Acivir Cream (Generic Acyclovir Cream) is a herpes simplex virus (HSV) deoxynucleoside analogue DNA polymerase inhibitor indicated for the treatment of recurrent herpes labialis (cold sores) in immunocompetent adults and adolescents 12 years of age and older.
Acivir cream is manufactured by Cipla Ltd., India in the strengths of 5% and is available in a 10 grams tube.

Active Pharmaceutical Ingredient

The active ingredient present in Acivir cream is Acyclovir (Aciclovir). Each 10 grams tube of Acivir 5% cream contains Aciclovir BP 5% w/w in a cream base. Each gram of Acivir Cream, 5% contains 50 mg of Acyclovir.

Uses of Acyclovir Cream

Acyclovir cream (Acivir cream) is used to treat the symptoms of herpes simplex virus infections of the skin, mucous membranes, and genitals (sex organs).
Although topical acyclovir will not cure herpes simplex, it may help relieve the pain and discomfort and may help the sores (if any) heal faster. Topical acyclovir may also be used for other conditions as determined by your doctor.

Acyclovir cream (Acivir cream) - Dosage

Generic Acyclovir cream should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Acivir cream depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Acivir Cream should be applied 5 times per day for 4 days. Therapy should be initiated as early as possible following onset of signs and symptoms (i.e. during the prodrome or when lesions appear). For adolescents 12 years of age and older, the dosage is the same as in adults.
Acivir cream works effectively if the amount of Acyclovir is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you apply the cream at about the same time every day, to get the maximum benefit of the medication. Continue using Acivir cream without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

Acyclovir cream Mechanism of Action – How Does Acivir Cream Work?

Acyclovir is an antiviral drug active against a-herpesviruses and hydrocortisone is an anti-inflammatory drug.
The inhibitory activity of acyclovir is highly selective due to its affinity for the enzyme thymidine kinase (TK) encoded by HSV and VZV. This viral enzyme converts acyclovir into acyclovir monophosphate, a nucleotide analogue. The monophosphate is further converted into diphosphate by cellular guanylate kinase and into triphosphate by a number of cellular enzymes. In vitro, acyclovir triphosphate stops replication of herpes viral DNA. This is accomplished in three ways: 1) competitive inhibition of viral DNA polymerase, 2) incorporation into and termination of the growing viral DNA chain, and 3) inactivation of the viral DNA polymerase. The greater antiviral activity of acyclovir against HSV compared to VZV is due to its more efficient phosphorylation by the viral TK.

Storage Instructions for Acivir Cream (Generic Acyclovir Cream)

Store Acivir Cream at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not use Generic Acyclovir Cream past its expiration date printed on the blister pack. Keep the pack of Generic Acyclovir Cream away from the reach of children and pets. Do not dispose Acivir Cream along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Acivir Cream is contraindicated in patients with a hypersensitivity to to Acyclovir, Valacyclovir, or any inactive ingredient present in Acivir cream.

Overdose

No untoward effects would be expected if the entire contents of a 10 gram tube of Acivir Cream containing 500 mg of Aciclovir were ingested orally. However the accidental, repeated overdose of oral Aciclovir (Acyclovir), over several days has resulted in gastrointestinal effects (nausea and vomiting) and neurological effects (headache and confusion). Acyclovir is dialysable by haemodialysis.

Warnings and Precautions

External Use Only:
Acyclovir Cream is only for topical use of recurrent HSV lesions on the external aspect of lips and the face in patients with herpes labialis. Acivir Cream should not be applied on mucous membranes including in the eye or inside the mouth or nose.
Irritation And Contact Sensitization:
There is a potential for irritation and contact sensitization.
Immunocompromised Patients:
The effect of Acyclovir cream has not been established in immunocompromised patients.
General:
The recommended dosage, frequency of applications, and length of treatment should not be exceeded. There are no data to support the use of Acyclovir Cream 5% to prevent transmission of infection to other persons or prevent recurrent infections when applied in the absence of signs and symptoms. Acyclovir Cream 5% should not be used for the prevention of recurrent HSV infections. Although clinically significant viral resistance associated with the use of Acyclovir Cream 5% has not been observed, this possibility exists.

Side Effects of Acivir Cream (Generic Acyclovir Cream)

The most common side effects at the site of topical application were dry lips, desquamation, dryness of skin, cracked lips, burning skin, pruritus, flakiness of skin, and stinging on skin; each adverse reaction occurred in less than 1% of patients receiving Acyclovir cream and placebo.
In addition to adverse events reported from clinical trials, the following events have been identified during postapproval use of acyclovir cream. Because they are reported voluntarily from a population of unknown size, estimates of frequency cannot be made. These events have been chosen for inclusion due to a combination of their seriousness, frequency of reporting, or potential causal connection to acyclovir cream.
General: Angioedema, anaphylaxis.
Skin: Contact dermatitis, eczema.

Acyclovir Cream During Pregnancy

Acyclovir Cream (Acivir Cream) has been classified by the US FDA as Pregnancy Category B.
A post-marketing aciclovir pregnancy registry has documented pregnancy outcomes in women exposed to any formulation of Acyclovir Cream. The registry findings have not shown an increase in the number of birth defects amongst Aciclovir exposed subjects compared with the general population, and any birth defects showed no uniqueness or consistent pattern to suggest a common cause. Systemic administration of Acyclovir in internationally accepted standard tests did not produce embryotoxic or teratogenic effects in rabbits, rats or mice.
In a non-standard test in rats, foetal abnormalities were observed but only following such high subcutaneous doses that maternal toxicity was produced. The clinical relevance of these findings is uncertain.
The use of Acyclovir Cream should be considered only when the potential benefits outweigh the possibility of unknown risks however the systemic exposure to aciclovir from topical application of Acyclovir Cream is very low.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

Limited human data show that the drug does pass into breast milk following systemic administration. However, the dosage received by a nursing infant following maternal use of Acivir Cream would be insignificant.
After oral administration of Aciclovir, Aciclovir concentrations have been documented in breast milk in 2 women and ranged from 0.6 to 4.1 times the corresponding plasma levels. These concentrations would potentially expose the nursing infant to a dose of aciclovir up to 0.3 mg/kg/day.
Nursing mothers who have active herpetic lesions near or on the breast should avoid nursing. Do not use this medication without telling your doctor if you are breast-feeding a baby.
