Plagril (Generic Clopidogrel 75 mg tablets) - Product Information

Plagril (Generic Clopidogrel tablets) is used alone or together with Aspirin to lessen the chance of a heart attack or stroke.
It is given to patients with acute coronary syndrome (ACS), to patients who have already had a heart attack, severe chest pain, or a stroke, or to patients with other circulation problems (eg, peripheral arterial disease) that could cause a stroke or heart attack. Plagril Tablets contain Clopidogrel Bisulfate as the active ingredient, which belongs to a class of medications known as P2Y12 inhibitors.

Active Pharmaceutical Ingredient

The active pharmaceutical ingredient in Plagril Tablets is Clopidogrel Bisulfate. Each film-coated Plagril tablet contains Clopidogrel Bisulfate equivalent to 75 mg of Clopidogrel.

Uses of Plagril (Generic Clopidogrel Bisulfate tablets)

Acute Coronary Syndrome (ACS)
Clopidogrel is used to reduce the rate of myocardial infarction (MI) and stroke in patients with non-ST-segment elevation ACS [unstable angina (UA)/non-ST-elevation myocardial infarction (NSTEMI)], including patients who are to be managed medically and those who are to be managed with coronary revascularization. Clopidogrel should be administered in conjunction with aspirin.
Clopidogrel is usew to reduce the rate of myocardial infarction and stroke in patients with acute ST-elevation myocardial infarction (STEMI) who are to be managed medically. Clopdiogrel should be administered in conjunction with aspirin.
Recent MI, Recent Stroke, or Established Peripheral Arterial Disease
In patients with established peripheral arterial disease or with a history of recent myocardial infarction (MI) or recent stroke clopdigorel is indicated to reduce the rate of MI and stroke.

Generic Clopidogrel Bisulfate tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Plagril tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Acute Coronary Syndrome
In patients who need an antiplatelet effect within hours, initiate Clopidogrel with a single 300 mg oral loading dose and then continue at 75 mg once daily. Initiating clopidogrel without a loading dose will delay establishment of an antiplatelet effect by several days.
Recent MI, Recent Stroke, or Established Peripheral Arterial Disease
75 mg once daily orally without a loading dose
Plagril tablets work effectively if the amount of Clopidogrel is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Plagril tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Clopidogrel Bisulfate tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Plagril tablet dose on time, take it whenever you remember.

Clopidogrel Bisulfate Mechanism of Action – How Does Plagril Work?

Clopidogrel is an inhibitor of platelet activation and aggregation through the irreversible binding of its active metabolite to the P2Y12 class of ADP receptors on platelets.
A heart attack or stroke may occur when a blood vessel is blocked by a blood clot. Clopidogrel is a platelet inhibitor. It reduces the chance that a harmful blood clot will form by preventing platelets from clumping together in the blood. Clopidogrel may also increase the chance of serious bleeding in some people.

Contraindications

Warnings and Precautions to be taken when using Plagril Tablets

Medical History
Before beginning the treatment with Plagril Tablets (Generic Clopidogrel Bisulfate tablets), discuss your personal and family medical history with your doctor. Inform your doctor if you have a bleeding or blood clotting disorder, such as TTP (thrombocytopenic purpura) or hemophilia; a history of stroke, including TIA ("mini-stroke"); a stomach ulcer or ulcerative colitis; or kidney disease; recent surgery, serious injury/trauma, liver disease, bleeding disease (such as hemophilia).
Bleeding and haematological disorders
Due to the risk of bleeding and haematological adverse reactions, blood cell count determination and/or other appropriate testing should be promptly considered whenever clinical symptoms suggestive of bleeding arise during the course of treatment
Thrombotic Thrombocytopenic Purpura (TTP)
Thrombotic Thrombocytopenic Purpura (TTP) has been reported very rarely following the use of clopidogrel, sometimes after a short exposure. It is characterised by thrombocytopenia and microangiopathic haemolytic anaemia associated with either neurological findings, renal dysfunction or fever. TTP is a potentially fatal condition requiring prompt treatment including plasmapheresis.
Acquired haemophilia
Acquired haemophilia has been reported following use of Clopidogrel. In cases of confirmed isolated activated Partial Thromboplastin Time (aPTT) prolongation with or without bleeding, acquired haemophilia should be considered. Patients with a confirmed diagnosis of acquired haemophilia should be managed and treated by specialists, and Clopidogrel should be discontinued.
Bleeding Time:
Plagril prolongs the bleeding time and therefore should be used with caution in patients who may be at risk of increased bleeding from trauma, surgery, or other pathological conditions (particularly gastrointestinal and intraocular). If a patient is to undergo elective surgery and an antiplatelet effect is not desired, Plagril should be discontinued 5 days prior to surgery.
Use in Hepatically-Impaired Patients:
Experience is limited in patients with severe hepatic disease, who may have bleeding diatheses. PLAVIX should be used with caution in this population.
Use in Renally-Impaired Patients:
Experience is limited in patients with severe renal impairment. Plagril should be used with caution in this population.
Live Vaccinations, Surgery and Regular Medical Tests
If you plan to have live vaccinations, surgery or any dental procedure, inform your doctor, anesthetist, or dentist about your course of Clopidogrel tablets. In case of an emergency medical treatment, please inform the practitioner about your course of Clopidogrel Bisulfate tablets and the details of your last dose.
Clopidogrel Bisulfate Tablets – Driving and Other Activities
Clopidogrel is unlikely to affect your ability to drive. Clopidogrel tablets may make some people feel dizzy, light-headed or impair their vision. Avoid activities such as driving, using machinery, or performing any activity that requires complete vigilance or perfect vision until you are sure about your safety.
Clopidogrel Bisulfate and Alcohol
Avoid or limit the consumption of alcohol during the treatment with Generic Clopidogrel Bisulfate tablets (Plagril Tablets) because it may increase the risk of certain side effects.

Clopidogrel Bisulfate Interactions

Before beginning the oral therapy of Clopidogrel tablets, please tell your doctor about all your present medications including prescription drugs, over the counter medications, mineral supplements, herbal products, and vitamins. Plagril Tablets are known to have drug interactions with certain other medications. Please inform your doctor if you are taking aspirin, ibuprofen (Motrin, Advil, Nuprin, others), naproxen (Aleve, Anaprox, Naprosyn, others), ketoprofen (Orudis, Orudis KT, Oruvail), indomethacin (Indocin), nabumetone (Relafen), oxaprozin (Daypro), or other non-steroidal anti-inflammatory medications (NSAIDs); abciximab, eptifibatide, tirofiban; dalteparin, enoxaparin, tinzaparin; fondaparinux, ticlopidine; heparin, warfarin, Coumadin; or urokinase (Abbokinase); armodafinil or modafinil; fluoxetine or fluvoxamine; gemfibrozil; isoniazid; a cancer medication--dasatinib, letrozole, ibritumomab, or tositumomab; certain stomach acid reducers--cimetidine, esomeprazole, omeprazole; antifungal medication--fluconazole, ketoconazole, voriconazole; HIV medication--delavirdine, efavirenz, etravirine, tipranavir; or seizure medication such as felbamate (Felbatol) or oxcarbazepine (Trileptal).

Overdose

Platelet inhibition by Clopidogrel is irreversible and will last for the life of the platelet. Overdose following clopidogrel administration may result in bleeding complications. A single oral dose of Clopidogrel at 1500 or 2000 mg/kg was lethal to mice and to rats and at 3000 mg/kg to baboons.
Symptoms of acute toxicity were vomiting, prostration, difficult breathing, and gastrointestinal hemorrhage in animals. Based on biological plausibility, platelet transfusion may restore clotting ability.

Clopidogrel Bisulfate Side Effects

The most commonly reported Clopidogrel Bisulfate side effect is Bleeding, including life-threatening and fatal bleeding. The following side effects have been reported during the course of treatment with Clopidogrel Bisulfate tablets (Plagril tablets):

  • Central Nervous System- severe headache, confusion, or dizziness
  • Cardiovascular System- chest pain or heavy feeling, pain spreading to the arm or shoulder, nausea, sweating, general ill feeling, fast heartbeat
  • ENT- bleeding nose
  • Gastrointestinal System- diarrhea, bloody (or black) vomit or stools, upset stomach, diarrhea, constipation
  • Genitourinary System- blood in urine
  • Musculoskeletal System- sudden numbness or weakness, especially on one side of the body
  • Hypersensitivity- hives; difficulty breathing; swelling of your face, lips, tongue, throat, hands, feet; tightness in the chest; redness; rash; itching
  • General- unexplained bruising or bleeding

Plagril Tablets during Pregnancy and Breastfeeding

Clopidogrel Bisulfate tablets has been classified by the US FDA as Pregnancy Category B .
Available data from cases reported in published literature and postmarketing surveillance with clopidogrel use in pregnant women have not identified any drug-associated risks for major birth defects or miscarriage.There are risks to the pregnant woman and fetus associated with myocardial infarction and stroke. No evidence of fetotoxicity was observed when clopidogrel was administered to pregnant rats and rabbits during organogenesis at doses corresponding to 65 and 78 times the recommended daily human dose.
Do not use this medicine without your doctor's consent in case you are pregnant. Stop using this medication and inform your doctor immediately if you become pregnant during treatment.
It is not known if Clopidogrel Bisulfate is transmitted through breast milk and if it may harm the nursing baby. Studies in rats have shown that Clopidogrel and/or its metabolites are present in the milk. No adverse effects on breastfed infants have been observed with maternal Clopidogrel use during lactation in a small number of postmarketing cases. Pregnant women and nursing mothers should consult their doctor before using Clopidogrel tablets for the treatment of their medical condition.
