Glycomet Tablets (Generic Metformin Hydrochloride) - Product Information

Glycomet (Generic Metformin Hydrochloride) is a biguanide used as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus.
Metformin hydrochloride tablets should not be used in patients with type 1 diabetes or for the treatment of diabetic ketoacidosis, as it would not be effective in these settings.

Active Pharmaceutical Ingredient

The active ingredient present in Glycomet Tablets is Metformin Hydrochloride.
Each Glycomet 500 mg tablet contains 500 mg Metformin Hydrochloride corresponding to 390 mg Metformin base. Each Glycomet 750 mg tablet contains 750 mg Metformin Hydrochloride corresponding to 585 mg Metformin base.
Each Glycomet 1000 mg tablet contains 1000 mg Metformin Hydrochloride corresponding to 780 mg Metformin base.

Uses of Glycomet tablets (Generic Metformin Hydrochloride)

Glycomet (Generic Metformin Hydrochloride) is used to treat high blood sugar levels that are caused by a type of diabetes mellitus or sugar diabetes called type 2 diabetes. With this type of diabetes, insulin produced by the pancreas is not able to get sugar into the cells of the body where it can work properly. Using Metformin alone, with a type of oral antidiabetic medicine called a sulfonylurea, or with insulin, will help to lower blood sugar when it is too high and help restore the way you use food to make energy.
Many people can control type 2 diabetes with diet and exercise. Following a specially planned diet and exercise will always be important when you have diabetes, even when you are taking medicines. To work properly, the amount of Metformin you take must be balanced against the amount and type of food you eat and the amount of exercise you do. If you change your diet or exercise, you will want to test your blood sugar to find out if it is too low.

Glycomet tablets (Generic Metformin Hydrochloride) - Dosage

Generic Metformin Hydrochloride tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Glycomet tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Adults with normal renal function: Monotherapy and combination with other oral antidiabetic agents The usual starting dose is 500 mg or 850 mg Metformin Hydrochloride 2 or 3 times daily given during or after meals. After 10 to 15 days the dose should be adjusted on the basis of blood glucose measurements. A slow increase of dose may improve gastrointestinal tolerability. The maximum recommended dose of Metformin Hydrochloride is 3 g daily, taken as 3 divided doses. If transfer from another oral antidiabetic agent is intended: discontinue the other agent and initiate Metformin at the dose indicated above.
Glycomet tablets work effectively if the amount of Metformin is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Glycomet tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose.

Glycomet Tablets (Generic Metformin Hydrochloride) Mechanism of Action

Metformin is an antihyperglycemic agent which improves glucose tolerance in patients with type 2 diabetes, lowering both basal and postprandial plasma glucose. Its pharmacologic mechanisms of action are different from other classes of oral antihyperglycemic agents.
Metformin decreases hepatic glucose production, decreases intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. Unlike sulfonylureas, metformin does not produce hypoglycemia in either patients with type 2 diabetes or normal subjects and does not cause hyperinsulinemia.
With Metformin therapy, insulin secretion remains unchanged while fasting insulin levels and day-long plasma insulin response may actually decrease.

Storage Instructions for Glycomet Tablets (Generic Metformin Hydrochloride)

Store Glycomet Tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Generic Metformin Hydrochloride tablets away from the reach of children and pets. Do not dispose Glycomet tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.

Contraindications

Glycomet tablets are contraindicated in patients with a hypersensitivity to Metformin Hydrochloride or any inactive ingredient present in tablets. Glycomet tablets are contraindicated in the following cases:
Renal impairment, Metabolic acidosis, including diabetic ketoacidosis. (

Overdose

No cases of overdose were reported during Metformin Hydrochloride tablets clinical trials. It would be expected that adverse reactions of a more intense character including epigastric discomfort, nausea, and vomiting followed by diarrhea, drowsiness, weakness, dizziness, malaise and headache might be seen. Should those symptoms persist, lactic acidosis should be excluded.
Overdose of Metformin Hydrochloride has occurred, including ingestion of amounts greater than 50 grams. Hypoglycemia was reported in approximately 10% of cases, but no causal association with Metformin Hydrochloride has been established. Lactic acidosis has been reported in approximately 32% of metformin overdose cases.

Warnings and Precautions

Lactic acidosis:
Warn against excessive alcohol intake. Metformin hydrochloride is not recommended in hepatic impairment and is contraindicated in renal impairment. Lactic acidosis can occur due to metformin accumulation. The risk increases with conditions such as sepsis, dehydration, excess alcohol intake, hepatic insufficiency, renal impairment, and acute congestive heart failure. Ensure normal renal function before initiating and at least annually thereafter.
Radiologic Studies:
Temporarily discontinue in patients undergoing radiologic studies with intravascular administration of iodinated contrast materials or any surgical procedures necessitating restricted intake of food and fluids.
Vitamin B 12 deficiency:
Metformin may lower vitamin B 12 levels. Monitor hematologic parameters annually.
Macrovascular outcomes:
No conclusive evidence of macrovascular risk reduction with metformin hydrochloride or any other antidiabetic drug

Side Effects of Glycomet Tablets (Generic Metformin Hydrochloride)

The incidence and type of side effects reported by greater than 5% of patients for the combined Metformin Hydrochloride group versus placebo group are hypoglycemia, diarrhea, and nausea.

Metformin Hydrochloride During Pregnancy

Metformin Hydrochloride has been classified by the US FDA as Pregnancy Category B.
Metformin was not teratogenic in rats and rabbits at doses up to 600 mg/kg/day, which represent 3 and 6 times the maximum recommended human daily dose of 2,000 mg based on body surface area comparison for rats and rabbits, respectively. However, because animal reproduction studies are not always predictive of human response, Metformin HCl should not be used during pregnancy unless clearly needed.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.

Metformin Hydrochloride Nursing Considerations

Studies in lactating rats show that Metformin is excreted into milk and reaches levels comparable to those in plasma. Similar studies have not been conducted in nursing mothers. Thus, the potential for hypoglycemia in nursing infants after Metformin HCl may exist.
Do not use this medication without telling your doctor if you are breast-feeding a baby.
