Ritomune (Generic Ritonavir Tablets) is indicated in combination with other antiretroviral agents for the treatment of HIV-infection (adults and children of 2 years of age and older).
HIV is the virus that causes acquired immune deficiency syndrome (AIDS). Consideration should be given to official treatment guidelines for HIV-1 infection.
Ritonavir will not cure or prevent HIV infection or AIDS. It helps keep HIV from reproducing and appears to slow down the destruction of the immune system. This may help delay problems that are usually related to AIDS or HIV disease from occurring.
Ritonavir will not keep you from spreading HIV to other people. People who receive this medicine may continue to have other problems related to AIDS or HIV disease.

Ritonavir is commonly used for the treatment of HIV (in combination with other medicine).
Ritomune Tablets (Generic Ritonavir) - Dosage
Generic Ritonavir tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Ritomune tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Ritomune is administered orally. Ritonavir tablets should be swallowed whole, and not chewed, broken or crushed. Take Ritonavir tablets with meals.
Recommended Dosage for Treatment of HIV-1
The recommended dosage of ritonavir is 600 mg twice daily by mouth to be taken with meals. Use of a dose titration schedule may help to reduce treatment-emergent adverse events while maintaining appropriate ritonavir plasma levels.
Ritonavir should be started at no less than 300 mg twice daily and increased at 2 to 3 day intervals by 100 mg twice daily. The maximum dose of 600 mg twice daily should not be exceeded upon completion of the titration.
Ritomune tablets work effectively if the amount of Ritonavir is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Ritomune tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your regular Ritomune tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.
Mechanism of Action
People with HIV have a virus that spreads in their body and damages the immune system, making it easier for them to get very sick from other infections.
Ritonavir works together with other medicines to help stop the spread of HIV in your body. This helps protect and preserve the immune system and can prevent the virus from being spread to other people.
Ritonavir is a peptidomimetic inhibitor of the HIV-1protease. Inhibition of HIV protease renders the enzyme incapable of processing the gag-pol polyprotein precursor, which leads to production of non-infectious immature HIV particles.
Storage Instructions for Ritomune Tablets
Store Ritomune tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Generic Ritonavir tablets past its expiration date printed on the blister pack. Keep the pack of Ritomune tablets away from the reach of children and pets. Do not dispose along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Ritomune tablets are contraindicated in patients with a hypersensitivity to Ritonavir or any inactive ingredient present in Ritomune tablets. Ritomune tablets are also contraindicated in the following cases:
Co-administration of Ritonavir with several classes of drugs (including sedative hypnotics, antiarrhythmics, or ergot alkaloid preparations) is contraindicated and may result in potentially serious and/or life-threatening adverse events due to possible effects of ritonavir on the hepatic metabolism of these drugs. Voriconazole and St. John’s Wort are exceptions in that co-administration of ritonavir and voriconazole results in a significant decrease in plasma concentrations of voriconazole, and co-administration of Ritonavir with St. John’s Wort may result in decreased Ritonavir plasma concentrations.
Overdose
Human experience of acute overdose with Ritonavir tablets is limited. One patient in clinical trials took Ritonavir 1500 mg per day for two days. The patient reported paresthesias which resolved after the dose was decreased. A post-marketing case of renal failure with eosinophilia has been reported with Ritonavir overdose. Treatment of overdose with Ritonavir consists of general supportive measures including monitoring of vital signs and observation of the clinical status of the patient. There is no specific antidote for overdose with Ritonavir.
Warnings and Precautions
The following have been observed in patients receiving Ritonavir: Concomitant use
The concomitant use of Ritonavir and certain other drugs may result in known or potentially significant drug interactions. Consult the full prescribing information prior to and during treatment for potential drug interactions.
Hepatotoxicity:
Fatalities have occurred. Monitor liver function before and during therapy, especially in patients with underlying hepatic disease, including hepatitis B and hepatitis C, or marked transaminase elevations
Pancreatitis:
Fatalities have occurred; suspend therapy as clinically appropriate.
Allergic Reactions/Hypersensitivity:
Allergic reactions have been reported and include anaphylaxis, toxic epidermal necrolysis, Stevens-Johnson syndrome, bronchospasm and angioedema. Discontinue treatment if severe reactions develop
PR interval prolongation:
PR interval prolongation may occur in some patients. Cases of second and third degree heart block have been reported. Use with caution with patients with preexisting conduction system disease, ischemic heart disease, cardiomyopathy, underlying structural heart disease or when administering with other drugs that may prolong the PR interval
Total cholesterol and triglycerides elevations:
Monitor prior to therapy and periodically thereafter. Triglyceride and cholesterol testing should be performed prior to initiating ritonavir therapy and at periodic intervals during therapy
Diabetes Mellitus And Hyperglycemia:
New onset diabetes mellitus, exacerbation of pre-existing diabetes mellitus, and hyperglycemia have been reported during post-marketing surveillance in HIV-infected patients receiving protease inhibitor therapy. Some patients required either initiation or dose adjustments of insulin or oral hypoglycemic agents for treatment of these events.
Immune Reconstitution Syndrome:
Immune reconstitution syndrome has been reported in HIV-infected patients treated with combination antiretroviral therapy, including ritonavir. During the initial phase of combination antiretroviral treatment, patients whose immune system responds may develop an inflammatory response to indolent or residual opportunistic infections, which may necessitate further evaluation and treatment.
Fat Redistribution:
Patients may develop redistribution/accumulation of body fat
Patients with Hemophilia:
Spontaneous bleeding may occur, and additional factor VIII may be required
Side Effects of Ritomune (Generic Ritonavir Tablets)
The most frequently reported Ritonavir tablets side effects occurring in in greater than or equal to 1% of adult patients receiving Ritonavir alone or in combination with other antiretroviral drugs were gastrointestinal (including diarrhea, nausea, vomiting, abdominal pain (upper and lower)), neurological disturbances (including paresthesia and oral paresthesia), rash, and fatigue/asthenia.
Ritonavir During Pregnancy
Ritonavir has been classified by the US FDA as Pregnancy Category B.
There are no adequate and well-controlled studies in pregnant women. Ritonavir should be used during pregnancy only if the potential benefit justifies the potential risk of the fetus. No treatment related malformations were observed when ritonavir was administered to pregnant rats or rabbits.Developmental toxicity observed in rats (early resorptions, decreased fetal body weight and ossification delays and developmental variations) occurred at a maternally toxic dosage at an exposure equivalent to approximately 30% of that achieved with the proposed therapeutic dose. A slight increase in the incidence of cryptorchidism was also noted in rats at an exposure approximately 22% of that achieved with the proposed therapeutic dose.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Nursing Considerations
The Centers for Disease Control and Prevention recommend that HIV-infected mothers not breastfeed their infants to avoid risking postnatal transmission of HIV. It is not known whether ritonavir is secreted in human milk. Because of both the potential for HIV transmission and the potential for serious adverse reactions in nursing infants, mothers should be instructed not to breastfeed if they are receiving Ritonavir.
