Imiquimod Cream is an immune response modifier (immunomodulator) which is topically administered for the treatment of sun damaged face and scalp, a condition called actinic keratosis. Imiquad Cream which contains Imiquimod as the active ingredient, is also used for the treatment of a particular skin cancer called superficial basal cell carcinoma, when surgical procedures are not a suitable treatment option.
Imiquad Cream are also prescribed as topical therapy for the effective treatment of genital warts (small lumps) that arise externally on the genitals and around the anus caused due to infection by Human Papilloma Virus (HPV).
Imiquad Cream cannot, however, be used as cure for the anogenital warts. The administration of generic Imiquimod Cream strengthens the immune system to fight the infection in skin cells.

Active Pharmaceutical Ingredient
The active ingredient present in Imiquad Cream is Imiquimod. Each 0.25 g sachet of Imiquad Cream contains 12.5 mg of Imiquimod.
The inactive ingredients present in Imiquad cream are benzyl alcohol, cetyl alcohol, glycerin, isostearic acid, methylparaben, polysorbate 60, propylparaben, purified water, sorbitan monostearate, stearyl alcohol, white petrolatum, and xanthan gum.
What is Imiquimod Cream Used For?
Imiquad 5% Cream, manufactured by Gracewell (Glenmark Pharmaceuticals Ltd.) in the strength of 5%, works as an immunomodulator which strengthens the immune system by activating the various defense mechanisms of the body.
Imiquad Cream are commonly used as topical therapy for the treatment of actinic keratosis, a condition characterized by the presence of flat, scaly growths on the face and scalp which develops in immunocompetent individuals due to excessive sun exposure. Imiquimod Cream are used as therapy for the treatment of basal cell carcinoma, a type a skin cancer which has the typical symptoms on the trunk, neck, arms, hands, legs, or feet and warts on the skin of the genital and anal areas.
Generic Imiquimod cream are administered as treatment to basal cell carcinoma patients when surgery is not a feasible treatment option. Imiquimod Cream are also used for the treatment of genital warts which arise on external sites on the body in patients who are at least 12 years due to infection with the Human Papilloma Virus (HPV).
Imiquad Cream can only be used for the treatment of genital warts because it shrinks the size of the warts but it cannot be used to cure the warts or to prevent the transmission of warts or HPV virus to your partner.
Imiquad Dosage - How to Apply Imiquimod Cream?
Generic Imiquimod Cream is meant for topical administration only and should be used exactly as suggested by your doctor. Imiquimod Cream should be applied only on the skin and not near sensitive areas like near the eyes, lips, nostrils, vagina, and anus. If you apply Imiquimod Cream to any of these areas, you should rinse these areas immediately with water.
Avoid the application of Imiquad Cream to skin surfaces which have wounds, blisters, infection, sunburn, cuts, or burn marks. Ideally, you should wait for these injuries to heal before you apply Imiquimod Cream. Make sure that the site of treatment is not concealed with a tight bandage or dressing, unless your doctor has recommends it. You can, however, use cotton gauze dressing to cover the area. You should be systematic about the schedule for the medication such that you take Imiquad Cream at the same time (preferably at bedtime) on medication days, to get the maximum benefit of the medication. Imiquad Cream should not be administered to children below the age of 12 years.
Continue taking Imiquad Cream for the recommended treatment period without missing any doses. Patients on a course of Imiquimod Cream can be prescribed a total of 36 packets of Imiquimod Cream for a treatment period of 16 weeks. Imiquad Cream is available in the form of single-use sachets.
Your doctor will suggest the amount of Imiquimod Cream you need to use from each sachet during an application, which may be the entire content of the sachet or just a small amount. Make sure that you use a new sachet of Imiquad Cream for every application and discard the any unused and partially used sachets of Imiquad Cream.
Before applying Imiquad Cream, wash your hands to avoid the contact of germs or dirt with the contents of the cream. Then, wash the area to which you will apply Imiquimod Cream using a mild soap and water. Allow it to dry for at least 10 minutes before you apply Imiquimod Cream.
Open the sachet of Imiquimod Cream and smear a thin layer of cream on the site of treatment. Rub the cream into the skin with your hands until all of the cream has been absorbed into the skin. Wash your hands off the Imiquimod Cream.
Allow the cream to remain on the skin surface for the recommended time period. Do not bathe, shower, or go for a swim when you have applied Imiquimod Cream on your skin. That’s why it is preferable to use Imiquad Cream at bedtime. After 8 hours, wash the site of treatment using water and a mild soap to remove the traces of any cream. Your doctor may need to monitor your progress continuously if you have a severe infection.
Imiquimod Cream for Actinic Keratosis
If you are using Imiquad cream for the treatment of actinic keratosis, your doctor may advise you to use the cream twice a week. The two weekly applications should have a gap of 3 to 4 days (e.g., Monday and Thursday or Tuesday and Friday). The area of application should not be larger than the size of your forehead or cheek (approximately 2 inches by 2 inches). Allow Imiquimod Cream to remain on the skin surface for approximately 8 hours. Continue the application of Imiquimod cream for the entire treatment period of 16 weeks, even if all the actinic keratosis clears out, unless your doctor advises you differently.
Imiquimod Cream for Basal Cell Carcinoma
If you are using Imiquad cream for the treatment of superficial basal cell carcinoma, your doctor may advise you to use the cream 5 times a week (e.g., Monday through Friday). Apply the Imiquimod Cream to the area which is affected by basal cell carcinoma and the adjacent areas. Allow Imiquimod Cream to remain on the skin surface for approximately 8 hours. Continue the application of Imiquad cream for the entire treatment period of 6 weeks, even if all the actinic keratosis clears out, unless your doctor advises you differently.
Imiquimod Cream for Genital Warts
If you are using Imiquad cream for the treatment of genital and anal warts, your doctor may advise you to use the cream 3 times a week (e.g., Monday, Wednesday, and Friday or Tuesday, Thursday, and Saturday). Allow Imiquimod Cream to remain on the skin surface for approximately 8 hours. Continue the application of Imiquimod cream for the entire treatment period of maximum of 16 weeks.
Patients using Imiquimod Cream for treatment of anogenital warts should wear cotton underwear during the course of treatment.
Avoid any type of sexual activity (oral, anal, genital) when you have applied Imiquad Cream on your skin. Imiquimod Cream can weaken the integrity of condoms and vaginal diaphragms. Men who have undergone circumcision and have genital warts under the foreskin are advised to pull back the penile foreskin and clean the area daily and also before each application of Imiquad Cream.
It is usually recommended that patients with genital warts avoid sexual contact until the warts have cleared completely. Women who are undergoing treatment with Imiquad Cream for genital warts around the vagina are advised to apply the cream only externally and avoid the sensitive inner layers of vaginal tissue.
Please consult your doctor for exact dosing instructions. Imiquad Cream (Generic Imiquimod 5% Cream) should be administered in the dose prescribed by the doctor and for the duration recommended by the doctor.
Missed Dose
If you miss your regular dose of Imiquimod Cream, take it as soon as you remember. You can continue your regular schedule the next day. If you remember the missed dose Imiquimod Cream the next day, take your regular dose and skip the missed dose.
Do not take a double dose to compensate the missed dose of Imiquad Cream.
Imiquad Mechanism of Action – How Does Imiquimod Cream Work?
Imiquimod Cream is an immunomodulator (immune response modifier) which is used for the treatment of different skin conditions such as genital warts, basal cell carcinoma, and actinic keratosis.
Imiquimod Cream works by activating the immune cells of the skin, which causes them to initiate the defense mechanism of the body's immune system. Imiquimod is thought to trigger the release of chemicals such as cytokines and interferons which enable the body to fight infections and also attack harmful cells, viruses, and cancers.
Storage
Store Imiquad Cream at controlled room temperature (20°C to 25°C), protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not use Imiquimod Cream (Imiquad) beyond its expiration date printed on the blister pack. Maintain the packets of Imiquimod Cream in the unopened state. Open the pack of Imiquimod Cream only when you need to apply it. Throw away the remaining contents of the sachet after use. Never reuse an opened pack.
Keep the pack of Generic Imiquimod Cream away from the reach of children and pets. Do not discard Imiquad Cream along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Imiquad Cream is contraindicated in patients with a known hypersensitivity to Imiquimod or any inactive ingredient present in Imiquad Cream. Imiquad Cream should not be used on children below the age of 18 years.
Patients who have warts present inside the genitals (urethra, vagina, or cervix) or anus should not be administered Imiquad Cream.
Overdose
Topical overdosing of Aldara Cream could result in an increased incidence of severe local skin reactions and may increase the risk for systemic reactions.
The most clinically serious adverse event reported following multiple oral Imiquimod doses of greater than 200 mg (equivalent to Imiquimod content of more than 16 packets) was hypotension, which resolved following oral or intravenous fluid administration.
Warnings and Precautions to be taken when using Imiquimod Cream
Medical History
Before beginning the treatment with Imiquad Cream (Generic Imiquimod Cream), discuss your personal and family medical history with your doctor. Inform your doctor if you have any existing medical conditions which may affect the treatment with Imiquimod Cream such as unusual sensitivity to sunlight, any skin disease such as psoriasis, graft vs. host disease, recent surgery to the affected area, a weak immune system, an autoimmune disorder (rheumatoid arthritis, lupus), any condition that affects the immune system (such as human immunodeficiency virus (HIV) or acquired immunodeficiency syndrome (AIDS), recently received a bone marrow transplant or cord blood transplant.
Allergy
Please inform your doctor if you have an allergy or have had an allergic reaction in the past to Imiquimod or any inactive ingredient in Imiquad Cream (Generic Imiquimod Cream). Please tell your doctor if you have any other allergies.
Imiquimod Interactions
Before beginning the topical therapy of Imiquad Cream, please tell your doctor about all your present medications including prescription drugs, over the counter medications, mineral supplements, herbal products, and vitamins. Generic Imiquimod Cream are known to have drug interactions with certain other medications. Please inform your doctor if you are taking corticosteroids (e.g., prednisone), denosumab, Echinacea, leflunomide, live vaccines (e.g., BCG, yellow fever), immunosuppressant (tacrolimus, cyclosporine, methotrexate), natalizumab, any other dermatological medication.
Imiquimod Cream - Sexually Transmitted Diseases
Imiquimod Cream cannot be deemed as a cure for genital warts and it may not prevent the transmission of HPV virus or the warts to your partner through vaginal, anal, or oral sex. Fresh warts may also arise during treatment with Imiquimod Cream. For successful therapeutic results, continue using Imiquad Cream for the entire treatment period as suggested by your doctor. Imiquimod will also not protect against the transmission of sexually transmitted diseases such as chlamydia, gonorrhea, herpes, HIV, syphilis, and trichomoniasis.
Side Effects of Imiquad Cream
The following side effects have been reported during the course of treatment with Imiquad Cream (Generic Imiquimod Cream):
- Cardiovascular System- chest pain, irregular heartbeat
- Central Nervous System- headache, dizziness
- Respiratory System- upper respiratory tract infections, sinus infection, stuffy nose, sneezing, sore throat, swollen lymph glands, trouble in breathing
- Gastrointestinal System- diarrhea, nausea, heartburn
- Genitourinary System- vaginal itching or discharge, difficulty or pain during urination
- Musculoskeletal System- back pain, muscle weakness or pain
- Dermatological System- flaking, scaling, dryness, or thickening of the skin, blisters, scabs, or bumps on the skin; skin breakdown or sores that may have drainage, especially during the first week of treatment
- Hypersensitivity- redness, itching, burning, bleeding, swelling, stinging, or pain in the treated area, fever, chills, changes in skin color
- General- tiredness, cold sores, loss of appetite, hair loss and increase or decrease in skin pigmentation at the site of treatment

Imiquimod Cream During Pregnancy and Breastfeeding
Imiquimod Cream has been classified by the US FDA as Pregnancy Category C.
It is not known if pregnant women are allowed to use Imiquad Cream. There are no adequate and well-controlled studies in pregnant women. Imiquad Cream should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
It is not known if Imiquimod is secreted into breastmilk. Nursing mothers should be careful if they are breastfeeding an infant during the course of Imiquad Cream since many drugs are excreted in human milk. It is usually preferred that pregnant women and breast feeding mothers, who are using Imiquimod Cream for their skin conditions, should check with their doctor and appreciate its therapeutic benefits and associated risks.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
