The active ingredient present in Sartel tablets is Telmisartan. Each Sartel tablet contains 20 mg, 40 mg or 80 mg of Telmisartan respectively.

Uses of Telmisartan Tablets

Telmisartan (Sartel Tablets) belongs to a class of medicines known as angiotensin II receptor blockers. Angiotensin II is a substance produced in your body which causes your blood vessels to narrow, thus increasing your blood pressure. Micardis blocks the effect of angiotensin II so that the blood vessels relax, and your blood pressure is lowered.
Telmisartan is used to treat essential hypertension (high blood pressure) in adults. ‘Essential’ means that the high blood pressure is not caused by any other condition.
Telmisartan is also used to reduce cardiovascular events (i.e. heart attack or stroke) in adults who are at risk because they have a reduced or blocked blood supply to the heart or legs, or have had a stroke or have high risk diabetes. Your doctor can tell you if you are at high risk for such events.

Sartel Tablets - Dosage

Generic Telmisartan tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Sartel tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Treatment Of Essential Hypertension
The usually effective dose is 40 mg once daily. Some patients may already benefit at a daily dose of 20 mg. In cases where the target blood pressure is not achieved, the dose of telmisartan can be increased to a maximum of 80 mg once daily. When considering raising the dose, it must be borne in mind that the maximum antihypertensive effect is generally attained 4 to 8 weeks after the start of treatment.
Cardiovascular Prevention
The recommended dose is 80 mg once daily. It is not known whether doses lower than 80 mg of telmisartan are effective in reducing cardiovascular morbidity. When initiating Telmisartan therapy for the reduction of cardiovascular morbidity, close monitoring of blood pressure is recommended, and if appropriate adjustment of medications that lower blood pressure may be necessary.
Sartel tablets work effectively if the amount of Telmisartan is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Sartel tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular tablet dose on time, take it whenever you remember.

Telmisartan Mechanism of Action

Telmisartan is an angiotensin II receptor blocker (ARB). It works by blocking a substance in the body that causes blood vessels to tighten. As a result, telmisartan relaxes the blood vessels.
This lowers blood pressure and increases the supply of blood and oxygen to the heart.

Storage Instructions for Sartel Tablets

Store Sartel tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Sartel tablets away from the reach of children and pets. Contraindications

Sartel tablets are contraindicated in patients with a hypersensitivity to Telmisartan or any inactive ingredient present in tablets. Sartel tablets are contraindicated in the following cases:
Second and third trimesters of pregnancy, Biliary obstructive disorders and Severe hepatic impairment
Do not co-administer Aliskiren with Telmisartan tablets in patients with diabetes

Overdose

Limited data are available with regard to overdosage in humans. The most likely manifestation of overdosage with Telmisartan (Sartel Tablets) would be hypotension, dizziness and tachycardia; bradycardia could occur from parasympathetic (vagal) stimulation. If symptomatic hypotension should occur, supportive treatment should be instituted. Telmisartan is not removed by hemodialysis.

Warnings and Precautions

 

  • Avoid fetal or neonatal exposure
  • Hypotension: Correct any volume or salt depletion before initiating therapy. Observe for signs and symptoms of hypotension
  • Monitor carefully in patients with impaired hepatic or renal function
  • Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker

 

Effects On Ability To Drive And Use Machines

When driving vehicles or operating machinery it should be taken into account that dizziness or drowsiness may occasionally occur when taking antihypertensive therapy such as Telmisartan

Side Effects of Telmisartan (Sartel Tablets)

Telmisartan (Sartel Tablets) side effects occurring at an incidence of 1% or more in patients treated with Telmisartan and at a greater rate than in patients treated with placebo, irrespective of their causal association, are Upper respiratory tract infection, Back pain, Sinusitis, Diarrhea and Pharyngitis.
In addition to the adverse events in the table, the following events occurred at a rate of 1% but were at least as frequent in the placebo group: influenza-like symptoms, dyspepsia, myalgia, urinary tract infection, abdominal pain, headache, dizziness, pain, fatigue, coughing, hypertension, chest pain, nausea and peripheral edema. In addition to those listed above, side effects that occurred in more than 0.3% of 3500 patients treated with Telmisartan tablets monotherapy in controlled or open trials are listed below. It cannot be determined whether these events were causally related to Telmisartan tablets:

  • Autonomic Nervous System: impotence, increased sweating, flushing;
  • Body as a Whole: allergy, fever, leg pain, malaise;
  • Cardiovascular: palpitation, dependent edema, angina pectoris, tachycardia, leg edema, abnormal ECG;
  • CNS: insomnia, somnolence, migraine, vertigo, paresthesia, involuntary muscle contractions, hypoaesthesia;
  • Gastrointestinal: flatulence, constipation, gastritis, vomiting, dry mouth, hemorrhoids, gastroenteritis, enteritis, gastroesophageal reflux, toothache, non-specific gastrointestinal disorders;
  • Metabolic: gout, hypercholesterolemia, diabetes mellitus; Musculoskeletal: arthritis, arthralgia, leg cramps;
  • Psychiatric: anxiety, depression, nervousness;
  • Resistance Mechanism: infection, fungal infection, abscess, otitis media;
  • Respiratory: asthma, bronchitis, rhinitis, dyspnea, epistaxis;
  • Skin: dermatitis, rash, eczema, pruritus;
  • Urinary: micturition frequency, cystitis; Vascular:
  • cerebrovascular disorder; and Special Senses: abnormal vision, conjunctivitis, tinnitus, earache.

 

Telmisartan Tablets During Pregnancy

Telmisartan (Sartel Tablets) has been classified by the US FDA as Pregnancy Category C (first trimester) and D (second and third trimesters .
Use of medications that act on the renin-angiotensin system during the second and third trimesters of pregnancy reduces fetal renal function and increases fetal and neonatal morbidity and death.
Resulting oligohydramnios can be associated with fetal lung hypoplasia and skeletal deformations. Potential neonatal adverse effects include skull hypoplasia, anuria, hypotension, renal failure, and death. When pregnancy is detected, discontinue telmisartan tablets as soon as possible.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Nursing Considerations

It is not known whether Telmisartan is excreted in human milk, but telmisartan was shown to be present in the milk of lactating rats. Because of the potential for adverse effects on the nursing infant, decide whether to discontinue nursing or discontinue the drug, taking into account the importance of the drug to the mother.
