Celecoxib Capsules are indicated for the following indications:

Cobix Capsules (Generic Celecoxib Capsules) - Dosage
Generic Celecoxib capsules should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Cobix capsules depending on your age, medical requirement, body weight, other medical conditions, and current medications. The lowest effective dosage for shortest duration consistent with individual patient treatment goals is usually prescribed.
- Osteoarthritis (OA): 200 mg once daily or 100 mg twice daily
- Rheumatoid Arthritis (RA): 100 to 200 mg twice daily.
- Juvenile Rheumatoid Arthritis (JRA): 50 mg twice daily in patients 10-25 kg. 100 mg twice daily in patients more than 25 kg.
- Ankylosing Spondylitis (AS): 200 mg once daily single dose or 100 mg twice daily. If no effect is observed after 6 weeks, a trial of 400 mg (single or divided doses) may be of benefit.
- Acute Pain (AP) and Primary Dysmenorrhea (PD): For management of Acute Pain and Treatment of Primary Dysmenorrhea, the dosage is 400 mg initially, followed by an additional 200 mg dose if needed on the first day. On subsequent days, the recommended dose is 200 mg twice daily as needed.
Cobix capsules work effectively if the amount of Celecoxib is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Cobix capsules without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your regular Cobix capsules dose on time, take it whenever you remember.

Celecoxib Capsules Mechanism of Action
Celecoxib is a nonsteroidal anti-inflammatory drug that exhibits anti- inflammatory, analgesic, and anti-pyretic activities in animals. The mechanism of action of Celecoxib is believed to be related to inhibition of cyclooxygenase-2 (COX-2). COX-2 is expressed at high levels in inflamed tissues where it is induced by mediators of inflammation.
COX-2 also plays Cobix capsules (Celecoxib) physiological roles in a limited number of tissues, including those of the female reproductive tract, the kidney and possibly the vascular endothelium. COX-2 has the same catalytic activity as COX-1. COX-1 is expressed constitutively in most tissues including the gastrointestinal tract, kidney, lungs, brain, and platelets.
The prostaglandins produced by COX- 1 play key roles in the maintenance of physiological functions such as platelet aggregation and are among the factors that maintain the GI mucosal barrier.
Storage Instructions for Cobix Capsules
Store Cobix capsules at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Cobix capsules away from the reach of children and pets. Do not dispose along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Cobix capsules are contraindicated in patients with a hypersensitivity to Celecoxib or any inactive ingredient present in tablets. Cobix capsules are also contraindicated in the following cases:
History of asthma, urticaria, or other allergic-type reactions after taking aspirin or other NSAIDs
In the setting of CABG surgery
Overdose
There is no clinical experience of overdose. Single doses up to 1200 mg and multiple doses up to 1200 mg twice daily have been administered to healthy subjects for nine days without clinically significant adverse effects.
In the event of suspected overdose, appropriate supportive medical care should be provided e.g. by eliminating the gastric contents, clinical supervision and, if necessary, the institution of symptomatic treatment. Dialysis is unlikely to be an efficient method of medicinal products removal due to high protein binding.
Warnings and Precautions
Cardiovascular Thrombotic Events
Clinical trials of several cyclooxygenase-2 (COX-2) selective and nonselective NSAIDs of up to three years duration have shown an increased risk of serious cardiovascular (CV) thrombotic events, including myocardial infarction (MI) and stroke, which can be fatal. Based on available data, it is unclear that the risk for CV thrombotic events is similar for all NSAIDs.
Gastrointestinal Bleeding, Ulceration, and Perforation
NSAIDs, including Celecoxib cause serious gastrointestinal (GI) adverse events including inflammation, bleeding, ulceration, and perforation of the esophagus, stomach, small intestine, or large intestine, which can be fatal. These serious adverse events can occur at any time, with or without warning symptoms, in patients treated with Velecoxib.
Hepatotoxicity:
Inform patients of warning signs and symptoms of hepatotoxicity. Discontinue if abnormal liver tests persist or worsen or if clinical signs and symptoms of liver disease develop.
Hypertension:
Patients taking some antihypertensive medications may have impaired response to these therapies when taking NSAIDs. Monitor blood pressure
Heart Failure and Edema:
Avoid use of Cobix capsules in patients with severe heart failure unless benefits are expected to outweigh risk of worsening heart failure
Renal Toxicity:
Monitor renal function in patients with renal or hepatic impairment, heart failure, dehydration, or hypovolemia. Avoid use of Cobix capsules in patients with advanced renal disease unless benefits are expected to outweigh risk of worsening renal function
Anaphylactic Reactions:
Seek emergency help if an anaphylactic reaction occurs
Exacerbation of Asthma Related to Aspirin Sensitivity:
Cobix capsules are contraindicated in patients with aspirin-sensitive asthma. Monitor patients with preexisting asthma (without aspirin sensitivity).
Serious Skin Reactions:
Discontinue Cobix capsules at first appearance of skin rash or other signs of hypersensitivity.
Premature Closure of Fetal Ductus Arteriosus:
Avoid use in pregnant women starting at 30 weeks of gestation
Hematologic Toxicity:
Monitor hemoglobin or hematocrit in patients with any signs or symptoms of anemia

Most common side effects of Celecoxib capsules in arthritis trials (greater than 2% and more than placebo) are: abdominal pain, diarrhea, dyspepsia, flatulence, peripheral edema, accidental injury, dizziness, pharyngitis, rhinitis, sinusitis, upper respiratory tract infection, rash
Cobix Capsules During Pregnancy and Breastfeeding
Celecoxib (Cobix capsules) has been classified by the US FDA as Pregnancy Category C. Pregnancy category D from 30 weeks of gestation onward.
Use of NSAIDs, including celecoxib, can cause premature closure of the fetal ductus arteriosus and fetal renal dysfunction leading to oligohydramnios and, in some cases, neonatal renal impairment. Because of these risks, limit dose and duration of celecoxib use between about 20 and 30 weeks of gestation and avoid celecoxib use at about 30 weeks of gestation and later in pregnancy.
Celecoxib is contraindicated in pregnancy and in women who can become pregnant. If a woman becomes pregnant during treatment, celecoxib should be discontinued.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Nursing Considerations
Celecoxib is excreted in the milk of lactating rats at concentrations similar to those in plasma. Limited data from 3 published reports that included a total of 12 breastfeeding women showed low levels of Celecoxib in breast milk. The calculated average daily infant dose was 10-40 mcg/kg/day, less than 1% of the weight-based therapeutic dose for a two-year old-child.
A report of two breastfed infants 17 and 22 months of age did not show any adverse events. Caution should be exercised when Celecoxib is administered to a nursing woman. The developmental and health benefits of breastfeeding should be considered along with the mother’s clinical need for Celecoxib and any potential adverse effects on the breastfed infant from the Celecoxib or from the underlying maternal condition
