Uses of Duovir Tablets
Duovir, an antiviral medicine, is used in combination with at least one other antiviral medicine to treat patients infected with human immunodeficiency virus (HIV), the virus that causes acquired immune deficiency syndrome (AIDS). Duovir contains two active substances, Lamivudine and Zidovudine. Lamivudine and zidovudine are antiviral medications that prevent human immunodeficiency virus (HIV) from multiplying in your body.
Duovir Tablets (Lamivudine and Zidovudine Tablets) - Dosage
Generic Lamivudine and Zidovudine tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Duovir tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Adults and Adolescents Weighing 30 kg or More:
The recommended oral dose of Duovir tablet for HIV-1 infected adults and adolescents weighing greater than or equal to 30 kg is 1 tablet (containing 150 mg of lamivudine and 300 mg of zidovudine) twice daily.
Pediatric Patients:
The recommended oral dosage of Duovir tablet for pediatric patients who weigh greater than or equal to 30 kg and for whom a solid oral dosage form is appropriate is 1 tablet administered twice daily.

Duovir tablets work effectively if the amount of Lamivudine and Zidovudine is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Duovir tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Duovir tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your regular tablet dose on time, take it whenever you remember.

Mechanism of Action
Lamivudine: Lamivudine is a synthetic nucleoside analogue. Intracellularly, lamivudine is phosphorylated to its active 5-triphosphate metabolite, lamivudine triphosphate (3TC-TP). The principal mode of action of 3TC-TP is the inhibition of HIV-1 reverse transcriptase (RT) via DNA chain termination after incorporation of the nucleoside analogue into viral DNA. 3TC-TP is a weak inhibitor of cellular DNA polymerases alpha, beta and gamma.
Zidovudine: Zidovudine is a synthetic nucleoside analogue. Intracellularly, zidovudine is phosphorylated to its active 5'-triphosphate metabolite, zidovudine triphosphate (ZDV-TP). The prinicipal mode of action of ZDV-TP is inhibition of RT via DNA chain termination after incorporation of the nucleotide analogue. ZDV-TP is a weak inhibitor of the cellular DNA polymerase alpha and gamma and has been reported to be incorporated into the DNA of cells in culture
Storage Instructions for Duovir Tablets (Lamivudine and Zidovudine Tablets)
Store Duovir Tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Lamivudine and Zidovudine Tablets away from the reach of children and pets. Do not dispose Duovir Tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Duovir tablets are contraindicated in patients with a hypersensitivity (eg, anaphylaxis, Stevens-Johnson Syndrome) to Lamivudine , Zidovudine or any inactive ingredient present in Duovir tablets.
Overdose
There is no known antidote for Duovir Tablets.
One case of an adult ingesting 6 grams of Lamivudine was reported; there were no clinical signs or symptoms noted and hematologic tests remained normal. Because a negligible amount of Lamivudine was removed via (4-hour) hemodialysis, continuous ambulatory peritoneal dialysis, and automated peritoneal dialysis, it is not known if continuous hemodialysis would provide clinical benefit in a lamivudine overdose event.
Acute overdoses of Zidovudine have been reported in pediatric patients and adults. These involved exposure up to 50 grams. The only consistent findings were nausea and vomiting. Other reported occurrences included headache, dizziness, drowsiness, lethargy, confusion, and one report of grand mal seizure. Hematologic changes were transient. All patients recovered.
Warnings and Precautions
- Hepatic decompensation, some fatal, has occurred in HIV-1/HCV coinfected patients receiving combination antiretroviral therapy and interferon alfa with/without ribavirin. Discontinue lamivudine and zidovudine tablets as medically appropriate and consider dose reduction or discontinuation of interferon alfa, ribavirin, or both.
- Exacerbation of anemia has been reported in HIV-1/HCV co-infected patients receiving ribavirin and zidovudine. Coadministration of ribavirin and zidovudine is not advised.
- Pancreatitis: Use with caution in patients with a history of pancreatitis or other significant risk factors for pancreatitis. Discontinue treatment as clinically appropriate.
- Immune reconstitution syndrome: Immune reconstitution syndrome and redistribution/accumulation of body fat have been reported in patients treated with combination antiretroviral therapy.
Side Effects of Duovir Tablets (Lamivudine and Zidovudine Tablets)
Most commonly reported side effects (incidence greater than or equal to 15%) in clinical trials of combination Lamivudine and Zidovudine were headache, nausea, malaise and fatigue, nasal signs and symptoms, diarrhea, and cough.
Duovir Tablets (Lamivudine and Zidovudine Tablets) During Pregnancy
has been classified by the US FDA as Pregnancy Category C.
There are no adequate and well-controlled studies of Lamivudine/Zidovudine in pregnant women. Clinical trial data demonstrate that maternal zidovudine treatment during pregnancy reduces vertical transmission of HIV-1 infection to the fetus. Animal reproduction studies performed with Lamivudine and Zidovudine showed increased embryo toxicity and fetal malformations (Zidovudine),and increased embryo lethality (Lamivudine). Duovir tablet should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Nursing Considerations
HIV-1-infected mothers should not breastfeed their infants to avoid risking postnatal transmission of HIV-1 infection. Because of the potential for HIV-1 transmission mothers should be instructed not to breastfeed.
