Ciplar-LA (Generic Propranolol Hydrochloride LA Tablets) is used to treat tremors, angina (chest pain), hypertension (high blood pressure), heart rhythm disorders, and other heart or circulatory conditions.
It is also used to treat or prevent heart attack, and to reduce the severity and frequency of migraine headaches.
Ciplar-LA tablets are manufactured by Cipla Limited, India, in the strengths of 20 mg and 40 mg.

Active Pharmaceutical Ingredient

The active ingredient present in Ciplar-LA tablets is Propranolol Hydrochloride. Each long acting Ciplar-LA tablet contains 40 mg of Propranolol Hydrochloride.

Uses of Propranolol Hydrochloride LA Tablets (Ciplar-LA Tablets)

Propranolol hydrochloride long-acting (LA) or extended-release (ER) tablets are a type of non-selective beta-blocker prescribed for various cardiovascular and other conditions. This medication works by relaxing blood vessels and slowing the heart rate, which improves blood flow and lowers blood pressure. Ciplar-LA tablets are used to treat or manage the following conditions: Hypertension (high blood pressure): Can be used alone or in combination with other antihypertensive agents, such as thiazide diuretics. Angina pectoris (chest pain): Used to decrease the frequency of angina episodes and increase exercise tolerance. Migraine prophylaxis: Prescribed for the prevention of common migraine headaches, but not for treating an ongoing migraine attack. Hypertrophic subaortic stenosis: A heart muscle disease where the muscular wall of the heart thickens.

Ciplar-LA Tablets - Dosage

Ciplar-LA provides Propranolol Hydrochloride in a sustained-release tablet for administration once daily. If patients are switched from Ciplar tablets to Ciplar-LA tablets, care should be taken to assure that the desired therapeutic effect is maintained.
Generic Propranolol Hydrochloride LA tablets should be administered exactly as suggested by your doctor.
Hypertension
The usual initial dosage is 80 mg Ciplar-LA once daily, whether used alone or added to a diuretic. The dosage may be increased to 120 mg once daily or higher until adequate blood pressure control is achieved. The usual maintenance dosage is 120 to 160 mg once daily.
Angina Pectoris
Starting with 80 mg Ciplar-LA once daily, dosage should be gradually increased at three-to seven-day intervals until optimal response is obtained. Although individual patients may respond at any dosage level, the average optimal dosage appears to be 160 mg once daily.
Migraine
The initial oral dose is 80 mg Ciplar-LA once daily. The usual effective dose range is 160 to 240 mg once daily. The dosage may be increased gradually to achieve optimal migraine prophylaxis. If a satisfactory response is not obtained within four to six weeks after reaching the maximal dose, Ciplar-LA therapy should be discontinued.
Essential Tremor and Thyrotoxicosis
Dose of 80 mg Ciplar-LA once daily, taken either morning or evening, may be sufficient to provide adequate control in many patients. If necessary, the dose may be increased to 160 mg once daily and also may be increased to 240 mg.
Situational and Generalised Anxiety
Ciplar-LA 80 mg taken daily should be sufficient to provide short-term relief of acute situational anxiety. Generalised anxiety, requiring longer term therapy, usually responds adequately at the same dosage. In individual cases, the dosage may be increased to 160 mg/day. Treatment should be continued according to response. Patients should be reviewed after 6 to 12 months of treatment.
Hypertrophic Subaortic Stenosis
The usual dose is 80 to 160 mg Ciplar-LA once daily
Ciplar-LA tablets work effectively if the amount of Propranolol Hydrochloride is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Ciplar-LA tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Ciplar-LA tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Propranolol Hydrochloride Mechanism of Action

The mechanism of the antihypertensive effect of Propranolol Hydrochloride has not been established.
Among factors that contribute to the antihypertensive action are: (1) decreased cardiac output, (2) inhibition of renin release by the kidneys, and (3) diminution of tonic sympathetic nerve outflow from vasomotor centers in the brain.
Although total peripheral resistance may increase initially, it readjusts to or below the pretreatment level with chronic use of Propranolol Hydrochloride. Effects of Propranolol Hydrochloride on plasma volume appear to be minor and somewhat variable.

Storage Instructions for Propranolol Hydrochloride LA Tablets (Ciplar-LA)

Store Ciplar-LA tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Generic Propranolol Hydrochloride LA Tablets away from the reach of children and pets. Do not dispose Ciplar-LA tablets along with domestic waste or sewage. Contraindications

Ciplar-LA tablets are contraindicated in patients with a hypersensitivity to Propranolol Hydrochloride or any inactive ingredient present in tablets. Ciplar-LA tablets are also contraindicated in the following cases:
Cardiogenic shock or decompensated heart failure
Sinus bradycardia, sick sinus syndrome, and greater than first-degree block unless a permanent pacemaker is in place
Bronchial asthma

Overdose

Most overdoses of propranolol are mild and respond to supportive care.
Propranolol is not significantly dialyzable.
Hypotension and bradycardia have been reported following propranolol overdose and should be treated appropriately. Glucagon can exert potent inotropic and chronotropic effects and may be particularly useful for the treatment of hypotension or depressed myocardial function after a propranolol overdose.
Monitor the electrocardiogram, pulse, blood pressure, neurobehavioral status and intake and output balance. Isoproterenol and aminophylline may be used for bronchospasm.

Warnings and Precautions

Cardiac Ischemia after Abrupt Discontinuation:
Following abrupt discontinuation of therapy with beta-blockers, exacerbations of angina pectoris and myocardial infarction have occurred.
Cardiac Failure
Beta-blockers, like , can cause depression of myocardial contractility and may precipitate heart failure and cardiogenic shock. If signs or symptoms of heart failure develop, treat the patient according to recommended guidelines.
Maintain During Major Surgery:
Chronically administered beta-blocking therapy, including Propranolol Hydrochloride tablets, should not be routinely withdrawn prior to major surgery; however, the impaired ability of the heart to respond to reflex adrenergic stimuli may augment the risks of general anesthesia and surgical procedures.
Hypoglycemia:
Beta-blockers may prevent early warning signs of hypoglycemia, such as tachycardia, and increase the risk for severe or prolonged hypoglycemia at any time during treatment, especially in patients with diabetes mellitus or children and patients who are fasting (i.e., surgery, not eating regularly, or are vomiting). If severe hypoglycemia occurs, patients should be instructed to seek emergency treatment.
Thyrotoxicosis:
Propranolol Hydrochloride tablets may mask clinical signs of hyperthyroidism, such as tachycardia. Avoid abrupt withdrawal of beta-blockade, which may precipitate a thyroid storm.

Bradycardia:

Reduced Effectiveness of Epinephrine in Treating Anaphylaxis:
Beta-adrenergic blocker-treated patients treated with epinephrine for a severe anaphylactic reaction may be less responsive to the typical doses of epinephrine. In these patients, consider other medications (e.g., intravenous fluids, glucagon.

Side Effects of Ciplar-LA (Propranolol Hydrochloride Tablets)

The following side effects were observed and have been reported in patients using Propranolol Hydrochloride tablets (Ciplar-LA).

  • Cardiovascular: Bradycardia; congestive heart failure; intensification of AV block; hypotension; paresthesia of hands; thrombocytopenic purpura; arterial insufficiency, usually of the Raynaud type.
  • Central Nervous System: Light-headedness; mental depression manifested by insomnia, lassitude, weakness, fatigue; catatonia; visual disturbances; hallucinations; vivid dreams; an acute reversible syndrome characterized by disorientation for time and place, short-term memory loss, emotional lability, slightly clouded sensorium, and decreased performance on neuropsychometrics. For immediate release formulations, fatigue, lethargy, and vivid dreams appear dose related.
  • Gastrointestinal: Nausea, vomiting, epigastric distress, abdominal cramping, diarrhea, constipation, mesenteric arterial thrombosis, ischemic colitis.
  • Allergic: Hypersensitivity reactions, including anaphylactic/anaphylactoid reactions; pharyngitis and agranulocytosis; erythematous rash; fever combined with aching and sore throat; laryngospasm; respiratory distress.
  • Respiratory: Bronchospasm.
  • Hematologic: Agranulocytosis, nonthrombocytopenic purpura, and thrombocytopenic purpura.
  • Autoimmune: Systemic lupus erythematosus (SLE).
  • Skin and mucous membranes: Stevens-Johnson Syndrome, toxic epidermal necrolysis, dry eyes, exfoliative dermatitis, erythema multiforme, urticaria, alopecia, SLE-like reactions, and psoriasisiform rashes. Oculomucocutaneous syndrome involving the skin, serous membranes, and conjunctivae reported for a beta-blocker (practolol) have not been associated with propranolol.
  • Genitourinary: Male impotence; Peyronie's disease.

 

Propranolol Hydrochloride (Ciplar-LA Tablets ) During Pregnancy

Propranolol Hydrochloride (Ciplar-LA Tablets ) has been classified by the US FDA as Pregnancy Category C.
There are no adequate and well-controlled studies in pregnant women. Intrauterine growth retardation, small placentas, and congenital abnormalities have been reported in neonates whose mothers received Propranolol Hydrochloride during pregnancy.
Neonates whose mothers are receiving propranolol at parturition have exhibited bradycardia, hypoglycemia and/or respiratory depression. Adequate facilities for monitoring such infants at birth should be available.
Ciplar-LA should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

Propranolol is excreted in human milk. Caution should be exercised when Ciplar-LA is administered to a nursing woman.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
