The active ingredient present in Sulpitac tablets is Amisulpride. Each uncoated Sulpitac tablet contain 50 mg, 100 mg, 200 mg or 400 mg of Amisulpride.
Uses of Amisulpride Tablets (Sulpitac Tablets)
Amisulpride tablets (Sulpitac tablets) is indicated for the treatment of acute and chronic schizophrenic disorders with:
- positive symptoms (such as delusions, hallucinations, thought disorders, hostility, suspiciousness), and/or
- negative symptoms (deficit syndrome) such as blunted affect, emotional and social withdrawal
This includes patients with predominant negative symptoms.
Sulpitac Tablets are used alone or with other medicines to prevent and treat nausea and vomiting that may occur after surgery. This medicine works in the stomach to block the signals to the brain that cause nausea and vomiting.
Sulpitac Tablets - Dosage
Generic Amisulpride tablets should be administered exactly as suggested by your doctor. Your doctor may recommend a dosage of Sulpitac tablets depending on your age, medical requirement, body weight, other medical conditions, and current medications.
Sulpitac tablets work effectively if the amount of Amisulpride is maintained at a constant level in the body. You should be systematic about the daily schedule for the medication such that you take Sulpitac tablets at about the same time every day, to get the maximum benefit of the medication. Continue taking Amisulpride tablets without missing any doses for the recommended treatment period and do not stop the treatment suddenly even if you feel better.

Missed Dose
If you forget to take your regular Sulpitac tablet dose on time, take it whenever you remember.
If you remember the next day, skip the missed dose and take your scheduled dose. Do not take a double dose to compensate the missed dose. Positive symptoms:
For acute psychotic episodes, a daily dose between 400 mg and 800 mg is recommended.
In individual cases, the daily dose may be increased up to 1200 mg. Doses above 1200 mg/day have not been extensively evaluated for safety and therefore should not be used.
No specific titration is required when initiating treatment. Doses should be adjusted according to individual response. For patients with mixed positive and negative symptoms, doses should be adjusted to obtain optimal control of positive symptoms.
Maintenance treatment should be established individually with the minimally effective dose.
Predominant negative symptoms (deficit syndrome)
A daily dose between 50 mg and 300 mg is recommended. Doses should be adjusted individually.
Amisulpride can be administered once daily at doses up to 300 mg, higher doses should be administered twice daily.
The minimum effective dose should be used.
Amisulpride tablets Mechanism of Action – How Does Sulpitac Work?
Amisulpride binds selectively to dopamine D(2) and D(3) receptors in the limbic system.
Low doses of amisulpride preferentially block presynaptic D(2)/D(3)-dopamine autoreceptors, thereby enhancing dopaminergic transmission, whereas higher doses block postsynaptic receptors, thus inhibiting dopaminergic hyperactivity.
Storage Instructions for Sulpitac Tablets
Store Sulpitac tablets at controlled room temperature (20°C to 25°C),protected from heat, moisture, and sunlight. Excursions are permitted from 15°C to 30°C.
Retain the original packing of the medication and do not consume past its expiration date printed on the blister pack. Keep the pack of Sulpitac tablets away from the reach of children and pets. Do not dispose Sulpitac tablets along with domestic waste or sewage. Consult your doctor or pharmacist about the correct method of disposal.
Contraindications
Sulpitac tablets are contraindicated in patients with a hypersensitivity to or any inactive ingredient present in tablets. Sulpitac tablets are also contraindicated in the following cases:
- Concomitant prolactin-dependent tumours e.g. pituitary gland prolactinomas and breast cancer.
- Phaeochromocytoma.
- Children up to puberty.
- Lactation.
- In combination with the following medication which could induce torsades de pointes: Class Ia antiarrhythmic agents such as quinidine and disopyramide, Class III antiarrhythmic agents such as amiodarone and sotalol, Other medications such as bepridil, cisapride, sultopride, thioridazine, methadone, intravenous erythromycin, intravenous vincamine, halofantrine, pentamidine, sparfloxacin.
- Levodopa; reciprocal antagonism between levodopa and neuroleptics
- In hepatic impairment, amisulpride may be contraindicated to avoid the possible risk of adverse events due to an influence of the disease on amisulpride metabolis
Overdose
Experience with Amisulpride Tablets (Sulpitac Tablets) in overdosage is limited. Exaggeration of the known pharmacological effects of the drug have been reported. These include drowsiness and sedation, coma, hypotension and extrapyramidal symptoms. Fatal outcomes have been reported mainly in combination with other psychotropic agents.
Warnings and Precautions
QT Prolongation:
Causes dose- and concentration-dependent QT interval prolongation. Avoid use in patients with congenital long QT syndrome and patients taking droperidol.
ECG monitoring recommended in patients with pre-existing arrhythmias or cardiac conduction disorders, electrolyte abnormalities (e.g., hypokalemia or hypomagnesemia), congestive heart failure, and in patients taking other QT-prolonging drugs (e.g., ondansetron) or with other medical conditions known to prolong the QT interval.
Neuroleptic Malignant Syndrome (NMS)
Neuroleptic Malignant Syndrome (NMS) is a potentially fatal syndrome that has been reported in association with antipsychotic medicines, including amisulpride. Neuroleptic malignant syndrome is characterised by hyperthermia, muscle rigidity, autonomic instability, and elevated CPK may occur. In the event of any symptoms which could suggest NMS, in particular hyperthermia, particularly with high daily doses, all antipsychotic medicines including amisulpride should be discontinued.
Rhabdomyolysis
Rhabdomyolysis has also been observed in patients without NMS.
Seizure Threshold:
Amisulpride can lower the seizure threshold. Therefore patients with a history of seizures should be closely monitored during amisulpride therapy.
Withdrawal Symptoms
Withdrawal symptoms have been described after abrupt cessation of high therapeutic doses of antipsychotic medicines. The emergence of involuntary movement disorders (such as akathisia, dystonia and dyskinesia) has been reported with amisulpride. Therefore, gradual withdrawal of amisulpride is advisable.
Leukopenia, Neutropenia and Agranulocytosis:
Leukopenia, neutropenia and agranulocytosis have been reported with antipsychotics, including amisulpride. Unexplained infections or fever may be evidence of blood dyscrasia and requires immediate haematological investigation.
Patients With Parkinson's Disease:
Caution should be also exercised when prescribing amisulpride to patients with Parkinson's disease since it may cause worsening of the disease. Amisulpride should be used only if neuroleptic treatment cannot be avoided.
Increase In Plasma Prolactin Levels:
Amisulpride causes an increase in plasma prolactin levels which is reversible after discontinuation of the medicine. This may result in galactorrhoea, amenorrhoea, gynaecomastia, breast pain, orgasmic dysfunction and impotence.
Acute Dystonia:
Acute dystonia (spasm torticollis, oculogyric crisis, trismus) may appear. This is reversible without discontinuation of amisulpride upon treatment with an anti-parkinsonian agent.
Extrapyramidal symptoms may occur:
tremor, rigidity, hypokinesia, hypersalivation, akathisia. These symptoms are generally mild at optimal dosages and partially reversible without discontinuation of amisulpride upon administration of anti-parkinsonian medication.
Hyperglycaemia and Diabetes Mellitus:
Hyperglycaemia has been reported in patients treated with some atypical antipsychotic agents, including amisulpride.
Side Effects of
The very common Amisulpride tablets (Sulpitac) side effects with an incidencr rate of 10% or more includes extrapyramidal symptoms.

There was no evidence of teratogenicity in embryofoetal development studies in mice and rabbits following oral doses vehicle of up to 2 (mice) and 4 (rabbits) times the maximum recommended human dose based on body surface area, administered daily during the period of organogenesis.
