Selgin (Selegiline Hydrochloride Tablets) - Product Information

Selgin (Selegiline Hydrochloride Tablets) is indicated as an adjunct in the management of Parkinsonian patients being treated with Levodopa/Carbidopa who exhibit deterioration in the quality of their response to this therapy.
There is no evidence from controlled studies that Selegiline has any beneficial effect in the absence of concurrent Levodopa therapy.

The active ingredient present in Selgin tablets is Selegiline Hydrochloride. Each Selgin tablet contains 5 mg of Selegiline Hydrochloride.

Uses of Selgin Tablets

Selgin (Selegiline Hydrochloride Tablets) is used in combination with levodopa or Levodopa and Carbidopa combination to treat Parkinson's disease (sometimes called "shaking palsy" or "paralysis agitans"). This medicine works to increase and extend the effects of levodopa, and may help to slow the progress of Parkinson's disease.

Selgin Tablets - Dosage

Generic Selegiline Hydrochloride tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Selgin tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
The recommended regimen for the administration of selegiline hydrochloride is 10 mg per day administered as divided doses of 5 mg each taken at breakfast and lunch. There is no evidence that additional benefit will be obtained from the administration of higher doses. Moreover, higher doses should ordinarily be avoided because of the increased risk of side effects.
Selgin tablets work effectively if the amount of Selegiline Hydrochloride is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Selgin tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Selgin tablet dose on time, take it whenever you remember.

Selegiline Hydrochloride Mechanism of Action

Selegiline belongs to a group of medicines called monoamine oxidase inhibitors (MAOI). It is believed to increase the amount of a natural chemical called dopamine in the brain, which helps alleviate the movement-related symptoms of Parkinson’s disease.

Storage Instructions for Selgin Tablets

Store Selgin tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Selgin tablets away from the reach of children and pets. Do not dispose along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Selgin tablets are contraindicated in patients with a hypersensitivity to Selegiline Hydrochloride or any inactive ingredient present in tablets.
Selgin is contraindicated for use with Meperidine (Demerol & other trade names). This contraindication is often extended to other opioids.

Overdose

No specific information is available about clinically significant overdoses with swallowed Selegiline. However, experience gained during development of the 5 mg swallowed dosage form reveals that some individuals exposed to doses of 600 mg of d,l-Selegiline suffered severe hypotension and psychomotor agitation

Warnings and Precautions

Hypertension:
Selgin tablets should not be used at daily doses exceeding those recommended (2.5 mg/day) because of the risks associated with non-selective inhibition of MAO
Serotonin Syndrome:
Serotonin syndrome and hyperpyrexia have been reported with the combined treatment of an antidepressant (e.g., selective serotonin reuptake inhibitors-SSRIs, serotonin-norepinephrine reuptake inhibitors-SNRIs, tricyclic antidepressants, tetracyclic antidepressants, triazolopyridine antidepressants) and a non-selective MAOI (e.g., phenelzine, tranylcypromine) or selective MAO-B inhibitors, such as Selegiline.
Falling Asleep During Activities of Daily Living and Somnolence:
Patients with Parkinson's disease treated with Selgin or other drugs increasing dopaminergic tone have reported falling asleep while engaged in activities of daily living, including the operation of motor vehicles, which sometimes resulted in accidents. Although many of these patients reported somnolence, some did not perceive warning signs, such as excessive drowsiness, and believed that they were alert immediately prior to the event. Some of these events have been reported as late as one year after initiation of treatment.
Hypotension/Orthostatic Hypotension
Assessments of orthostatic (supine and standing) blood pressures at different times throughout the 12 week study period in two controlled trials showed that the frequency of orthostatic hypotension (>20 mm Hg decrease in systolic blood pressure and/or >10 mm Hg decrease in diastolic blood pressure) was greater with Selgin treatment than with placebo treatment.
Dyskinesia
Selgin may potentiate dopaminergic side effects of levodopa and may cause dyskinesia or exacerbate preexisting dyskinesia. In controlled trials, the incidence of dyskinesia was 6% in Selgin -treated patients and 3% in placebo-treated patients
Hallucinations/Psychotic-Like Behavior
In controlled trials, hallucination was reported by 4% of Selegiline Hydrochloride -treated patients and 2% in placebo-treated patients. Hallucinations led to drug discontinuation and premature withdrawal from clinical trials in about 1% of Selegiline Hydrochloride -treated patients, compared to no patient on placebo.
Impulse Control/Compulsive Behaviors
Case reports suggest that patients can experience intense urges to gamble, increased sexual urges, intense urges to spend money, binge eating, and/or other intense urges, and the inability to control these urges while taking one or more of the medications, including Selegiline Hydrochloride , that increase central dopaminergic tone and that are generally used for the treatment of Parkinson's disease. In some cases, although not all, these urges were reported to have stopped when the dose was reduced or the medication was discontinued.
Withdrawal Emergent Hyperpyrexia and Confusion
Although not reported with Selegiline Hydrochloride in the clinical development program, a symptom complex resembling the neuroleptic malignant syndrome (characterized by elevated temperature, muscular rigidity, altered consciousness, and autonomic instability), with no other obvious etiology, has been reported in association with rapid dose reduction, withdrawal of, or changes in antiparkinsonian therapy.
Melanoma
Epidemiological studies have shown that patients with Parkinson's disease have a higher risk (2- to approximately 6-fold higher) of developing melanoma than the general population.
Irritation of the Buccal Mucosa
In the controlled clinical trials, periodic examinations of the tongue and oral mucosa were performed. At the end of the study, the frequency of mild oropharyngeal abnormality (e.g., swallowing pain, mouth pain, discrete areas of focal reddening, multiple foci of reddening, edema, and/or ulceration) in patients without similar abnormality at baseline was 10% in Selegiline Hydrochloride-treated patients compared to 3% in placebo-treated patients
Effect on Renal Function
Small increments in serum BUN and creatinine have been observed in patients treated with high dose Selegiline Hydrochloride

Side Effects of Selegiline Hydrochloride (Selgin Tablets)

The most common side effects(treatment difference incidence at least 3% greater than placebo incidence) reported in the double-blind, placebo-controlled trials during Selegiline Hydrochloride treatment were constipation, skin disorders, vomiting, dizziness, dyskinesia, insomnia, dyspnea, myalgia, and rash.

Selegiline Hydrochloride Tablets During Pregnancy and Breastfeeding

Selegiline Hydrochloride has been classified by the US FDA as Pregnancy Category C.
There are no adequate and well-controlled studies in pregnant women. In animal studies, administration of Selegiline during pregnancy was associated with developmental toxicity (decreased embryofetal and postnatal offspring growth and survival) at doses greater than those used clinically. Selgin should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Do not use Selgin (Selegiline Hydrochloride Tablets) without telling your doctor if you are breast-feeding a baby.
