It is also used to treat Major depression and Cataplexy associated with narcolepsy.
Clomipramine may also be used for other conditions as determined by your healthcare provider.

Uses of Clomipramine Hydrochloride Tablets

Depression in Adults

Clomipramine therapy should be initiated at daily doses of 25 mg. Dosage may be increased by 25 mg increments, as tolerated, at 3 to 4 day intervals up to a total daily dose of 150 mg by the end of 2 weeks. Thereafter, the dose may be gradually increased over a period of several weeks to 200 mg.
Doses in excess of 200 mg daily are not recommended for outpatients.
Dosage during maintenance therapy should be kept at the lowest effective level. To minimize daytime sedation during maintenance treatment, the total daily dosage may be given as a single dose at bedtime.
Obsessive Compulsive Disorders in Adults
Clomipramine therapy in adult Obsessive Compulsive patients should be initiated at daily doses of 25 mg.
Dosage may be increased by 25 mg increments, as tolerated, at 3 to 4 day intervals up to a total daily dose of 100 or 150 mg by the end of 2 weeks. Thereafter, the dose may be gradually increased over a period of several weeks to 200 mg.
Doses in excess of 200 mg per day are not generally recommended for outpatients. However, in the treatment of severe cases of Obsessive Compulsive Disorder, daily doses of up to 250 mg may be required.Dosage adjustments may be made during maintenance therapy with the objective of maintaining the patient at the lowest effective dose.
Obsessive Compulsive Disorders in Children and Adolescents
Limited information is available for the use of Clomipramine tablets In children aged 10 to 17 years. For this age group, an initial dose of 25 mg per day is recommended. Dosage may be increased by 25 mg increments, as tolerated, at 3 to 4 day intervals.
By the end of 2 weeks, patients may be titrated up to 100-150 mg per day or 3 mg/kg, whichever is lower. Thereafter, the dose may be gradually increased to 200 mg or 3 mg/kg whichever is lower. A total daily dose above 200 mg should not be used in children or adolescents.
Dosage adjustments may be made during maintenance therapy with the objective of maintaining the patient at the lowest effective dose.
Clofranil tablets work effectively if the amount of Clomipramine is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose

If you forget to take your regular Clofranil or Clonil tablet dose on time, take it whenever you remember.

Clomipramine Hydrochloride Tablets Mechanism of Action

Clomipramine belongs to a group of medicines called tricyclic antidepressants (TCAs). It works by increasing the amount of natural chemicals called norepinephrine and serotonin in the brain.
Like other tricyclics, Clomipramine inhibits norepinephrine and serotonin uptake into central nerve terminals, possibly by blocking the membrane-pump of neurons. Clomipramine thereby increases the concentration of transmitter monoamines at receptor sites.
Clomipramine is presumed to influence depression and obsessive and compulsive behavior through its effects on serotonergic neurotransmission. The actual neurochemical mechanism is unknown, but Clomipramine’s capacity to inhibit serotonin reuptake is thought to be important.
Clomipramine appears to have a mild sedative effect that may be helpful in alleviating the anxiety component often accompanying depression

Store Clofranil tablets and Clonil tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.

Contraindications

Clofranil tablets and Clonil tablets are contraindicated in patients with a hypersensitivity to Clomipramine Hydrochloride, other tricyclic antidepressants or any inactive ingredient present in tablets.

Overdose

Signs and symptoms generally appear within 4 hours of ingestion and reach maximum severity after 24 hours. Owing to delayed absorption (increased anticholinergic effect due to overdose), long half-life and enterohepatic recycling of the drug, the patient may be at risk for up to 4 to 6 days.
Signs and symptoms may include drowsiness, stupor, ataxia, vomiting, cyanosis, restlessness, agitation, delirium, severe perspiration, hyperactive reflexes, muscle rigidity, athetoid and choreiform movements, and convulsions. Hyperpyrexia, mydriasis, bowel and bladder paralysis, and respiratory depression may occur.
No specific antidote is available and treatment is essentially symptomatic and supportive.
Gastric lavage or aspiration should be performed promptly and is recommended up to 12 hours or even more after the overdose, since the anticholinergic effect of the drug may delay gastric emptying. Administration of activated charcoal may help to reduce absorption of the drug.

Warnings and Precautions

Clinical Worsening and Suicide Risk Patients with major depressive disorder (MDD), both adult and pediatric, may experience worsening of their depression and/or the emergence of suicidal ideation and behavior (suicidality) or unusual changes in behavior, whether or not they are taking antidepressant medications, and this risk may persist until significant remission occurs.
Screening Patients for Bipolar Disorder
However, prior to initiating treatment with an antidepressant, patients with depressive symptoms should be adequately screened to determine if they are at risk for bipolar disorder; such screening should include a detailed psychiatric history, including a family history of suicide, bipolar disorder, and depression. It should be noted that Clomipramine hydrochloride is not approved for use in treating bipolar depression.
Serotonin Syndrome
The development of a potentially life-threatening serotonin syndrome has been reported with SNRIs and SSRIs, including Clomipramine hydrochloride, alone but particularly with concomitant use of other serotonergic drugs (including triptans, tricyclic antidepressants, fentanyl, lithium, tramadol, tryptophan, buspirone, and St. John's Wort) and with drugs that impair metabolism of serotonin
Angle-Closure Glaucoma
The pupillary dilation that occurs following use of many antidepressant drugs including Clomipramine hydrochloride may trigger an angle closure attack in a patient with anatomically narrow angles who does not have a patent iridectomy.
Seizures
During premarket evaluation, seizure was identified as the most significant risk of Clomipramine hydrochloride use.
DRESS
Rare cases of drug rash with eosinophilia and systemic symptoms (DRESS) have been reported with the use of Clomipramine. In the event of severe acute reactions such as DRESS, discontinue Clomipramine therapy immediately and institute appropriate treatment.

No teratogenic effects were observed in studies performed in rats and mice at doses up to 100 mg/kg, which is 24 times the maximum recommended human daily dose (MRHD) on a mg/kg basis and 4 times (rats) and 2 times (mice) the MRHD on a mg/m2 basis.
There are no adequate or well-controlled studies in pregnant women. Withdrawal symptoms, including jitteriness, tremor, and seizures, have been reported in neonates whose mothers had taken Anafranil until delivery.
