Levoflox (Generic Levofloxacin tablets) is an antibiotic used to treat a variety of bacterial infections. Some infections that Levoflox is used to treat include Lung infections, such as bronchitis or pneumonia, Sinus infections, Skin infections, Kidney or bladder infections, Prostate infections, Certain anthrax infections and plague.

Active Pharmaceutical Ingredient

The active ingredient present in Levoflox tablets is Levofloxacin Hemihydrate.
Each film-coated tablet of Levoflox contains Levofloxacin Hemihydrate equivalent to 250 mg , 500 mg or 750 mg of Levofloxacin.

Uses of Levofloxacin Tablets (Levoflox)

Levofloxacin is Fluoroquinolone antibacterial indicated in adults ( 18 years years of age or greater) to treat bacterial infections caused by designated, susceptible bacteria in many different parts of the body. It is also used to treat anthrax infection after inhalational exposure. Levofloxacin is also used to treat and prevent plague (including pneumonic and septicemic plague).
Levofloxacin belongs to the class of medicines known as quinolone antibiotics. It works by killing bacteria or preventing their growth. However, this medicine will not work for colds, flu, or other virus infections. Levoflox tablets are used to treat the following infections:

  • Pneumonia: nosocomial and community acquired
  • Acute bacterial sinusitis
  • Acute bacterial exacerbation of chronic bronchitis
  • Skin and skin structure infections: complicated and uncomplicated
  • Chronic bacterial prostatitis
  • Urinary tract infections: complicated and uncomplicated

Levoflox (Levofloxacin Tablets) - Dosage

Generic Levofloxacin Hemihydrate tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Levoflox tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
The dosage of Levofloxacin tablets (Levoflox) for various indications is given below: Nosocomial Pneumonia:
The dosage is 750 mg a day for 7-14 days.
Community Acquired Pneumonia:
The dosage is 500 mg a day for 7-14 days.
Community Acquired Pneumonia:
The dosage is 750 mg a day for 5 days.
Acute Bacterial Sinusitis :
The dosage is 750 mg a day for 5 days or 500 mg a day for 10-14 days.
Acute Bacterial Exacerbation of Chronic Bronchitis :
The dosage is 500 mg a day for 7 days.
Complicated Skin and Skin Structure Infections (SSSI):
The dosage is 750 mg a day for 7-14 days.
Uncomplicated SSSI:
The dosage is 500 mg a day for 7-10 days.
Chronic Bacterial Prostatitis :
The dosage is 500 mg a day for 28 days.
Complicated Urinary Tract Infection or Acute Pyelonephritis:
The dosage is 750 mg a day for 5 days.
Complicated Urinary Tract Infection or Acute Pyelonephritis:
The dosage is 250 mg a day for 10 days.
Uncomplicated Urinary Tract Infection:
The dosage is 250 mg a day for 3 days.
Inhalational Anthrax (Post-Exposure):
The dosage for Adults and Pediatric Patients more than 50 kg and 6 months of age or more is 500 mg daily for 60 days. In case of Pediatric Patients 6 months of age or more and weighing less than 50 kg the dosage is 8 mg/kg twice a day subject to the condidition that the dose should not exceed 250 mg per dose for 60 days.
Levoflox tablets work effectively if the amount of Levofloxacin is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Levoflox tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Please consult your doctor for exact dosing instructions. Levoflox tablets (Generic Levofloxacin Hemihydrate tablets) should be administered in the dose prescribed by the doctor and for the duration recommended by the doctor.

Missed Dose

If you forget to take your regular Levoflox tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Levofloxacin Tablets Mechanism of Action

Levofloxacin acts by disrupting two vital bacterial enzymes, leading to lethal damage of the bacterial chromosome.
Inhibition of DNA gyrase: This enzyme is responsible for the negative supercoiling of bacterial DNA, a process that allows the large bacterial chromosome to be condensed and fit inside the cell. By blocking DNA gyrase, levofloxacin prevents the DNA from being properly packaged.
Inhibition of topoisomerase IV: This enzyme is necessary to separate the replicated DNA strands before a bacterial cell can divide. By inhibiting topoisomerase IV, levofloxacin stops the separation process, blocking the bacteria's ability to multiply.
By inhibiting these two enzymes, levofloxacin causes the accumulation of irreversible DNA strand breaks, which ultimately leads to the death of the bacterial cell.

Storage Instructions for Levoflox Tablets (Levofloxacin Tablets)

Store Levoflox tablets (Levofloxacin Tablets) at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Levofloxacin tablets past its expiration date printed on the blister pack. Keep the pack of Levoflox tablets away from the reach of children and pets. Do not dispose Levofloxacin tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Levoflox tablets are contraindicated in patients with a hypersensitivity to Levofloxacin, other quinolone antibacterials or any inactive ingredient present in tablets. Levoflox tablets are also contraindicated in the following cases:
in patients with epilepsy
in patients with history of tendon disorders related to fluoroquinolone administration
in children or growing adolescents
during pregnancy
in breast-feeding women

Overdose

According to toxicity studies in animals or clinical pharmacology studies performed with supra- therapeutic doses, the most important signs to be expected following acute overdosage of Levofloxacin tablets are central nervous system symptoms such as confusion, dizziness, impairment of consciousness, and convulsive seizures, increases in QT interval as well as gastro- intestinal reactions such as nausea and mucosal erosions.
CNS effects including confusional state, convulsion, myoclonus, hallucination, and tremor have been observed in post marketing experience.
In the event of overdose, symptomatic treatment should be implemented, ECG monitoring should be undertaken, because of the possibility of QT interval prolongation. Antacids may be used for protection of gastric mucosa. Haemodialysis, including peritoneal dialysis and CAPD, are not effective in removing levofloxacin from the body. No specific antidote exists.

Warnings and Precautions

Disabling and Potentially Irreversible Serious Adverse Reactions Including Tendinitis and Tendon Rupture, Peripheral Neuropathy, and Central Nervous System Effects
Fluoroquinolone, including Levofloxacin, have been associated with disabling and potentially irreversible serious adverse reactions from different body systems that can occur together in the same patient. Commonly seen side effects include tendinitis, tendon rupture, arthralgia, myalgia, peripheral neuropathy, and central nervous system effects (hallucinations, anxiety, depression, insomnia, severe headaches, and confusion). These reactions can occur within hours to weeks after starting Levofloxacin.
Tendinitis and Tendon Rupture
Fluoroquinolones, including Levofloxacin, have been associated with an increased risk of tendinitis and tendon rupture in all ages. This adverse reaction most frequently involves the Achilles tendon and has also been reported with the rotator cuff (the shoulder), the hand, the biceps, the thumb, and other tendon sites. Tendinitis or tendon rupture can occur within hours or days of starting Levofloxacin or as long as several months after completion of fluoroquinolone therapy. Tendinitis and tendon rupture can occur bilaterally.
Peripheral Neuropathy
Fluoroquinolones, including Levofloxacin, have been associated with an increased risk of peripheral neuropathy. Cases of sensory or sensorimotor axonal polyneuropathy affecting small and/or large axons resulting in paresthesias, hypoesthesias, dysesthesias and weakness have been reported in patients receiving fluoroquinolones, including Levofloxacin. Symptoms may occur soon after initiation of Levofloxacin and may be irreversible in some patients.
Central Nervous System Effects
Fluoroquinolones, including Levofloxacin, have been associated with an increased risk of central nervous system (CNS) effects, including convulsions, toxic psychoses, increased intracranial pressure (including pseudotumor cerebri). Fluoroquinolones may also cause central nervous system stimulation which may lead to tremors, restlessness, anxiety, lightheadedness, confusion, hallucinations, paranoia, depression, nightmares, insomnia, and, rarely, suicidal thoughts or acts. These reactions may occur following the first dose. If these reactions occur in patients receiving Levofloxacin, discontinue Levofloxacin and institute appropriate measures.
Exacerbation of Myasthenia Gravis
Fluoroquinolones, including Levofloxacin, have neuromuscular blocking activity and may exacerbate muscle weakness in patients with myasthenia gravis. Post marketing serious adverse reactions including deaths and requirement for ventilator support, have been associated with fluoroquinolone use in patients with myasthenia gravis. Avoid Levofloxacin in patients with a known history of myasthenia gravis.
Other Serious and Sometimes Fatal Adverse Reactions
Other serious and sometimes fatal adverse reactions, some due to hypersensitivity, and some due to uncertain etiology, have been reported rarely in patients receiving therapy with fluoroquinolone, including Levofloxacin.
Hypersensitivity Reactions
Serious and occasionally fatal hypersensitivity and/or anaphylactic reactions have been reported in patients receiving therapy with fluoroquinolones, including Levofloxacin. These reactions often occur following the first dose. Some reactions have been accompanied by cardiovascular collapse, hypotension/shock, seizure, loss of consciousness, tingling, angioedema (including tongue, laryngeal, throat, or facial edema/swelling), airway obstruction (including bronchospasm, shortness of breath, and acute respiratory distress), dyspnea, urticaria, itching, and other serious skin reactions. Levofloxacin should be discontinued immediately at the first appearance of a skin rash or any other sign of hypersensitivity.
Hepatotoxicity
Post-marketing reports of severe hepatotoxicity (including acute hepatitis and fatal events) have been received for patients treated with Levofloxacin.
Clostridium difficile-Associated Diarrhea
Clostridium difficile-associated diarrhea (CDAD) has been reported with the use of nearly all antibacterial agents, including Levofloxacin, and may range in severity from mild diarrhea to fatal colitis. Treatment with antibacterial agents alters the normal flora of the colon leading to overgrowth of Clostridium difficile.
Patients with G-6- Phosphate Dehydrogenase Deficiency
Patients with latent or actual defects in glucose-6-phosphate dehydrogenase activity may be prone to haemolytic reactions when treated with quinolone antibacterial agents. Therefore, if Levofloxacin has to be used in these patients, potential occurrence of haemolysis should be monitored.
Prolongation of the QT Interval
Some fluoroquinolones, including Levofloxacin, have been associated with prolongation of the QT interval on the electrocardiogram and infrequent cases of arrhythmia. Rare cases of torsade de pointes have been spontaneously reported during post marketing surveillance in patients receiving fluoroquinolones, including Levofloxacin. Levofloxacin should be administered with caution in patients with known prolongation of the QT interval, patients with uncorrected hypokalemia, patients with cardiac disease
Blood Glucose Disturbances
As with other fluoroquinolone, disturbances of blood glucose, including symptomatic hyper-and hypoglycemia, have been reported with Levofloxacin, usually in diabetic patients receiving concomitant treatment with an oral hypoglycemic agent (e.g., glyburide) or with insulin.
Photosensitivity/Phototoxicity
Moderate to severe photosensitivity/phototoxicity reactions, the latter of which may manifest as exaggerated sunburn reactions (e.g., burning, erythema, exudation, vesicles, blistering, edema) involving areas exposed to light (typically the face, "V" area of the neck, extensor surfaces of the forearms, dorsa of the hands), can be associated with the use of fluoroquinolones after sun or UV light exposure.
Psychotic Reactions
Psychotic reactions have been reported in patients receiving quinolones, including Levofloxacin. In very rare cases these have progressed to suicidal thoughts and self-endangering behavior - sometimes after only a single dose of Levofloxacin.
Vision Disorders
If vision becomes impaired or any effects on the eyes are experienced, an eye specialist should be consulted immediately
Development of Drug-Resistant Bacteria
Prescribing Levofloxacin in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to provide benefit to the patient and increases the risk of the development of drug-resistant bacteria. The use of Levofloxacin, especially if prolonged, may result in overgrowth of non-susceptible organisms. If superinfection occurs during therapy, appropriate measures should be taken.

Side Effects of Levofloxacin Tablets

The most common side effects with an incidence rate of 3% or more were nausea, headache, diarrhea, insomnia, constipation and dizziness

Levofloxacin Tablets During Pregnancy

Levofloxacin tablets has been classified by the US FDA as Pregnancy Category C.
There are limited amount of data from the use of levofloxacin in pregnant women. Animal studies do not indicate direct or indirect harmful effects with respect to reproductive toxicity. However in the absence of human data and due to the experimental data suggest a risk of damage by fluoroquinolones to the weight-bearing cartilage of the growing organism, Levofloxacin tablets must not be used in pregnant women
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

Levofloxacin is contraindicated in breast-feeding women. There is insufficient information on the excretion of Levofloxacin in human milk; however other Fluoroquinolones are excreted in breast milk. In the absence of human data and due to the experimental data suggest a risk of damage by fluoroquinolones to the weight-bearing cartilage of the growing organism, Levofloxacin tablets must not be used in breast-feeding women.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
