Rivaroxaban is used to treat or prevent blood clots (venous thromboembolism, or VTE). Blood clots can occur in the legs (deep vein thrombosis, DVT) or the lungs (pulmonary embolism, PE).
Rivaroxaban is sometimes used to lower your risk of a blood clot coming back after you have received treatment for blood clots.
Rivaroxaban is also given together with aspirin to lower the risk of stroke, heart attack, or other serious heart and blood circulation problems in adults with coronary artery disease (clogged arteries) or peripheral artery disease.
Rpigat tablets are manufactured by Natco Pharma Limited, India, and are available in the strengths of 15 mg and 20 mg.

Active Pharmaceutical Ingredient
The active ingredient present in Rpigat tablets is Rivaroxaban. Each film-coated tablet of Rpigat contains 15 mg or 20 mg of Rivaroxaban.
Uses of Rpigat Tablets (Generic Rivaroxaban tablets)
Rpigat Tablets contain the active substance Rivaroxaban.Rivaroxaban belongs to a group of medicines called antithrombotic agents. It works by blocking a blood clotting factor (factor Xa) and so reducing the risk of blood clots.
This medicine is used in adults to:
• prevent blood clots in the brain (stroke) and other blood vessels in your body if you have a form of irregular heart rhythm called non-valvular atrial fibrillation.
• treat blood clots in the veins of your legs (deep vein thrombosis) and in the blood vessels of your lungs (pulmonary embolism), and to prevent such blood clots from re-occurring.
This medicine is used in children and adolescents below 18 years and with a body weight of 30 kg or more to:
• treat and prevent re-occurrence of blood clots in the veins or in the blood vessels of the lungs, after having had initial treatment of at least 5 days with injectable medicines used to treat blood clots.
Rpigat Tablets (Generic Rivaroxaban tablets) - Dosage
Generic Rivaroxaban tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Rpigat tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Reduction of Risk of Major Cardiovascular Events (CV Death, MI, and Stroke) in CAD
2.5 mg twice daily, plus aspirin (75 to 100 mg) once daily. No dose adjustment needed based on CrCl. Take with or without food.
Reduction of Risk of Major Thrombotic Vascular Events in PAD, Including Patients after Lower Extremity Revascularization due to Symptomatic PAD
2.5 mg twice daily, plus aspirin (75 to 100 mg) once daily. When starting therapy after a successful lower extremity revascularization procedure,initiate once hemostasis has been established.Take with or without food. No dose adjustment needed based on CrCl
Rpigat tablets work effectively if the amount of Rivaroxaban is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Rpigat tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your regular Rpigat tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.
Rivaroxaban tablets Mechanism of Action
Rivaroxaban (Rpigat Tablets) is a direct oral anticoagulant that works by inhibiting Factor Xa, a key enzyme in the blood clotting process. By directly and selectively blocking Factor Xa, it prevents the conversion of prothrombin to thrombin, which is the final step in forming a blood clot.
This mechanism prevents the amplification of the coagulation cascade and the formation of thrombi.
Mechanism of action
Direct Factor Xa inhibition:
Rivaroxaban directly binds to and inhibits both free and clot-bound Factor Xa, a critical enzyme in the coagulation cascade.
Coagulation cascade:
Factor Xa is the rate-limiting step in thrombin generation. By inhibiting Factor Xa, rivaroxaban prevents the formation of thrombin (Factor IIa).
Thrombin generation:
The inhibition of thrombin, a key enzyme that converts fibrinogen to fibrin to form a clot, stops the amplification of the clotting process.
No cofactors needed:
Unlike indirect Factor Xa inhibitors, rivaroxaban does not require cofactors like antithrombin to exert its anticoagulant effect.
Selective inhibition:
It is highly selective for Factor Xa, showing over 10,000-fold greater selectivity for Factor Xa than for other related serine proteases like thrombin.
Platelet aggregation:
Rivaroxaban has no direct effect on platelet aggregation.
Storage Instructions for Rpigat Tablets
Store Rpigat tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Rpigat Tablets past its expiration date printed on the blister pack. Keep the pack of Generic Rivaroxaban tablets away from the reach of children and pets. Do not dispose Rpigat Tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Rpigat tablets are contraindicated in patients with a hypersensitivity to Rivaroxaban or any inactive ingredient present in tablets. Rpigat tablets are also contraindicated in the following cases:
active pathological bleeding
Overdose
Overdose of Rivaroxaban tablets may lead to hemorrhage. Discontinue rivaroxaban tablets and initiate appropriate therapy if bleeding complications associated with overdosage occur.
Rivaroxaban systemic exposure is not further increased at single doses greater than 50 mg due to limited absorption.
The use of activated charcoal to reduce absorption in case of rivaroxaban overdose may be considered. Due to the high plasma protein binding, rivaroxaban is not dialyzable.
Warnings and Precautions
Risk of bleeding:
Rpigat tablets can cause serious and fatal bleeding. An agent to reverse the activity of rivaroxaban is available.
Pregnancy-related hemorrhage:
Use Rpigat tablets with caution in pregnant women due to the potential for obstetric hemorrhage and/or emergent delivery.
Prosthetic heart valves:
Rpigat tablets use not recommended.
Increased Risk of Thrombosis in Patients with Triple Positive Antiphospholipid Syndrome:
Rpigat tablets use not recommended.
Spinal/Epidural Anesthesia or Puncture
When neuraxial anesthesia (spinal/epidural anesthesia) or spinal puncture is employed, patients treated with anticoagulant agents for prevention of thromboembolic complications are at risk of developing an epidural or spinal hematoma which can result in long-term or permanent paralysis.
Use in Patients with Renal Impairment
Discontinue rivaroxaban tablets in patients who develop acute renal failure while on treatment
Use in Patients with Hepatic Impairment
No clinical data are available for adult patients with severe hepatic impairment. Avoid use of rivaroxaban tablets in patients with moderate (Child-Pugh B) and severe (Child-Pugh C) hepatic impairment or with any hepatic disease associated with coagulopathy since drug exposure and bleeding risk may be increased
Risk of Pregnancy-Related Hemorrhage
In pregnant women, rivaroxaban should be used only if the potential benefit justifies the potential risk to the mother and fetus. Rivaroxaban dosing in pregnancy has not been studied.
Side Effects of Rivaroxaban (Rpigat Tablets)
The most common Rivaroxaban (Rpigat Tablets) Side Effects with an incidence rate of 5% in adult patients was bleeding.
The most common Side Effects with an incidence rate of 10% or more in pediatric patients were bleeding, cough, vomiting, and gastroenteritis.
Rivaroxaban Tablets During Pregnancy
Rivaroxaban (Rpigat tablets) has been classified by the US FDA as Pregnancy Category C.
The limited available data on rivaroxaban in pregnant women are insufficient to inform a drug-associated risk of adverse developmental outcomes.
Use rivaroxaban with caution in pregnant patients because of the potential for pregnancy related hemorrhage and/or emergent delivery. The anticoagulant effect of rivaroxaban cannot be reliably monitored with standard laboratory testing.
Consider the benefits and risks of rivaroxaban for the mother and possible risks to the fetus when prescribing rivaroxaban to a pregnant woman.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
Nursing Considerations
Rivaroxaban has been detected in human milk. There are insufficient data to determine the effects of rivaroxaban on the breastfed child or on milk production.
Rivaroxaban and/or its metabolites were present in the milk of rats. The developmental and health benefits of breastfeeding should be considered along with the mother’s clinical need for rivaroxaban and any potential adverse effects on the breastfed infant from Rivaroxaban or from the underlying maternal condition
Do not use this medication without telling your doctor if you are breast-feeding a baby.
