Arreno ER (Aspirin Plus Extended Release Dipyridamole Capsules) - Product Information

Arreno ER (Aspirin Plus Extended Release Dipyridamole Capsules) is a combination antiplatelet agent indicated to reduce the risk of stroke in patients who have had transient ischemia of the brain or completed ischemic stroke due to thrombosis.
Aspirin and Dipyridamole capsules may also be used for other conditions as determined by your healthcare provider.
Arreno ER Capsules, a combination of Aspirin and extended-release Dipyridamole are manufactured by Intas Pharmaceuticals Limited, India, in the strengths of 200 mg Dipyridamole in an extended-release form and 25 mg Aspirin.

The active ingredient present in Arreno ER capsules are Aspirin and extended-release Dipyridamole. Each hard gelatin capsule of Arreno ER contains 25 mg of Aspirin (as film coated tablet) and 200 mg of Dipyridamole (as extended release granules).

Uses of Arreno ER (Aspirin Plus Extended Release Dipyridamole Capsules)

Aspirin and Dipyridamole combination is used to prevent you from having a stroke. Strokes occur when a blood vessel in the brain is blocked by blood clots. This medicine will thin the blood and help prevent blood clots from forming. It is only given when there is a high risk that these problems may occur.

Arreno ER Capsules - Dosage

Generic Aspirin Plus Extended Release Dipyridamole capsules should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Arreno ER capsules depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Do not chew capsule
One capsule twice daily (morning and evening) with or without food. In case of intolerable headaches during initial treatment, switch to one capsule at bedtime and low-dose aspirin in the morning; resume twice a day dosing within one week.
This capsule is not interchangeable with the individual components of Aspirin and Dipyridamole tablets
Arreno ER capsules work effectively if the amount of Aspirin Plus Extended Release Dipyridamole is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Arreno ER capsules without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Arreno ER capsule dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Aspirin Plus Extended Release Dipyridamole Capsules Mechanism of Action

The combination of aspirin and extended-release dipyridamole (e.g., Aggrenox) works through two complementary antiplatelet mechanisms to prevent blood clots and reduce the risk of stroke.

Aspirin Mechanism of Action
Aspirin is an antiplatelet agent that works by irreversibly inhibiting the cyclooxygenase (COX-1) enzyme in platelets. This action blocks the production of thromboxane A2 (TXA2), a powerful substance that causes platelets to clump together (aggregate) and blood vessels to constrict. Because platelets lack a nucleus and cannot synthesize new enzymes, this inhibitory effect lasts for the entire lifespan of the platelet (about 7-10 days).

Dipyridamole Mechanism of Action
Extended-release dipyridamole has two primary antiplatelet actions:
Inhibition of Adenosine Uptake: Dipyridamole blocks the uptake of adenosine into platelets, endothelial cells, and red blood cells. This increases local concentrations of adenosine, which then acts on platelet A2-receptors, stimulating a rise in cyclic adenosine monophosphate (cAMP) levels. High levels of cAMP inhibit platelet activation and aggregation in response to various stimuli like ADP and collagen.
Phosphodiesterase (PDE) Inhibition: Dipyridamole also inhibits phosphodiesterase enzymes, which are responsible for breaking down cAMP and cyclic guanosine monophosphate (cGMP). By preventing this breakdown, it further increases cAMP and cGMP levels within platelets, which helps to keep them in an inactive state and prevents them from sticking together.
Combined Effect
The use of both drugs together provides additive antiplatelet effects through these different biochemical pathways, which has been shown to be more effective at reducing the risk of stroke than either agent alone. This dual mechanism helps to maintain more normal blood flow and significantly reduces the formation of harmful blood clots in patients who have had a previous ischemic stroke or transient ischemic attack (TIA).

Store Arreno ER Capsules at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Arreno ER Capsules past its expiration date printed on the blister pack. Keep the pack of Aspirin Plus Extended Release Dipyridamole Capsules away from the reach of children and pets. Do not dispose Arreno ER Capsules along with domestic waste or sewage. Contraindications

Arreno ER Capsules are contraindicated in patients with a hypersensitivity to Aspirin, Dipyridamole or any inactive ingredient present in tablets. Arreno ER Capsules are also contraindicated in the following cases:
Patients with known allergy to NSAIDs
Patients with the syndrome of asthma, rhinitis, and nasal polyps Do not use aspirin in children or teenagers with viral infections because of the risk of Reye syndrome.

Arreno ER (Aspirin Plus Extended Release Dipyridamole Capsules) Overdose

Because of the dose ratio of Dipyridamole to Aspirin, overdosage of Arreno ER is likely to be dominated by signs and symptoms of Dipyridamole overdose. In case of real or suspected overdose, seek medical attention or contact a Poison Control Center immediately. Careful medical management is essential.
Based upon the known hemodynamic effects of dipyridamole, symptoms such as warm feeling, flushes, sweating, restlessness, feeling of weakness and dizziness may occur. A drop in blood pressure and tachycardia might also be observed.
Salicylate toxicity may result from acute ingestion (overdose) or chronic intoxication. Severity of aspirin intoxication is determined by measuring the blood salicylate level. The early signs of salicylic overdose (salicylism), including tinnitus (ringing in the ears), occur at plasma concentrations approaching 200 µg/mL. In severe cases, hyperthermia and hypovolemia are the major immediate threats to life.
Treatment of overdose consists primarily of supporting vital functions, increasing drug elimination, and correcting acid-base disturbances. Consider gastric emptying and/or lavage as soon as possible after ingestion, even if the patient has vomited spontaneously.
After lavage and/or emesis, administration of activated charcoal as a slurry may be beneficial if less than 3 hours have passed since ingestion. Charcoal absorption should not be employed prior to emesis and lavage. Follow acid-base status closely with serial blood gas and serum pH measurements. Maintain fluid and electrolyte balance.
Administer replacement fluid intravenously and augment with correction of acidosis. Treatment may require the use of a vasopressor. Infusion of glucose may be required to control hypoglycemia.

Warnings and Precautions

Risk of Bleeding
Aspirin Plus Extended Release Dipyridamole Capsules increases the risk of bleeding. Risk factors for bleeding include the use of other drugs that increase the risk of bleeding (e.g., anticoagulants, antiplatelet agents, heparin, anagrelide, fibrinolytic therapy, and chronic use of NSAIDs).
The risk of GI bleeding is increased, especially in patients who are heavy alcohol users, have a history of peptic ulcer, or have coagulation abnormalities due to liver disease or vitamin K deficiency
Intracranial Hemorrhage
As with other antiplatelets, there is a risk of intracranial hemorrhage. In European Stroke Prevention Study-2 (ESPS2), the annualized event rate for intracranial hemorrhage was 0.39%/year in the aspirin and extended-release dipyridamole capsules group, 0.26%/year in the extended-release dipyridamole (ER-DP) group, 0.24%/year in the aspirin (ASA) group, and 0.29%/year in the placebo groups.
Gastrointestinal (GI) Side Effects
GI side effects include stomach pain, heartburn, nausea, vomiting, and gross GI bleeding. Although minor upper GI symptoms, such as dyspepsia, are common and can occur anytime during therapy, physicians should remain alert for signs of ulceration and bleeding, even in the absence of previous GI symptoms. Inform patients about the signs and symptoms of GI side effects and what steps to take if they occur .
Peptic Ulcer Disease
Avoid using aspirin in patients with a history of active peptic ulcer disease, which can cause gastric mucosal irritation and bleeding.
Alcohol Warning
Because aspirin and extended-release Dipyridamole capsules contains Aspirin, counsel patients who consume three or more alcoholic drinks every day about the bleeding risks involved with chronic, heavy alcohol use while taking Aspirin.
Renal Failure
Avoid aspirin in patients with severe renal failure (glomerular filtration rate less than 10 mL/minute)
Hepatic Insufficiency
Elevations of hepatic enzymes and hepatic failure have been reported in association with Dipyridamole administration.

Coronary Artery Disease
Dipyridamole has a vasodilatory effect. Chest pain may be precipitated or aggravated in patients with underlying coronary artery disease who are receiving dipyridamole.

Aspirin Plus Extended Release Dipyridamole Capsules During Pregnancy

Aspirin Plus Extended Release Dipyridamole has been classified by the US FDA as Pregnancy Category D.
Available data from published studies and postmarketing experience with Aspirin and extended-release Dipyridamole capsules use during pregnancy have not identified a clear association between aspirin and extended-release dipyridamole capsules use and major birth defects, miscarriage, or adverse maternal or fetal outcomes.
Aspirin and extended-release dipyridamole capsule contains low-dose aspirin which is an NSAID. In animal reproduction studies, there were adverse developmental effects with administration of aspirin in rats and rabbits at doses about 66 and 44 times, respectively, the human exposure at the maximum recommended daily dose of Aspirin-Dipyridamole. Reproduction studies with dipyridamole in mice, rabbits, and rats have revealed no evidence of harm to the fetus up to doses about 25 times the maximum recommended daily human dose of Aspirin-Dipyridamole.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

Based on data from a clinical lactation study in breastfeeding women taking low-dose aspirin, the metabolite salicylic acid is present in human milk in low levels. Dipyridamole is also present in human milk. There is no information on the effects of Aspirin and extended-release Dipyridamole capsules or Dipyridamole on the breastfed infant or on milk production.
There is insufficient information to determine the effects of aspirin on the breastfed infant and no information on the effects of aspirin on milk production.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
