Kenacort tablets (Generic Triamcinolone) are used to treat a number of different medical conditions, such as allergic disorders, eczema, Lichen sclerosus, psoriasis, ulcerative colitis, arthritis, ulcerative colitis, lupus, sympathetic ophthalmia, temporal arteritis, uveitis, ocular inflammation, Urushiol-induced contact dermatitis, aphthous ulcers, visualization during vitrectomy and the prevention of asthma attacks.

Triamcinolone Acetonide Mechanism of Action – How Do Kenacort Tablets Work?

Triamcinolone Acetonide (Kenacort Tablets) is a glucocorticoid that works by binding to cytoplasmic glucocorticoid receptors, which then translocate to the cell nucleus. In the nucleus, the complex binds to glucocorticoid response elements (GREs) in target genes, suppressing the expression of pro-inflammatory proteins (like cytokines and enzymes) and increasing the expression of anti-inflammatory proteins.
This ultimately reduces inflammation by decreasing inflammatory cell migration, inhibiting the production of inflammatory mediators, and decreasing the tissue response to inflammation.

Storage Instructions for Kenacort Oral Tablets(Triamcinolone Acetonide Tablets)

Store Kenacort Oral tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume Triamcinolone Acetonide tablets past its expiration date printed on the blister pack. Keep the pack of Kenacort tablets away from the reach of children and pets. Do not dispose Triamcinolone tablets along with domestic waste or sewage. Contraindications

Kenacort tablets are contraindicated in patients with a hypersensitivity to Triamcinolone Acetonide or any inactive ingredient present in tablets. Kenacort are contraindicated in the following cases:
Patients with systemic fungal infections

Overdose

The symptoms of glucocorticoid overdose may include confusion, anxiety, depression, gastrointestinal cramping or bleeding, ecchymosis, moon face, and hypertension. After long-term use, rapid withdrawal can result in acute adrenal insufficiency (which may also occur in times of stress). Cushingoid changes can result from continued use of large doses.
There is no specific treatment for acute overdose, but supportive therapy should be instituted and, if gastrointestinal bleeding occurs, it should be managed.

Warnings and Precautions

Ophthalmic effects:
May include cataracts, infections, and glaucoma. Monitor intraocular pressure.
Hypothalamic-pituitary-adrenal (HPA) axis suppression:
Hypothalamic-pituitary-adrenal (HPA) axis suppression, Cushing's syndrome and hyperglycemia: Monitor patients for these conditions and taper doses gradually.
Infections:
Increased susceptibility to new infection and increased risk of exacerbation, dissemination, or reactivation of latent infection.
Elevated blood pressure, salt and water retention, and hypokalemia:
Monitor blood pressure and sodium, potassium serum levels.
GI perforation:
Increased risk in patients with certain GI disorders.
Behavioral and mood disturbances:
May include euphoria, insomnia, mood swings, personality changes, severe depression, and psychosis.
Decreases in bone density:
Monitor bone density in patients receiving long term corticosteroid therapy.
Live or live attenuated vaccines:
Do not administer to patients receiving immunosuppressive doses of corticosteroids.
Negative effects on growth and development:
Monitor pediatric patients on long-term corticosteroid therapy.
Use in pregnancy:
Fetal harm can occur with first trimester use.
Weight gain:
May cause increased appetite.

Side Effects of Triamcinolone Acetonide (Kenacort Tablets)

 

  • General: Following administration by any route, anaphylactoid reactions, anaphylactic reactions, anaphylactic shock, aggravation or masking of infections.
  • Cardiovascular: hypertension, syncope, congestive heart failure, arrhythmias, necrotizing angiitis, thromboembolism, thrombophlebitis.
  • Fluid and electrolyte disturbances: sodium retention, fluid retention associated with hypertension or congestive heart failure, potassium loss which may lead to cardiac arrhythmias or ECG changes, hypokalemic alkalosis.
  • Musculoskeletal: muscle weakness, fatigue, myopathy, loss of muscle mass, osteoporosis, vertebral compression fractures, delayed healing of fractures, aseptic necrosis of femoral and humeral heads, pathologic fractures of long bones, spontaneous fractures.
  • Gastrointestinal: peptic ulcer with possible subsequent perforation and hemorrhage, pancreatitis, abdominal distention, ulcerative esophagitis.
  • Dermatologic: impaired wound healing, thin fragile skin, petechiae and ecchymoses, facial erythema, increased sweating, purpura, striae, hirsutism, acneiform eruptions, lupus erythematosus-like lesions, hives, rash, suppressed reactions to skin tests.
  • Neuropsychiatric: convulsions, increased intracranial pressure with papilledema (pseudotumor cerebri) usually after treatment, vertigo, headache, insomnia, neuritis, parasthesias, aggravation of pre-existing psychiatric conditions, depression (sometimes severe), euphoria, mood swings, psychotic symptoms, personality changes.
  • Endocrine: menstrual irregularities, postmenopausal vaginal haemorrhage, development of the cushingoid state, suppression of growth in children, secondary adrenocortical and pituitary unresponsiveness, particularly in times of stress (e.g., trauma, surgery, or illness), decreased carbohydrate tolerance, manifestations of latent diabetes mellitus, increased requirements for insulin or oral hypoglycemic agents in diabetics.
  • Ophthalmic: central serous chorioretinopathy, posterior subcapsular cataracts, increased intra- ocular pressure, glaucoma, exophthalmos, blurred vision, corneal perforation.
  • Metabolic: hyperglycemia, glycosuria, negative nitrogen balance due to protein catabolism

 

Triamcinolone Acetonide (Kenacort Oral Tablets) During Pregnancy

Triamcinolone Acetonide has been classified by the US FDA as Pregnancy Category C. Corticosteroids have been observed to be teratogenic in many species when given in doses equivalent to the human dose.
Triamcinolone acetonide can cause fetal harm when administered to a pregnant woman. Human and animal studies suggest that use of corticosteroids during the first trimester of pregnancy is associated with an increased risk of orofacial clefts, intrauterine growth restriction and decreased birth weight.
If this drug is used during pregnancy, or if the patient becomes pregnant while using this drug, the patient should be apprised of the potential hazard to the fetus.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Many corticosteroids have been shown to be teratogenic in laboratory animals at low doses.
