Amisulpride Tablets (Solian 50 mg, 100 mg, 200 mg and 400 mg) - Product Information

Amisulpride Tablets (Solian Tablets) is indicated for the treatment of acute and chronic schizophrenic disorders, in which positive symptoms (such as delusions, hallucinations, thought disorders) and/or negative symptoms (such as blunted affect, emotional and social withdrawal) are prominent, including patients characterised by predominant negative symptoms.
Solian Tablets are also used for prevention of postoperative nausea and vomiting (PONV), either alone or in combination with an antiemetic of a different class.
This medicine ia also used for the treatment of PONV in patients who have received antiemetic prophylaxis with an agent of a different class or have not received prophylaxis.

The active ingredient present in Solian tablets is Amisulpride. Each uncoated Solian tablet contain 50 mg, 100 mg, 200 mg or 400 mg of Amisulpride.

Uses of Amisulpride Tablets (Solian Tablets)

Amisulpride tablets (Solian tablets) belong to a group of medicines called antipsychotics. Amisulpride is used for the treatment of acute and chronic schizophrenic disorders, in which positive symptoms (such as delusions, hallucinations, thought disorders) and/or negative symptoms (such as blunted affect, emotional and social withdrawal) are prominent, including patients characterised by predominant negative symptoms.
Amisulpride also regulates secondary negative symptoms in productive state, as well as affective disorders such as depressive mood.
Schizophrenia is a condition which affects the way you think, feel and/or act. Schizophrenia may cause symptoms such as hallucinations (eg hearing, seeing or sensing things which are not there), delusions, unusual suspiciousness, emotional and social withdrawal. People with schizophrenia may also feel depressed, anxious or tense.
Solian Tablets are also used alone or with other medicines to prevent and treat nausea and vomiting that may occur after surgery. This medicine works in the stomach to block the signals to the brain that cause nausea and vomiting.

Solian Tablets - Dosage

Generic Amisulpride tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Solian tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
If the daily dose does not exceed 400 mg, it should be administered once daily. If the daily dose exceeds 400 mg, it should be administered as two divided doses.
Acute psychotic episodes
Doses between 400 mg/day and 800 mg/day are recommended for oral administration. The maximum daily dose should never exceed 1 200 mg. Given that there has been no large-scale safety assessment of doses higher than 1 200 mg/day, these doses should not be used.
The dosage should then be maintained or adjusted depending on the patient's individual response. In all cases, the maintenance treatment should be established individually with the minimum effective dose.
Predominantly negative episodes
Doses between 50 mg/day and 300 mg/day are recommended. Doses should be adjusted individually. The optimum dose is about 100 mg/day.
Children and adolescents
The efficacy and safety of amisulpride from puberty to the age of 18 years have not been established: there are limited data available on the use of Amisulpride in adolescents in schizophrenia. Therefore, the use of amisulpride from puberty to the age of 18 years is not recommended.
Solian tablets work effectively if the amount of Amisulpride is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Solian tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Amisulpride tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Solian tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Amisulpride tablets Mechanism of Action – How Does Solian Work?

Amisulpride binds selectively to dopamine D(2) and D(3) receptors in the limbic system.
Low doses of amisulpride preferentially block presynaptic D(2)/D(3)-dopamine autoreceptors, thereby enhancing dopaminergic transmission, whereas higher doses block postsynaptic receptors, thus inhibiting dopaminergic hyperactivity.

Storage Instructions for Solian Tablets

Store Solian tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Solian tablets away from the reach of children and pets. Do not dispose Solian tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Solian tablets are contraindicated in patients with a hypersensitivity to Amisulpride or any inactive ingredient present in Solian tablets. Solian tablets are also contraindicated in the following cases:

  • Concomitant prolactin-dependent tumours e.g. pituitary gland prolactinomas and breast cancer.
  • Phaeochromocytoma.
  • Amisulpride is contraindicated in children under 15 years of age, since the safety of the drug has not been established .
  • Lactation.
  • In combination with the following medication which could induce torsades de pointes: Class Ia antiarrhythmic agents such as quinidine and disopyramide, Class III antiarrhythmic agents such as amiodarone and sotalol, Other medications such as bepridil, cisapride, sultopride, thioridazine, methadone, intravenous erythromycin, intravenous vincamine, halofantrine, pentamidine, sparfloxacin.
  • Levodopa; reciprocal antagonism between levodopa and neuroleptics
  • In hepatic impairment, amisulpride may be contraindicated to avoid the possible risk of adverse events due to an influence of the disease on amisulpride metabolism.

 

Overdose

Experience with Amisulpride Tablets (Solian Tablets) in overdosage is limited. Exaggeration of the known pharmacological effects of the drug have been reported. These include drowsiness and sedation, coma, hypotension and extrapyramidal symptoms. Fatal outcomes have been reported mainly in combination with other psychotropic agents.
In cases of acute overdosage, the possibility of multiple drug intake should be considered. Since amisulpride is weakly dialysed, haemodialysis is of no use to eliminate the drug.
There is no specific antidote to Amisulpride.

Warnings and Precautions

QT Prolongation:
Amisulpride induces a dose-dependent prolongation of the QT interval (see section 4.8). This effect is known to potentiate the risk of serious ventricular arrhythmias such as torsade de pointes. Before any administration, and if possible according to the patient's clinical status, it is recommended to exclude the following factors which could favour the occurrence of this rhythm disorder: bradycardia less than 55 bpm, cardiac disease or family history of sudden death or QT prolongation, electrolyte imbalance, in particular hypokalaemia, congenital prolongation of the QT interval, on-going treatment with a medicinal product likely to produce pronounced bradycardia (less than 55 bpm), hypokalaemia, decreased intracardiac conduction, or prolongation of the QT interval (
ECG monitoring recommended in patients with pre-existing arrhythmias or cardiac conduction disorders, electrolyte abnormalities (e.g., hypokalemia or hypomagnesemia), congestive heart failure, and in patients taking other QT-prolonging drugs (e.g., ondansetron) or with other medical conditions known to prolong the QT interval.
Neuroleptic Malignant Syndrome (NMS)
Neuroleptic Malignant Syndrome (NMS) is a potentially fatal syndrome that has been reported in association with antipsychotic medicines, including amisulpride. Neuroleptic malignant syndrome is characterised by hyperthermia, muscle rigidity, autonomic instability, and elevated CPK may occur. In the event of any symptoms which could suggest NMS, in particular hyperthermia, particularly with high daily doses, all antipsychotic medicines including amisulpride should be discontinued.
Rhabdomyolysis
Rhabdomyolysis has also been observed in patients without NMS.
Seizure Threshold:
Amisulpride can lower the seizure threshold. Therefore patients with a history of seizures should be closely monitored during amisulpride therapy.
Withdrawal Symptoms
Withdrawal symptoms have been described after abrupt cessation of high therapeutic doses of antipsychotic medicines. The emergence of involuntary movement disorders (such as akathisia, dystonia and dyskinesia) has been reported with amisulpride. Therefore, gradual withdrawal of amisulpride is advisable.
Leukopenia, Neutropenia and Agranulocytosis:
Leukopenia, neutropenia and agranulocytosis have been reported with antipsychotics, including amisulpride. Unexplained infections or fever may be evidence of blood dyscrasia and requires immediate haematological investigation.
Patients With Parkinson's Disease:
Caution should be also exercised when prescribing amisulpride to patients with Parkinson's disease since it may cause worsening of the disease. Amisulpride should be used only if neuroleptic treatment cannot be avoided.
Increase In Plasma Prolactin Levels:
Amisulpride causes an increase in plasma prolactin levels which is reversible after discontinuation of the medicine. This may result in galactorrhoea, amenorrhoea, gynaecomastia, breast pain, orgasmic dysfunction and impotence.
Acute Dystonia:
Acute dystonia (spasm torticollis, oculogyric crisis, trismus) may appear. This is reversible without discontinuation of amisulpride upon treatment with an anti-parkinsonian agent.
Extrapyramidal symptoms may occur:
tremor, rigidity, hypokinesia, hypersalivation, akathisia. These symptoms are generally mild at optimal dosages and partially reversible without discontinuation of amisulpride upon administration of anti-parkinsonian medication.
Hyperglycaemia and Diabetes Mellitus:
Hyperglycemia has been reported hence appropriate glycemic monitoring for patients with diabetes mellitus or with risk factors for diabetes. The seizure threshold can be lowered.

Side Effects of Amisulpride tablets (Solian)

The very common Amisulpride tablets (Solian) side effects with an incidencr rate of 10% or more includes extrapyramidal symptoms.
The common Amisulpride tablets (Solian) side effects with an incidence rate of 1% or more to less than 10% include acute dystonia somnolence, insomnia, anxiety, agitation, orgasmic dysfunction, constipation, nausea, vomiting, dry mouth, hypotension, weight gain, blurred vision, increase in plasma prolactin levels may result in galactorrhoea, amenorrhoea, gynaecomastia, breast pain, and erectile dysfunction.

Amisulpride Tablets (Solian Tablets) During Pregnancy

Amisulpride has been classified by the (US FDA as Pregnancy Category: Not Assigned) AU TGA pregnancy category: C .
In animals, amisulpride did not show reproductive toxicity. A decrease in fertility linked to the pharmacological effects of the drug (prolactin mediated effect) was observed. No teratogenic effects of amisulpride were noted.
Very limited clinical data on exposed pregnancies are available. Therefore, the safety of amisulpride during human pregnancy has not been established, and therefore use of this medicine is not recommended during pregnancy unless the benefits justify the potential risks.
Do not use Solian tablets without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
It is not known whether amisulpride or its metabolites are excreted in animal or human breastmilk. Breast-feeding is therefore contraindicated during amisulpride treatment. Do not use this medication without telling your doctor if you are breast-feeding a baby.
