inhaler)

Most common Fluticasone Propionate and Salmeterol Xinafoate Inhaler side effects with an incidence rate of 3% or more include: upper respiratory tract infection or inflammation, throat irritation, dysphonia, headache, dizziness, nausea and vomiting.
