Active Pharmaceutical Ingredient

The active ingredient present in tablets is Tamoxifen Citrate.
Each Tamodex tablet contains either 15.2 mg or 30.4 mg of Tamoxifen Citrate which is equivalent to 10 mg or 20 mg of Tamoxifen respectively.

Uses of Tamoxifen Citrate Tablets (Tamodex)

Tamodex (Generic Tamoxifen Citrate tablets) is an estrogen agonist/antagonist indicated:

• For treatment of adult patients with estrogen receptor-positive metastatic breast cancer
• For adjuvant treatment of adult patients with early stage estrogen receptor- positive breast cancer
• To reduce risk of invasive breast cancer following breast surgery and radiation in adult women with ductal carcinoma in situ (DCIS)
• To reduce the incidence of breast cancer in adult women at high risk

Tamoxifen Citrate Tablets (Tamodex) - Dosage

Generic Tamoxifen Citrate tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Tamodex tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Adults (including the elderly)
Breast cancer
The dosage range is 20 to 40 mg daily given either in divided doses twice daily or as a single dose once daily. In early disease, it is currently recommended that treatment is given for not less than 5 years. The optimal duration of Tamoxifen therapy remains to be determined.
Tamodex tablets work effectively if the amount of Tamoxifen is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Missed Dose

Mechanism of Action  

Tamodex Tablets (Generic Tamoxifen Citrate Tablets) is a non-steroidal substance which acts as anti-estrogen a SERM (Selective Estrogen Receptor Modulator) by competing with estrogen for its specific binding sites in cancer cells.
Tamoxifen Citrate treatment is effective only in those tumors which are positive for the presence of estrogen receptors (ER+), which signifies the requirement of estrogen for the growth of the tumor. Tamoxifen successfully inhibits the effect of estrogen on these cancer cells thus preventing their growth and survival.

Store at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.

Contraindications

Tamodex tablets are contraindicated in patients with a hypersensitivity to Tamoxifen Citrate or any inactive ingredient present in tablets. Tamodex tablets are also contraindicated in the following cases:
Tamoxifen must not be administered during pregnancy. Premenopausal patients must be carefully examined before treatment for breast cancer to exclude the possibility of pregnancy.
When used in women with DCIS and women at high risk for breast cancer, history of DVT or pulmonary embolism.
When used in women with DCIS and women at high risk for breast cancer, concurrent anticoagulant therapy with a warfarin derivative.

Overdose

On theoretical grounds, an overdosage would be expected to cause an enhancement of the pharmacological side-effects. Animal studies have shown that extreme overdosage (100 to 200 times the recommended daily dose) may produce estrogenic effects.
There have been reports in the literature that Tamoxifen given at several times the standard dose may be associated with prolongation of the QT interval of the ECG.
There is no specific antidote and treatment must be symptomatic.

Warnings and Precautions

Uterine malignancies:
Promptly evaluate abnormal vaginal bleeding in a woman with current or past tamoxifen use.
Thromboembolic events:
Risk increases with coadministered chemotherapy. For treatment of breast cancer, consider risks and benefits in patients with a history of thromboembolic events.
Embryo-fetal toxicity:
Can cause fetal harm. Advise females of reproductive potential of the potential risk to a fetus and to use effective contraception.
Effects on the liver:
Liver cancer and liver abnormalities, some fatal, have occurred. Perform periodic liver function testing
Effects in Metastatic Breast Cancer Patients:
As with other additive hormonal therapy (estrogens and androgens), hypercalcemia has been reported in some breast cancer patients with bone metastases within a few weeks of starting treatment with tamoxifen. If hypercalcemia does occur, appropriate measures should be taken and, if severe, tamoxifen should be discontinued.
Effects on the Eye:
Ocular disturbances, including corneal changes, decrement in color vision perception, retinal vein thrombosis, and retinopathy have been reported in patients receiving Tamoxifen.

Side Effects of Tamoxifen Citrate Tablets (Tamodex)

Most common side effects are hot flashes, mood disturbances, vaginal discharge, vaginal bleeding, nausea, and fluid retention.

Tamoxifen may cause fetal harm when administered to a pregnant woman. Women should be advised not to become pregnant while taking Tamoxifen or within 2 months of discontinuing tamoxifen and should use barrier or nonhormonal contraceptive measures if sexually active. Tamoxifen does not cause infertility, even in the presence of menstrual irregularity. Effects on reproductive functions are expected from the antiestrogenic properties of the drug.

Nursing Considerations

There is no information regarding the presence of tamoxifen or its metabolites in human milk or its effects on the breastfed infant.
