Active Pharmaceutical Ingredient
The active ingredient present in Gemfos tablets is Risedronate Sodium Hemi-Pentahydrate. Each Gemfos tablet contains Risedronate Sodium Hemi-Pentahydrate equivalent to 150 mg of Risedronate Sodium.
Uses of Risedronate Sodium tablets (Gemfos)
Risedronate Sodium tablets (Gemfos) are used to prevent and treat osteoporosis (thinning of the bone) in women after menopause. Risedronate tablets may also be used to increase bone mass in men who have osteoporosis, and in men and women to prevent and treat osteoporosis caused by long-term use of corticosteroids (cortisone-like medicine). Risedronate tablets are also used to treat Paget's disease of the bone.
Gemfos Dosage - How to Take Risedronate Sodium Tablets?
You should use Gemfos tablets exactly as recommended by your doctor. Your doctor may suggest a dosage of Risedronate tablets depending on the severity of medical condition, body weight, other medical conditions, and current medications.
Gemfos must only be taken with plain water. Plain water is the only drink that should be taken with Gemfos tablets. Please note that some mineral waters or water from regional areas may have a higher concentration of calcium and therefore should not be used.
Gemfos must be taken 30 minutes before the first food or drink other than water. To facilitate delivery to the stomach, Actonel should be taken in an upright position and the patient should avoid lying down for 30 minutes. Patients should not chew or suck on the tablet because of the potential for oropharyngeal irritation.
Osteoporosis:
Gemfos 150 mg tablets should be taken orally once a month. The tablet should be taken on the same date each month.

Missed Dose

If the next month’s scheduled doses of Actonel 150 mg are within 7 days, patients should wait until their next month’s scheduled doses and then continue taking Actonel 150 mg as originally scheduled.
Gemfos Mechanism of Action – How Does Risedronate Work?
Gemfos (Generic Risedronate Sodium tablets) is a bisphosphonate medication which slows down the process of bone loss. The structure and dimensions of the human bone constantly undergoes changes in shape and form. The structure of the bone is dependent on the storage of calcium and phosphorous minerals in the form of a protein network. The human bone consists of specialized cells called osteoblasts and osteoclasts, which are responsible for the continuous accumulation and elimination of calcium and phosphorous minerals in the bone. Osteoclasts regulate the breakdown of old bone and osteoblasts are responsible for the formation of new bone.
Women undergoing the phase of menopause experience a decrease in the blood levels of the female hormone called oestrogen. This estrogen decline results in an increase in the rate of bone breakdown by the osteoclasts, eventually leading to a loss in bone density. The rate of bone loss is especially high during the first ten years immediately following menopause, which may lead to the advancement of osteoporosis - a medical condition characterized by weak and brittle bones which are prone to fractures.
Risedronate exerts its mechanism of action by forming a strong attachment to the bone structure, thus effectively inhibiting the removal of calcium performed by the osteoclasts. This osteoclast inhibition allows Risedronate to prevent bone breakdown by increasing the thickness of bones i.e. bone mineral density and results in stronger bones which are less prone to damage.
Storage Instructions for Gemfos Tablets
Store Gemfos tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Risedronate Sodium tablets (Gemfos) beyond its expiration date printed on the blister pack. Keep the pack of Gemfos tablets away from the reach of children and pets. Do not discard Risedronate tablets along with domestic waste or sewage. Contraindications
The treatment with Risedronate Sodium tablets is contraindicated in patients who are allergic to Risedronate Sodium or any inactive ingredients present in Gemfos tablets (Generic Risedronate), who cannot stand in an upright position for at least 30 minutes, who have low blood calcium levels, abnormalities of the esophagus (such as stricture and achalasia) that delay esophageal emptying or who have severe kidney impairment.
Overdose
Decreases in serum calcium following substantial overdose may be expected in some patients. Signs and symptoms of hypocalcemia may also occur in some of these patients.
Milk or antacids containing calcium, magnesium, and aluminum may be given to bind Gemfos(film-coated tablets) and reduce absorption of the drug; the impact of this intervention for Gemfos DR (delayed-release tablets) has not been evaluated. The Gemfos DR formulation is less sensitive to the binding effects of divalent cations.
In cases of substantial overdose, gastric lavage may be considered to remove unabsorbed drug if performed within 30 minutes of ingestion. Standard procedures that are effective for treating hypocalcemia, including the administration of calcium intravenously, would be expected to restore physiologic amounts of ionized calcium and to relieve signs and symptoms of hypocalcemia.

Upper Gastrointestinal Adverse Reactions
Upper Gastrointestinal Adverse Reactions can occur. Instruct patients to follow dosing instructions. Gemfos like other bisphosphonates may cause local irritation of the upper GI mucosa. Since some bisphosphonates have been associated with oesophagitis and oesophageal ulcerations and gastroduodenal ulceration, doctors should therefore be alert to any signs or symptoms signalling a possible oesophageal reaction, especially in patients with a history of upper GI disease or who are using NSAIDS or aspirin concomitantly. Discontinue use if new or worsening symptoms occur
Hypocalcemia
Hypocalcemia may worsen and must be corrected prior to use
Osteonecrosis
Osteonecrosis of the Jaw has been reported. Known risk factors for osteonecrosis of the jaw include invasive dental procedures (for example, tooth extraction, dental implants, boney surgery), diagnosis of cancer, concomitant therapies (for example, chemotherapy, corticosteroids, angiogenesis inhibitors), poor oral hygiene, and co-morbid disorders.
Severe Bone, Joint, Muscle Pain
Severe Bone, Joint, Muscle Pain may occur. Discontinue use if severe symptoms develop
Atypical Femur Fractures
Atypical Femur Fractures have been reported. Patients with new thigh or groin pain should be evaluated to rule out a femoral fracture. These fractures can occur anywhere in the femoral shaft from just below the lesser trochanter to above the supracondylar flare and are traverse or short oblique in orientation without evidence of comminution.
Osteomalacia
The potential for Risedronate to induce osteomalacia was investigated in the Schenk rat assay. This assay is based on histologic examination of the epiphyses of the growing rats after drug treatment. Risedronate did not interfere with bone mineralisation even at the highest dose tested
Use in Renal impairment
Enteric-coated Actonel is not recommended for use in patients with severe renal impairment.
Side Effects of Risedronate Sodium tablets (Gemfos)
Most common Risedronate Sodium tablets (Gemfos) side effects with an incidence rate greater than 5% include: diarrhea, influenza, arthralgia, back pain, and abdominal pain.
Hypersensitivity reactions (angioedema, generalized rash, bullous skin reactions, Stevens-Johnson syndrome, and toxic epidermal necrolysis), and eye inflammation (iritis, uveitis) have been reported rarely.
Risedronate Sodium (Gemfos Tablets) During Pregnancy
Risedronate Sodium has been classified by the US FDA as Pregnancy Category C.
Available data on the use of risedronate sodium tablets in pregnant women are insufficient to inform a drug-associated risk of adverse maternal or fetal outcomes. Discontinue risedronate sodium tablets when pregnancy is recognized.
In animal reproduction studies, daily oral administration of Risedronate to pregnant rats during organogenesis decreased neonatal survival and body weight at doses approximately 5 and 26 times, respectively, the highest recommended human daily dose of 30 mg (based on body surface area, mg/m2). A low incidence of cleft palate was observed in fetuses of dams treated at doses approximately equal to the 30 mg human daily dose.

There are no data on the presence of risedronate in human milk, the effects on the breastfed infant, or the effects on milk production.
A small degree of lacteal transfer occurred in nursing rats. The concentration of the drug in animal milk does not necessarily predict the concentration of drug in human milk. However, when a drug is present in animal milk, it is likely that the drug will be present in human milk.
