The active ingredients present in Super Tadalis SX are Tadalafil and Dapoxetine Hydrochloride.
Each Super Tadalis SX tablet contains 20 mg of Tadalafil and 60 mg of Dapoxetine.

Uses of Super Tadalis SX

Super Tadalis SX contains a combination of Tadalafil 20 mg and Dapoxetine 60 mg. Tadalafil is a phosphodiesterase-5 (PDE5) inhibitor indicated for the treatment of erectile dysfunction (ED), also called sexual impotence. Tadalafil along with other medicines called called phosphodiesterase 5 (PDE5) inhibitors prevent an enzyme called phosphodiesterase type-5 from working too quickly. Tadalafil improves the flow of blood to the penis upon sexual stimulation. This enhanced blood flow helps in attaining a long-lasting erection during sexual activity.
Meanwhile, Dapoxetine is a short-acting selective serotonin reuptake inhibitor (SSRI) which acts on the neuronal responses that control and defer the process of ejaculation, thus efficaciously averting the obstacle of premature ejaculation in men. Dapoxetine should only be prescribed to patients who meet all the following criteria:

Erection during sexual stimulation occurs when there is an improved blood flow in the penis due to the expansion in the size of blood vessels that bring the blood to the penis. Tadalafil belong to a group of drugs called phosphodiesterase-5 (PDE5) inhibitors. Other drugs in this group include Sildenafil (Viagra) and Vardenafil (Levitra).

Contraindications

Super Tadalis SX tablets are contraindicated in patients with a hypersensitivity to Tadalafil, Dapoxetine or any inactive ingredient present in tablets. Super Tadalis SX tablets are contraindicated in the following cases:

• Administration of Tadalafil to patients using any form of organic nitrate is contraindicated. Tadalafil was shown to potentiate the hypotensive effect of nitrates.
• Coadministration of GC stimulators such as riociguat with Tadalafil is contraindicated. Tadalafil may potentiate the hypotensive effects of GC stimulators.
• In patients with significant pathological cardiac conditions such as heart failure (NYHA class II-IV), conduction abnormalities such as AV block or sick sinus syndrome, significant ischemic heart disease or significant valvular disease, history of syncope
• In patients with a history of mania or severe depression
• For concomitant treatment with monoamine oxidase inhibitors (MAOIs), or within 14 days of discontinuing treatment with an MAOI. Similarly, an MAOI should not be administered within 7 days after Super Tadalis SX has been discontinued
• For concomitant treatment with thioridazine, or within 14 days of discontinuing treatment with thioridazine. Similarly, thioridazine should not be administered within 7 days after Super Tadalis SX has been discontinued
• For concomitant treatment with serotonin reuptake inhibitors [selective serotonin reuptake inhibitors (SSRIs), serotonin-norepinephrine reuptake inhibitors (SNRIs), tricyclic antidepressants (TCAs)] or other medicinal/herbal products with serotonergic effects [e.g., L-tryptophan, triptans, tramadol, linezolid, lithium, St. John’s Wort (Hypericum perforatum)] or within 14 days of discontinuing treatment with these medicinal/herbal products. Similarly these medicinal/herbal products should not be administered within 7 days after Super Tadalis SX has been discontinued
• For concomitant treatment with potent CYP3A4 inhibitors such as Ketoconazole, Itraconazole, Ritonavir, Saquinavir, Telithromycin, Nefazodone, Nelfinavir, Atazanavir, etc.
• In patients with moderate and severe hepatic impairment.
• Moderate and severe hepatic impairment.

Overdose

Symptoms of overdose with Dapoxetine HCl include serotonin-mediated adverse reactions such as somnolence, gastrointestinal disturbances such as nausea and vomiting, tachycardia, tremor, agitation and dizziness. Single doses up to 500 mg of Tadalafil have been given to healthy male subjects, and multiple daily doses up to 100 mg have been given to male patients with erectile dysfunction. Adverse reactions were similar to those seen at lower doses.
In cases of Super Tadalis SX overdose, standard supportive measures should be adopted as required. Due to high protein binding and large volume of distribution of Dapoxetine Hydrochloride as well as Tadalafil, forced diuresis, dialysis, hemoperfusion and exchange transfusion are unlikely to be of benefit. No specific antidotes for Super Tadalis SX are known at present.

Warnings and Precautions

Cardiovascular Status
Patients should not use Super Tadalis SX if sex is inadvisable due to cardiovascular status
Use of Super Tadalis SX with alpha blockers, antihypertensives or substantial amounts of alcohol (5 units or more) may lead to hypotension
Caution is advised when Super Tadalis SX is used as a treatment for ED in men taking alpha blockers.
If taking potent inhibitors of CYP3A4, dose should be adjusted to 10 mg of Tadalafil every 72 hours. This combination medication may not be suitable.
Patients should seek emergency treatment if an erection lasts for more than 4 hours. Use Super Tadalis SX with caution in patients predisposed to priapism
Patients should stop Super Tadalis SX and seek prompt medical attention in the event of sudden decrease or loss of hearing
NAION
Patients should stop Super Tadalis SX and seek medical care if a sudden loss of vision occurs in one or both eyes, which could be a sign of Non Arteritic Ischemic Optic Neuropathy (NAION). Discuss increased risk of NAION in patients with history of NAION
General recommendations
Dapoxetine, a constituent of Super Tadalis SX is only indicated in men with Premature Ejaculation who meet all the criteria. Super Tadalis SX tablets should not be prescribed to men who have not been diagnosed with Premature Ejaculation. Safety has not been established and there are no data on the ejaculation-delaying effects in men without Premature Ejaculation.
Orthostatic hypotension
Before treatment initiation, a careful medical examination including history of orthostatic events should be performed by the physician. An orthostatic test should be performed before initiating therapy (blood pressure and pulse rate, supine and standing). In case of a history of documented or suspected orthostatic reaction, treatment with Super Tadalis SX should be avoided
Suicide/suicidal thoughts
Antidepressants, including SSRIs, increased the risk compared to placebo of suicidal thinking and suicidality in short-term studies in children and adolescents with Major Depressive Disorder and other psychiatric disorders. Short-term studies did not show an increase in the risk of suicidality with antidepressants compared to placebo in adults beyond age 24. In clinical trials with Dapoxetine for the treatment of premature ejaculation, there was no clear indication of treatment-emergent suicidality in evaluation of possibly suicide-related adverse events.
Syncope
Patients should be cautioned to avoid situations where injury could result, including driving or operating hazardous machinery, should syncope or its prodromal symptoms such as dizziness or lightheadedness occur. Possibly prodromal symptoms such as nausea, dizziness/lightheadedness, and diaphoresis were reported more frequently among patients treated with Dapoxetine compared to placebo. The number and percentage of subjects with syncope (characterized as loss of consciousness) was greater in Super Tadalis SX-treated subjects than in those who were treated with placebo.
Patients with cardiovascular risk factors
Subjects with underlying cardiovascular disease were excluded from Phase 3 clinical trials. The risk of adverse cardiovascular outcomes from syncope (cardiac syncope and syncope from other causes) is increased in patients with underlying structural cardiovascular disease (e.g., documented outflow obstruction, valvular heart disease, carotid stenosis and coronary artery disease).
Combination With Recreational Drugs
Patients should be advised not to use Super Tadalis SX in combination with recreational drugs. Recreational drugs with serotonergic activity such as ketamine, methylenedioxy-methamphetamine (MDMA) and lysergic acid diethylamide (LSD) may lead to potentially serious reactions if combined with this medicine. Patients should be advised not to use Super Tadalis SX in combination with alcohol. Combining alcohol with dapoxetine may increase alcohol-related neurocognitive effects and may also enhance neurocardiogenic adverse events such as syncope, thereby increasing the risk of accidental injury; therefore, patients should be advised to avoid alcohol while taking Super Tadalis SX
Moderate CYP3A4 inhibitors
Caution is advised in all patients concomitantly treated with moderate CYP3A4 inhibitors. Unless the patient is known to be a CYP2D6 extensive metaboliser, caution is advised when used concomitantly with moderate CYP3A4 inhibitors
Potent CYP2D6 inhibitors
Caution is advised in patients taking potent CYP2D6 inhibitors or in patients known to be of CYP2D6 poor metaboliser genotype, as this may increase exposure levels, which may result in a higher incidence and severity of dose dependent adverse events
Mania
Super Tadalis SX should not be used in patients with a history of mania/hypomania or bipolar disorder and should be discontinued in any patient who develops symptoms of these disorders.
Seizure
Due to the potential of SSRIs to lower the seizure threshold, Super Tadalis SX should be discontinued in any patient who develops seizures and avoided in patients with unstable epilepsy. Patients with controlled epilepsy should be carefully monitored.
Paediatric population
Super Tadalis SX should not be used in individuals below 18 years of age.
Depression and/or psychiatric disorders
Men with underlying signs and symptoms of depression should be evaluated prior to treatment with Super Tadalis SX to rule out undiagnosed depressive disorders. Concomitant treatment of Super Tadalis SX with antidepressants, including SSRIs and SNRIs, is contraindicated
Haemorrhage
There have been reports of bleeding abnormalities with SSRIs. Caution is advised in patients taking Super Tadalis SX, particularly in concomitant use with medicinal products known to affect platelet function
Renal impairment
Super Tadalis SX is not recommended for use in patients with severe renal impairment and caution is advised in patients with mild or moderate renal impairment.
Withdrawal effects
Abrupt discontinuation of chronically administered SSRIs used to treat chronic depressive disorders has been reported to result in the following symptoms: dysphoric mood, irritability, agitation, dizziness, sensory disturbances (e.g., paraesthesias such as electric shock sensations), anxiety, confusion, headache, lethargy, emotional lability, insomnia, and hypomania.

Nursing Considerations

Super Tadalis SX tablets are not indicated for use in females. Tadalafil is not indicated for use in women. It is not known whether tadalafil is excreted into human milk. While Tadalafil or some metabolite of tadalafil was excreted into rat milk, drug levels in animal breast milk may not accurately predict levels of drug in human breast milk. Dapoxetine is not indicated for use in women. It is not known if dapoxetine or its metabolites are excreted in breast milk.
